        근막 결손의 복원과 자궁천골인대 고정을 통한 전자궁적출술 후 질탈출의 성공적 교정

        목적 : 본 연구는 전자궁적출술 후 질탈출을 부위-특이적인 결함의 개념에 입각하여 치골경부 근막과 직장질 격막의 연결을 통한 골반내 근막의 복원과 자궁천골인대 고정술을 이용한 질탈출의 성공적인 수술적 교정의 방법과 결과를 보고하고자 한다. 연구 방법 : 1999년부터 2001년 8월까지 자궁적출술 후 질궁탈출 증상으로 문화병원 산부인과를 내원, 탈출된 질의 근막을 재건시킨 다음, 이를 자궁천골인대에 고정시킴으로써 질의 부위-특이적인 교정술을 받은 3명의 환자를 대상으로 하였다. 3명중 1명은 개복술을 하였고 2명은 내시경 수술을 하였다. 결과 : 환자의 연령 분포는 49-67세였으며, 수술시간은 평균 113분 (105-125)이었다. 세 환자 모두 수술 후 질의 상태가 해부학 및 기능적으로 정상이었으며, 9일째 특이사항 없이 퇴원하였다. 수술 후 HRT를 시행한 1명을 포함한 3명 모두 각각 24개월, 12개월, 7개월간의 추적 검사에서도 특이한 합병증이나 재발 없이 좋은 경과를 보였다. 결론 : 골반 근막 결손의 복원과 자궁천골인대 고정술은 전자궁적출술 후 질탈출의 교정에 매우 안전하고 효과적이며, 기능적인 방법으로 사료되며, 자궁천골인대를 질탈출 교정에 이용한 연구는 본 연구가 국내에서 처음이다. Objective : To report the method and efficacy of the site specific-defects repair in the endopelvic fascia and uterosacral ligament suspension for the correction of posthysterectomy vaginal vault prolapse. Methods : This study was performed in 3 patients with posthyterectomy vaginal vault prolapse who underwent the surgery for the reconstruction of endopelvic fascia and the correction of vault prolapsed vagina by using uterosacral ligament suspension at the department of Obstetrics and Gynecology in Moon-Hwa Hospital. Among them, 1 patient was treated by laparotomy, and 2 patients were treated by laparoscope. Results : The age of patients ranged from 49 to 67 years. The mean operation time was 111 minutes (105-120 minutes). The status of postoperative vagina was normal in all 3 patients, who were discharged 9 days after operation without symptoms. No complication or recurrence were found in them despite long-term follow-up for 24 months, 12 months, 7 months, respectively after operation. One patient has been treated with HRT therapy. Conclusion : The reconstruction of the defect of endopelvic fascia and uterosacral ligament fixation may be very safe, effective, and functional in the correction of vaginal vault prolapse. This study reports the treatment of vaginal vault prolapse using uterosacral ligament within Korea for the first time.

        조선시대 충정관의 보존처리

        강원도 원성군 호저면에서 출토된 조선시대 모자(충정관)를 보존처리하였다. 모자는 양모 펠트 직물로 구성되어 있었으며, 견섬유의 끈으로 장식되어 있었다. 처리 전의 모자는 심하게 오염되어 있었고 좀벌레에 의해 손상된 상태였다. 유물의 처리과정은 부드러운 붓과 핀셋으로 고형오염물을 제거한 후 유물을 지지 할 수 있는 스테인레스 망을 이용하여 퍼클로로에틸렌으로 2차례에 걸쳐 건식세탁하였다. 여분의 용매는 압지법을 사용하여 제거하였다. 세탁 후 유물의 감량률은 7.99%, 세탁 전후 유물의 색차는 2.57을 나타냄으로써 유물에 부착되어 있던 각종 오염물들이 탈락되었으며, 모자의 외관이 깨끗해지고 밝아졌다. 세탁 후 견봉사를 이용하여 장식용 끈들을 제자리에 고정시키고 모자의 모양을 잡아주는 보수 작업을 실행하였다. The hat excavated from the tomb of Hojeo-myon Wonsung-gun in Kangwon province was dry-cleaned and restored. The hat was made of wool felt and decorated with several strands of silk cord. It was very dirty and moth-eaten. After getting rid of the particle soils on a surface with painting brush, the hat was dry-cleaned using perchloroethylene applying the minimum force, and blotting paper was used for drying. During cleaning, it was supported by the stainless steel net. The weight loss was 7.94% after cleaning. The soils sticked on the hat was removed clearly. The texture of the hat became softer after cleaning. Color difference(∆E) was 2.57, which is a noticeable change. The color became clear and changed to the red and yellow range. After dry-cleaning, the hat was repaired. Dislocated silk cords were fixed to the right place by stitching with silk yarn. To keep the shape of the hat, it should be supported by a hat-shaped stiff net during exibition and storing.

        기업회생관점에서 본 EU국가의 파산제도 비교

        본고는 EU 주요국의 파산제도를, 첫째, 비효율적인 기업의 청산 및 자금난에 빠진 유망기업의 구제, 둘째, 중소기업의 이용용이성이란 측면에서 비교분석한다. EU 국가들은 80년대 이래로 기업회생의 기회를 강화하고 중소기업에 대한 특별 고려를 하는 방향으로 수차례에 걸쳐 파산제도를 개정하여 왔다. 그러나 그럼에도 불구하고 미국의 11장에 비해 EU 국가의 파산제도는 기업회생에 매우 까다로운 편인 것으로 나타난다. EU 국가중에서는 최근까지 독일이 특히 기업회생에 가장 어려운 제도를 가지고 있었던 것으로 나타난다. 그러나 1999년의 새로운 법안에 담보채권에 대한 자동유예제가 도입된 이후 독일의 파산제도도 기업회생에 약간 관대해 졌다. 영국은 담보권자가 언제든지 기업을 청산할 수 있다는 점에서 1980년대 중반의 파산법개정에도 불구하고 상대적으로 채권자에 제일 유리한 제도를 가지고 있는 국가로 보인다. 한편 프랑스는 독일과 영국에 비해 기업회생에 매우 유리한 파산제도를 갖고 있다. 특히 프랑스는 중소기업을 위한 간이파산절차를 갖고 있다. 그러나 고용유지에 대한 지나친 강조가 조속한 구조조정을 어렵게 만듦으로써 실질적으로 회생되는 기업은 극소수에 불과하다. 이러한 결과를 우리 나라에 적용할 때 대기업에 대해서는 상당히 관대한 회생절차가 마련되어 있는 데 반해 중소기업의 회생을 위한 제도는 크게 미흡한 것으로 보인다. This paper examines European bankruptcy codes and provides a comparison to US law and between them on the basis of two criteria. The first criterion is the existence of a framework permitting viable but financially distressed firms to reorganize and continue doing business while nonviable firms can be liquidated, The second one is the practical possibility for small and medium sized firms to resort to a formal bankruptcy process. Since the 1980s, many European countries have been reforming their bankruptcy procedures to provide more incentives for saving financially distressed but viable firms. Nevertheless, compared to the Chapter 11, which allows the most flexibility for firms, European bankruptcy laws remain unsuccessful in these efforts. Of the European countries, German law was the least effective in re-organizing firms because of its over-emphasis on the protection of creditors' rights. However, new German law introduces a three-month automatic stay and thereby enhances efficiency in the reorganization process. The UK, despite its reform of bankruptcy laws in 1985, has the most difficult law for saving distressed firms because the individualistic behavior of the secured creditors can lead economically viable firms into premature liquidation. Compared to Germany and the UK, France has gone the furtherest toward providing a framework for resuscitating distressed firms. Moreover, it has a simple bankruptcy procedure for small and medium sized firms. However, by forcing firms to maintain employment and creditor contracts, French law can prevents firms from becoming viable. It can be inferred from this comparative analysis that Korean bankruptcy law allows too much room for large corporations to be saved while forcing small and medium sized firms to be liquidated.

        疾病豫防 및 健康曾進 綜合 對策

        While the threat from traditional communicable diseases have been decreasing non communicable chronic diseases are increasing due to the aging of population and change in life pattern of the people such as over intake of cholesterol and lack of physical exercise etc. On the other hand, since 1980s, AIDS is spreading rapidly throughout the globe and environmental pollution, accidents, addictive diseases such as drug abuse and alcoholism are becoming serious factors to hinder the health promotion of the people. In order to improve general public health and promote individual health status, existing program for communicable disease control by the government such as tuberculosis leprosy, STD and acute communicable diseases should be effectively continued. In principle, effort should be placed on eradication of source of infection, reduction of communicability of source in infection, treatment of source of infection as well as increase of individual registance to the diseases through immunization and improvement of physical status Since the pattern of illness is being shifted from communicable diseases to non communicable chronic diseases such as cancer, cerebral hemorrhage, heart disease and hypertention etc., special emphasis should to placed on the prevention and control of those adult diseases. Early detection of the patients, registration and treatment of patients and health education should be systematically developed for effective control of the diseases. In addition, program activities on MCH, nutrition, dentistry, mental health environmental health, accident prevention, medial delivery system, health insurance, and all other health issues should be improved in order to achieve our goal of health promotion.

        응급진료센터 과밀화와 특별관찰구역의 운영

        Overcrowding of emergency departments is a growing problem. Delays in admitting patients to inpatient units have been reported as a contributing factor to overcrowding. Overcrowding in ED results in many patients not receiving needed care and impairs the ability of the ED provider to meet the needs of those patients that are treated. The effect of the critically ill patients on the overcrowded emergency department make emergency physicians frustrated. So we created the 'special unit for observation' of critically ill patients and managed them continuously and closely. The special unit for observation have 4 physical beds and monitors and 2 ventilators. One nurse and senior ED residents were asigned to the management of the patients in the special unit. 160 patients(3.4% of all ED visits) admitted to special unit during the period of March 1, 1996 to May 31, 1996. we could continue assessment and management of critically ill patients without psychologic strain or burnout generated by overcrowding. We think that special unit for critically ill patients is a very effective alternative to overcrowded ED.

      • 균열을 내포하는 기계구조물의 파괴강도 및 피로수명설계 자동화를 위한 3차원 수치 모델링 기법 개발

        본 논문에서는 최근 각종대형사고의 대책기술이 되고 있는 기계구조물의 안전설계 및 파괴 관리 제어 기술의 핵심부분으로서, 기계구조물에 필연적으로 존재하게 되는 임의의 평면 크랙 또는 표면 크랙등 다양한 결함이 되고 있는 파괴강도 평가 및 피로 수명설계 자동화를 위한 3차원 수치 모델링 기법을 유한요소법과 선형파괴역학을 이용하여 개발ㆍ제시하였다.개발된 기법은 공학상의 실제 문제인 기계구조물내 다양한 형태의 결함에 대한 해석에 많은 잇점이 있었으며, 기존의 해석방법보다 정확하고 간편함을 보였다. This paper describes a versatile finite element technique which has been used to investigate the fatigue life and fracture strength of a wide range of structural defects of practical importance.The procedure automatically remeshes the three-dimensional finite element model during the stages of crack growth for the design automation of the fatigue life and fracture strength. Problems analyzed to date include the subsurface cracks in leak-before-break situations, the development of quarter-elliptical corner defects, the interaction and coalescence of five adjacent coplanar semi-elliptical surface defects and the fatigue life and fracture strength according to the fatigue growth of a defect in machine structure.

        On the synthesis of 2.4­Dichloro­5­fluoroacetophenone, the reaction of 2.4­dichlorofluoro benzene with acetic anhydride gives rise to 2.4­dichloro­5­fluoroacetophenone of the main product, and to 2.6­dichloro­3­fluoroacetophenone of the side product. But, the reaction of 3­chloro­4fluoroiodobenzene with acetic anhydride gives rise to only 2­Iodo­4­chloro­5­fluoroacetophenone. S_N2 Reaction of 2.4­dichloro­5­fluoroacetophenone with cyclopropylamine gives rise to 1:3.5 ratio 2 position and 4­position isomers substituted with cyclopropylamine, but the reaction of 2­Iodo­4­chloro­5­fluoroacetophenone with cyclopropylamine gives rise to only 4­position compound substituted with cyclopropylamine.

        The moving behaviour of anchovy schools (There were small schools of sardine and round-herring in these anchovy schools) in the hauling process of a large scale set-net was investigated using a scanning sonar. in the Kishihata set-net fishing ground located in Nanao city Ishigawa prefecture , Japan from Dec 1st to Dec 4th. 1992. The moving behaviour of anchovy schools in the hauling process a large scale set-net made the term of moving narrowed from the front of the first funnel-net to the second funnel-net as the hauling proceeded. We found that when the ship of picking up the net approched to the second funnel-net, the anchovy school entered in the second bag-net fast hard. The moving speed before hauling and during the hauling process was most of 50cm/sec and below and there was no difference in the moving speed.

        In this paper, we attempt to control the nonlinear power control system for resistance spot welders to be a fully linearized system by applying the recently developed nonlinear feedback linearization techniques based on differential geometry theories. The controller in this paper is computationally simple. In additionm the easy gain tuning as well as the high dunamic performance of resistance spot welding systems can be obtained. To illumunate the performance of our controller further we present some simulation results.

      • 이온빔에 의해 제조한 TiN 박막의 기계적 성질

        Hard coating on cutting tools have become an imteresting research area in mechanical engineering. Thin film of titanium nitride(TiN) has beem used to extend tool life using its high hardness, wear resistance and high melting point. This film material can be prepared usually by CVD or PVD, but CVD involves high temperature processing and low deposition rate, Dual ion beam(DIM) deposition, a simultaneous process of sputter deposition and ion beam assist, is a movel technique for achieving excellent film properties with enhanced adhesion to the substrate. TiN films hace been prepared on the WC insert tip using DIB deposition techniqe. The effects of processing parameters such as N/Ti arrival ratio and ion beam crrent to the crystal structures and mechanical properties of the films were investigated. XRD analysis indicated that the crystalline structure of the TiN films was changed from amorphous phase forr N/Ti ratio of 0.5 to TiN(200) for high N/Tiratio of 1.0. The lowest wear rate of the film was obtained with N/Ti ration of 1.25 and ion beam current of 0.6 mA whih showed both high hardness and good adhesion to the substrate.
