Sequential chemotherapy and radiotherapy as sandwich therapy for the treatment of high risk endometrial cancer

        Lisa N. Abaid,Mark A. Rettenmaier,John V. Brown III,John P. Micha,Alberto A. Mendivil,Marie A. Wabe,Bram H. Goldstein 대한부인종양학회 2012 Journal of Gynecologic Oncology Vol.23 No.1

Objective: The purpose of this retrospective study was to assess the tolerability and efficacy of sequential chemotherapy and radiotherapy for the treatment of high risk endometrial cancer. Methods: We conducted a retrospective study of previously untreated high risk endometrial cancer patients who received sequential chemotherapy and radiotherapy in accordance with the sandwich approach from June 2008 until June 2011. High risk endometrial cancer patients underwent complete surgical staging followed by adjuvant therapy encompassing sequential chemotherapy, radiation therapy and consolidation chemotherapy. Results: The study analysis comprised 32 endometrial cancer patients. All subjects were treated with carboplatin and paclitaxel chemotherapy; currently, 186 cycles have been administered and 94% of patients have completed the planned number of cycles. Grade 3 neutropenia developed in 1 (3.1%) patient; there was no incidence of grade 4 neutropenia. Moreover, we observed grade 3 anemia in four (12.5%) patients and grade 4 anemia in one (3.1%) patient. One (3.1%) patient developed grade 3 thrombocytopenia; grade 4 thrombocytopenia was not observed. Five patients exhibited progressive disease, three of whom have since expired; mean progression free survival and follow-up were 17.4 months and 18.9 months, respectively. Conclusion: The preliminary results from our study suggest that the sandwich approach to treating high risk endometrial cancer patients is feasible. Hematologic toxicity was well tolerated and non-hematologic toxicity was mild and easily managed. Further study of this novel regimen in a larger patient population with extended follow-up is necessary. • Post Intensive Care Syndrome in Parents of Critically Ill Pediatric Patients

        Lisa Yeo,Sun-Mi Chae 한국간호과학회 2021 한국간호과학회 학술대회 Vol.2021 No.10

Aim(s): Family members can suffer from psychological symptoms and social problems when their loved one is in the intensive care unit. The symptoms experienced by families of critically ill patients are called "post-intensive care syndrome-family." The purpose of this study was to describe the symptoms of post intensive care syndrome-family in parents of critically ill pediatric patients. This study examined (1) psychological and social problems, (2) changes in symptoms over 1 month, and (3) the differences in symptoms by the participants’ characteristics. Method(s): A longitudinal descriptive study design was used for a pediatric intensive care unit in a tertiary care hospital in Korea. Parents of children admitted to the pediatric intensive care unit for at least 48hours were questioned on the day of the child’s discharge from the pediatric intensive care unit and 1 month after discharge. Anxiety, depression, post-traumatic stress symptoms and social impacts were measured as symptoms of post intensive care syndrome-family. Result(s): Among 41 participants, 36 completed the follow-up survey. Most of the parents reported all three psychological problems, anxiety, depression, and post-traumatic stress symptoms, on the day of the child’s discharge from the pediatric intensive care unit (77.8%) and more than a half (58.3%) on 1 month after discharge. Social problems significantly increased over time (p=.04). Factors associated with symptoms of post intensive care syndrome-family were emotional support from the staff of the pediatric intensive care unit. Emotional support showed a significant negative correlation with post-traumatic stress symptom(p=.003) and social problems(p=.02). Conclusion(s): This study revealed a high prevalence of psychological symptoms and increased social problems among parents of critically ill pediatric patients after discharge from the pediatric intensive care unit. Emotional support from health care professionals can help reduce symptoms of post intensive care syndrome in parents of children in pediatric intensive care units.

AI and the Legal Profession: Ethical and Regulatory Considerations

        Lisa Webley 서울대학교 공익산업법센터 2019 경제규제와 법 Vol.12 No.2

Legal and other professional services are entering the fourth industrial age, with predictions that fundamental technological transformation will result in massive disruption to lawyers, legal services and legal systems. AI may yet not have seriously disrupted professional service markets, but with increased commodification of products and services in global law and accountancy firms, these sectors are ripe for change. This may impact the way in which the legal and financial systems operate within countries and across countries that may have profound consequences for all in society. AI raises fundamental questions about the reach, limits and appropriateness of current legislative and regulatory frameworks governing their use. There are ethical challenges and risks for professionals, businesses, consumers, governments and nation states. These powerful technologies might be employed to reduce social inequality and improve access to justice, alongside enhancing productivity and wealth. They may also lead to invasions of privacy, herald an era of greater state or business surveillance of individuals, provide a means for increased authoritarian control and/or greater societal inequality. Lawyers can, and must, play a key role in securing the rule of law, the protection of rights, good governance and fair decision-making in this new context. This paper considers the use of AI systems in legal services and the justice system. It suggests ways in which technology may be harnessed by lawyers and beyond to provide legal help to clients. It touches upon some regulatory and ethical challenges that are yet to be addressed including the powerlessness of jurisdictional boundaries in a virtual age and the powerfulness of those with access to big data to develop AI in a legal context. The paper also considers, in brief, issues of transparency of decision-making, data quality and inferences drawn from data, professional responsibility and ethics, liability, public acceptance and by implication public trust. These discussions are framed within a discussion of the likely impact that these changes may bring to our legal systems and ways in which the rule of law may be strengthened or weakened depending on approaches to regulation of AI technologies and their use.

Lisa Wiyartanti,Myon Woong Park,Choon Hak Lim,Gyu Hyun Kwon 제어로봇시스템학회 2013 제어로봇시스템학회 국제학술대회 논문집 Vol.2013 No.10

The operating room as part of the surgery department in the hospital spends significant operation cost of the whole organization, and to maximizing its occupancy rate is needed to optimize the operation cost. Thus, efficient and effective scheduling system is needed. The authors suggest that a flexible scheduling framework and communication between stakeholders become two influenced aspects for a better surgery scheduling in the perioperative planning. The framework considers the patients’ evaluation process during the perioperative period, as of the communication may affect the surgical teamwork performance and the patient safety. It is believed that an effective design of operating room scheduling system can be developed to support the decision making process, thus implicate to the better continuity planning during the perioperative period and increasing the stakeholders’ satisfaction.
