Most studies have agreed that there is a considerable decline in sexual activity following stroke. but the hypersexuality after stroke as a relatively isolated behavioral abnormality has been less frequently reported Hypersexuality has no proven treatment, although reports have described reductions in hypersexual behavior did not response to three medications were not uncommonly observed in clinical practice A 65-year-old right-handed male presented with acute left hemipararesis. His MRI showed a right middle cerebral artery infarct involving the temporal, parietal regions and the basal ganglia. Four weeks after stroke he showed abnormal behavior, such as incessantly holding and hugging the nursing staff without assent, but no increased orality or change in eating habits But several weeks after taking cimetidine. a histamine H₂-receptor antagonist. his sexual behavior showed a marked improvement This case demonstrates that cimetidine as a non-hormonal anti-androgenic effect decreased libido and hypersexual behavior without serious side effects in patients with cognitive decline after stroke Further studies that obviously define the response and long-term side effects of cimetidine are needed to confirm our observations.

This is a report of familial carpal tunnel syndrome(CTS) occuring in four members of two generations of a Korean family. One of those with CTS had surgical release and its pathologic finding was nonspecific fibrotic change. The age of those affected by CTS ranged form 35 to 76 years of age. Family pedigree suggests an autosomal dominant gene. This is belived to be the first case report of familial carpal tunnel syndrome in Korea.

Background and Objective:Subcortical vascular dementia (SVD) incorporates small vessel disease as the primary vascular etiology, represents in MRI by lacunar infarct and deep white matter lesions. In clinical practice, a number of SVD patients present with various vascular risk factors for large artery disease (LAD) i.e., old age, hypertension, and diabetes. However the current diagnostic criteria for SVD did not include angiographic findings as a key feature. Therefore we tried to find out the frequency of large artery disease in SVD and to elucidate whether the presence of LAD affects clinico-radiological manifestations of SVD. Methods:Thirty three patients fulfilling 'Research criteria for SVD' by Erkinjuntti from 17 study centers were recruited. Of these 33 patients, 21 who underwent comprehensive neuropsychological test battery, MRI ^(18)FDG-PET, and MR angiography were classified into two subgroups according to the patterns of MRI abnormality:white matter dominant (WM) type (n=5) and multiple lacunar (LC) type (n=16). Clinical and neuropsychological profile and ^(18)FDG-PET findings of 21 SVD patients with and without LAD were compared. Results: Twelve (57%) of 21 SVD patients had mild stenotic lesions in intracranial large arteries:4 in middle cerebral artery, 8 in posterior cerebral artery, and 4 in vertebrobasilarj artery. There were no differences in vascular risk factors, characteristics of cognitive dysfunctions, and ^(18)FDG-PET findings between SVD with and without LAD. Conclusions:Our results showed that the presence of mild degree of large artery stenosis did not affect the clinical., neuropsychological profile and cerebral metabolism in PET. Therefore, the current 'Research criteria for SVaD' by Erkinjuntti could be used to diagnose SVD as a homogenous clinical group regardless of angiographic abnormalities.

Aims: The optimal indication of LT in HCC patients has evolved from Milan criteria to morphologic criteria with biologic markers. The role of circulating cancer stem cells has not been reported in patients who underwent LT for HCC. Methods: From April 2014 to March 2017, 25 patients who underwent LDLT for HCC were prospectively enrolled. EpCAM, CD90 and EpCAM/CD90 were sorted by FACS and mRNA expression of EPCAM, KRT19, THY1 were analyzed by RT-PCR in peripheral blood at preoperative, postoperative day 1 and 7, respectively. The median follow-up duration was 40 months. Results: The mean age was 55.9 years, and HBV was the most common underlying liver disease (88%). 10 patients were above Milan criteria at diagnosis, and 20 patient received preoperative treatments before LT. HCC recurred in 4 patients. The detected numbers of EpCAM (+) cells and CD90 (+) cells were well correlated with their mRNA expression levels in the peripheral blood (P<0.05). EpCAM protein in HCC tissue was highly expressed in patients with recurrence (66% vs. 20%) but didn’t reach statistical significance (P=0.172). HCCs with EpCAM (+) protein expression showed more detection of EpCAM (+) circulating cells than EpCAM (-). The detection of EpCAM (+) or EpCAM(+)/CD90(+) cells before surgery and at postoperative day 1. Conclusions: Detection of EpCAM (+) or EpCAM(+)/CD90(+) cells in the peripheral blood before surgery and at postoperative day 1 was the only variable significantly associated with HCC recurrence after LDLT but should be validated in a largescale prospective study.
노인 파킨슨병 환자의 장기요양서비스 이용 양상과 이용 장소에 따른 부적정 약물 사용 현황 및 관련 요인 분석

The objective of this study was to evaluate the prevalence rate of potentially inappropriate medication (PIM) in elderly patients with Parkinson’s disease and compare the prevalence rate by Long-term Care service use and service type. We analysed the National Health Insurance Claims data and Long-term Care Claims data. In this study, the prevalence rate of PIM in Korean elderly patients with Parkinson’s disease was very high (LTC user group-45.6% vs. non user-40%). We found that prevalence rate of PIM was related with poly-pharmacy, women, advanced age, high number of medical institutions, high hospitalization days and outpatient days, and disabled. The commonly used PIM drugs were found as benzodiazepines, first generation antihistamines. In particular, the prevalence of PIM was higher in the Long-term care facility user group than in-home service user group. The high prevalence of PIM in elderly patients with Parkinson’s disease suggests that systematic and comprehensive measures should be prioritized.
