        From 1992 to 1998, 41 patients received therapy for primary cancers of the extrahepatic biliary system(seven gallbladder and 34 extrahepatic biliary duct). Chemotherapy was administered to 12 patients(29 percent). Fifteen patients(37 percent) also received postoperative radiotherapy. All patients received megavoltage external beam radiation therapy(range, 3000 to 5580 cGy; median, 5040 cGy). The median survival of all patients was 10 months. Median survivals according to treatment mode, radiation therapy only, chemotherapy with radiotherapy, and postoperative radiotherapy were 5, 15, and 13 months, respectively(p=0.0194). Another prognostic factors were age(p=0.0248), weight loss(p=0.0446), and performance status(p=0.0296). Local failure was 53%, distent metastasis was 26% and both are 21%. extent of survival, chemotherapy agent and new treatment modalities, as IORT(intraoperaive radiotherapy and intraluminal brachytherapy will be introduce.

      • 수술후 재발한 자궁경부암의 방사선 치료 성적

        Surgical treatment is important modality for treatment of early stage uterine cervical cancer. But some of the cases may have treatment failure depending on the prognostic factors. Treatment of choice after locoregional recurrence is radiation modality. This is a retrospective analysis of 31 patients with locoregionally recurrent uterine cervix cancer with irradiation alone and/or palliative chemotherapy between August 1985 and July 1994. Fourteen patients had central (vaginal) recurrence and seventeen patients had other pelvic site recurrence. All patients had whole pelvis irradiation at least 50Gy and/or intracavitary radiotherapy(ICR). The results and conclusions are as follows. 1. Complete response was achieved in 16 patients (52%), 2 and 5 years survival rate were 38% and 13%, respectively. 2. Prognostic factors by univariate analysis were age, recurrence site, size of recurrent mass, ICR and response to treatment. Stage was marginally significant prognostic factor. By multivariate analysis, only size of recurrent mass was significant factor. 3. To increase control rate after recurrence, regular follow up after surgery and early detection of recurrence is important. 4. To decrease treatment failure, external radiotherapy and ICR is necessary and roles of other modalities including chemotherapy must be identified.

      • 자궁경부암의 수술후 방사선치료 결과와 림프절 전이상태의 관련성에 관한 연구

        This is a retrospective analysis of 124 patients with uterine cervix cancer who were treated with postoperative radiation therapy after radical hysterectomy and lymph node dissection between May 1985 and May 1994 at the Department of Radiation Oncology, Soonchunhyang University Hospital. The results were as follows. 1. Overall and relapse free 5 year survival rate were 75.4% and 73.5%, respectively. 2. The numbers and sites of lymph node metastasis were prognostic factors by univariate analysis. Also the lymph node dissection numbers in patients with lymph node metastasis negative were prognostic factor. 3. Treatment related failures were 33 cases, the majority of them (23 patients) were in patients with lymph node metastasis. 4. To control recurrence for patients with high risk lymph node metastasis, postoperative radiotherapy is not sufficient. This result suggests that additional radiosensitizing agent or chemotherapy may be necessary for higher disease control. Also this study suggests the " through dissection of lymph node " in patients with uterine cervix cancer instead of " a few lymph node sampling ".

      • 근원 불명 전이암의 예후인자와 생존율

        Purpose : Patients with cancer of unknown primary site represent 5% to 10% of all cancer patients. This study was to evaluate survival and prognostic factors of patients with malignancy of unknown origin. Materials and Methods : A retrospective analysis was done for one hundred and two patients with malignancy of unknown origin who were treated from January 1989 to December 1998. There were 52 male and 50 female patients. Age of patients ranged from 20 to 94 years (mean 58 years). Clinical presentation depended on the site of metastases. In cases of multiple metastatic sites, we selected the first apparent metastasis or the symptomatic one for which the patient was first seen. The three most frequent initial metastatic sites were peritoneum, lymph node, and liver. Median follow up period was 4 months(0-100 months) and survival rate measured form the time of diagnosis. Results : The two years survival rate was 13.6% with a median survival time of 4 months. The factors associated with the overall survival rate were the tumor marker CEA(p=0.0061), smoking history(p=0.0039), and histologic type(squamous cell carcinoma versus non-squamous cell carcinoma, p=0.0295). Marginal statistical difference was found according to performance status. No statistical differences were found according to sex, weight loss, initial metastatic site and numbers, primary lesion site, and treatment modality. Conclusion : Squamous cell carcinoma was associated with survival rate. In squamous cell carcinoma of unknown origin with good prognostic factor, especially cervical lymph node metastasis, we suggest an approach of extensive initial workup for staging and aggressive treatment.

        The biochemical composition of muscle from three deep-sea animals, Tanaka`s eelpout Lycodes tanakae magistrate armhook squid Berryteuthis magister, and ocean sunfish Mola mola caught in the East sea in South Korea was determined, The moisture (81.4-93.8 g/100 g), crude protein (5.0-15.6 g/100 g), crude lipid (0.1-1.5 g/100 g), and ash (1.0-1.1 g/100 g) contents of the two fishes were analyzed. The proximate composition of magistrate armhook squid was: moisture 84.0 g/100 g, crude protein 12.4 g/100 g, crude lipid 0.7 g/100 g, and ash 1.5 g/100 g. There was little difference in the proximate compositions of the three animals. The total amino acid contents of Tanaka`s eelpout, magistrate armhook squid, and ocean sunfish were 14.64, 10.75, 3.10-9.95 g/100 g, respectively. High levels of glutamic acid, aspartic acid, lysine, leucine, and arginine were found, while low levels of histidine, glycine, and cysteine were detected in the animals. A survey of the free amino acid contents of the animals revealed large amounts of alanine and glycine. Significant differences were found in the fatty acid composition among the three species. Specially, different saturated fatty acids (17.14-40.49%) such as C16: 0, and C18:0, monounsaturated fatty acids (19.19-46.88%) such as C16:1, and C18:1, and polyunsaturated fatty acids (35.41-57.08%) such as EPA, and DHA was identified. Our results suggest that each of these deep sea animals possesses nutritional value and should be considered as a foodstuff.

        한국 연근해 어류 14종의 일반성분 및 지방산 조성

        Proximate and fatty acid compositions in muscle tissue were studied in 14 species of coastal and offshore fishes in Korea. Lipid content ranged from 0.57 to 10.5% and was higher in migratory and reef dwelling fishes than in demersal fishes. Protein content ranged from 14.4 to 20.3% and moisture content ranged from 72.4 to 81.8% in all fish samples. There was a negative correlation between lipid and moisture content in all fish samples (r= -0.90, P<0.001). The prominent fatty acids were 16:0, 22:6n-3 (docosahexaenoic acid, DHA), 18:1n-9, 20:5n-3 (eicosapentaenoic acid, EPA), 16:1n-7, 18:0 and 18:1n-7. Most demersal fishes contained considerably higher amounts of 20:4n-6 and/or 22:5n-3 compared with migratory and reef dwelling fishes. The proportion of total polyunsaturated fatty acid (PUFA), including DHA and EPA, was higher in demersal fishes (38.6-54.0%) than in migratory and reef dwelling fishes (23.5-35.2%).
