Objective: Early stage primary carcinoma of the fallopian tube (PCFT) is an uncommon condition when strict criteria are applied. The aim of this study was to compare the outcome stage IA-IB PCFT to a matched group of ovarian cancer (OC). Methods: Between 1990 and 2008, 32 patients with stage IA-IB of PCFT were recorded in the database of three French Institutions. A control group of patients with OC was constituted. Results: Eleven eligible PCFT cases and 29 OC controls fulfilled the stringent inclusion criteria. Median follow-up was 70.2 months. Five-year overall survival was 83.3% (95% confidence interval [CI], 27.3 to 97.5) for PCFT and 88.0% (95% CI, 66.9 to 96.0) for OC (p=0.93). In the subgroup of patients with grade 2-3, the outcome was similar in PCFT compared to OC patients (p=0.75). Five-year relapse-free survival was respectively 62.5% (95% CI, 22.9 to 86.1) and 85.0% (95% CI, 64.6 to 94.2) in the PCFT and OC groups (p=0.07). In the subgroup of patients (grade 2-3), there was no difference between PCFT and OC (p=0.65). Conclusion: The findings did not reveal any difference in prognosis between early stage of PCFT and OC when grade is taken into account. Management of PCFT should mirror that of ovarian carcinoma. Self-Avocacy : A Plarform for Inclusion

The culture of self-advocacy groups is somewhat similar and comparisons as well as contrasts can be foung. We see what some of the obstacles are now and whether they have changed over time and we can speculate on how this movement has an effect on changing the system of services for people with mental retardation and the professionals who design and administer them. Certainly advocates and personal supporters who work in the trenches to help the movement grow have also changed their perspectives and the kind of support they provide to adjust to the new self-advocacy. In essence, the attainment of leadership roles by people with cognitive disabilites is recognition of the inherent right of human beings to become self-determining and of an individuals right to partivcipate in decisions affecting his or her life. it is both right and beneficial that individuals with cognitive disabilities assume roles of leadership, both within disability organizations and in other parts of the community.
Management of vesicoureteral reflux in neurogenic bladder

Vesicoureteral reflux (VUR) is a significant risk factor for pyelonephritis and renal scarring. VUR can occur through a defective ureterovesical junction (UVJ) or an overwhelmed normal UVJ mechanism such as in bladder dysfunction of congenital, acquired, or behavioral etiology. There are numerous causes for the development of a neurogenic bladder from spinal dysraphisms to spinal cord trauma and even centrally based abnormalities in children with apparently normal motor function (inappropriately termed nonneurogenic neurogenic bladder). The foundation of managing reflux in these neurogenic bladders is to maintain low bladder pressures which will commonly mean that compliance will be normal as well. There have been several publications that have shown that if bladder pressures are lowered simply with clean intermittent catheterization and medications that the reflux can resolve spontaneously. Alternatively, the patients that are in need of bladder augmentation can have spontaneous resolution of their reflux with the resulting increase in capacity. Surgical intervention is called for when bladder capacity is adequate and the reflux persists or if it is part of a larger operation to increase capacity and to manage outlet resistance. In some instances, reimplantation is necessary because the ureters interfere with the bladder neck procedure. Aside from open and robotic surgical intervention the use of endoscopic injectable agents is beginning to become more popular especially when combined with intravesical botulinum toxin A injections. Great strides are being made in the management of patients with neurogenic bladders and we are seeing more choices for the urologist to be able to manage these patients.

Arsenic is a known hazardous metalloid not only to the animals but also to plants. With high concentrations, it can impede normal plant growth and cause even death of plants at extremely high levels. A known plant response to stress conditions such as toxic levels of metal (loids) is the production of stress ethylene, causing inhibitory effect on root growth in plants. When the effect of various arsenic concentrations was tested to maize plant, the stress ethylene emission proportionately increased with increasing concentration of As(V). The inoculation of two arsenic tolerant bacteria; Pseudomonas grimonti JS126 and Pseudomonas taiwanensis JS238 having respective high and low 1-aminocyclopropane-1-carboxylate (ACC) deaminase activity reduced stress ethylene emission by 59% and 30% in maize grown in arsenic polluted soils. The result suggested the possible use of Pseudomonas grimonti JS126 for phytoremediation of arsenic polluted soils.
Recent Advances in Re-engineering Modular PKS and NRPS Assembly Lines

Polyketides such as the antibiotic erythromycin or the immunosuppressant rapamycin, and non-ribosomal peptides, such as the antibiotics penicillin or vancomycin, are important classes of natural products. The core of these molecules are biosynthesized by large polyketide synthases (PKS) and non-ribosomal peptide synthetases (NRPS), respectively. The modular architecture of these enzymatic assembly lines makes them interesting candidates for synthetic biology approaches. The re-engineering efforts aim to understand the molecular structure, produce new compounds, produce analogs of known compounds, tag the products or improve activity and/or yield. Here, we first consider the definition of PKS and NRPS modules, then give an overview of different strategies for re-engineering and finally review recent examples of PKS and NRPS reengineering.
