        Nicardipine이 당뇨병성 자율신경병증 환자에서 기관내 삽관에 따른 혈역학적 변화에 미치는 영향

        Background: A laryngoscopy and tracheal intubation often provoke an undesirable increase in blood pressure and heart rate. These hemodynamic responses to tracheal intubation in diabetics are blunted due to autonomic neuropathy. This study was designed to determine the optimal dose of nicardipine in diabetic autonomic neuropathy patients. Methods: According to the nicardipine dose, 80 diabetics were randomly allocated to four groups of 20 patients. Tracheal intubation by direct laryngoscopy was performed. After intravenous thiopental d mg/kg, vecuronium 0.13 mg/kg and one of the dosages of nicardipine 3, 5, or 7 ㎍/kg followed by mask ventilation of three minutes with enflurane, nitrous oxide and oxygen. Heart rate and blood pressure were mesured five times at one minute intervals. Results: There was no significant difference in the 3 ㎍/kg group compared with the control group. On the other hand, there was a sufficient blood pressure decrease in the ㎍/kg and 7 ㎍/kg group. However, in the 7 ㎍/kg group, 55% of cases showed severe hypotension (< 70 mmHg). Conchusions: We suggest that the appropriate dose of nicardipine for prevention of tachycardia, and hypertension in diabetic autonomic neuropathy patients is 5 ㎍/kg.       • 로보틱셀폰(RCP)의 이동시스템 구현에 관한 연구

        Most recently, CP(Cellular phone) has been one of the most important technologies in the IT(Information Tech-nology) fieid,and it is situated in a position of great importance industrially and economically. To produce the best CP in the world, a new technological concept and its advanced implementation technique is required, due to the extreme level of competition in the world market. The RT(Robot Technology) has been developed as the next generation of a future technology. Current robots require advanced technology, such as soft computing, human-friendly interface, interaction technique, speech recognition, object recognition etc.unlike the industrial robots of the past. Therefore, this paper explains conceptual research for development of the RCP(Rpbptic Cellular Phone), a new technological concept, in which a synergy effect is generated by the merging of IT & RT. RCP infra consists of RCP^(Mobility), RCP^(Interaction), RCP^(Integration) technologies. For RCP^(Mobility) human-friendly motion automation and personal service with walking and arming ability are developed. RCP^(Interaction) ability is achieved by modeling an emotion-generating engine and RCP^(Integration) that recognizes environmental and self conditions is developed. By joining intelligent algorithms and CP communication network with the three base modules, a RCP system is constructed. Especially, the RCP mobility system is focused in this paper. RCP^(Mobility) is to apply a mobility technology, which is popular robot technology, to CP and combine human-friendly motion and navigation function to CP. It develops a new technological application system of auto-charging and real-world entertainment function etc. This technology can make a CP companion pet robot. It is an automation of human-friendly motions such as opening and closing of CPs, rotation of antenna, manipulation and wheel-walking. It's target is the implementation of wheel and manipulator functions that can give service to human with human-friendly motion. So, this paper presents the definition, the basic theory and experiment results of the RCP mobility system. We confirm a good performance of the RCP mobility system through the experiment results.

        술후 진통을 위해 투여한 Ketorolac이 혈액응고 기능에 미치는 영향 : 혈전탄성묘사도를 이용한 평가 A Thromboelastographic Evaluation

        Background: Thromboelastography (TEG) has recently become popular for assessment of whole blood coagulation in the operating room. Ketorolac, a potent injectable nonsteroidal anti-inflammatory drug (NSAID), is commonly used for postoperative analgesia. NSAID inhibit plated aggregation in coagulation process. This study was designed to determine whether ketorolac used for postoperative analgesia can affect hemostatic function using a TEG. Methods: Seventy-four female patients, ASA physical status 1 or 2, scheduled for an elective gynecologic surgery were randomly allocated into one of four groups (Group 1: n=10, control without patient-controlled analgesia (PCA); Group 2: n=21, PCA with morphine 60 mg; Group 3: n=20, PCA with morphine 30 mg + ketorolac 90 mg; Group 4: n=23, PCA with ketorolac 180 mg). Blood samples were obtained for TEG analysis preoperatively and 24, 48 and 72 h after surgery. Cumulative drug dosage, visual analog pain scale, satisfaction degree and side effects were measured at 24, 48 and 72 h after surgery. Results: There were no significant differenced in TGE parameters among the four groups at each time. There were no significant differenced in visual analog pain scales and satisfaction degree among the three groups using PCA for postoperative analgesia. Among the three groups using PCA for postoperative analgesia, Group 2 experienced more side effects. Conclusions: Ketorolac does not affect hemostatic function for 3 days after surgery when administrated as a PCA drug. (Korean J Anesthesiol 2002; 43: 755~762)

        Development of MIS for Vertically Integrated Broiler Farm. This study develops management information system(MIS) for vertically integrated broiler farm. Development procedure is devided into five steps, System Planning, System Analysis, System Design, and System Implementation, System design suggested by Whitten, Bentley and Barlow. The study produces usual DfD and Marcf style ERD for Broiler farm MIS. The final MIS is constrcuted based on the ERD using Visual Basic 5.0 and Access 7.0. The system supports broiler farm managers in managing the production, marketing, accounting, financial analysis, and diagnosing and planning their business. The system adopts user-friendly functions and lloesn't require farmers knowledge in accounting and information technology.

        지역특화작목시험장의 혁신전파 유형화와 커뮤니케이션 만족도 연구

        본 연구는 지역특화작목시험장에서 농업인에게 혁신기술을 전파하는 과정을 유형화하고,각 유형별로 나타나는 커뮤니케이션 특성과 만족도를 분석함으로써 지역 내 혁신전파역량 향상을 위한 시사점을 제시하는 것을 목적으로 한다. 이를 위해,지역특화작목시험장이 수행하고 있는 혁신전파활동방식과 농업인간의 상호작용수준을 중심으로 대응분석을 실시한 후,그 결과를 활용하여 군집분석을 실시하여 개별유형을 명명하였다. 최종적으로 분산분석을 실시하여 개별 유형별 성과 차이를 분석하였다. 이 과정 에서 280부의 농업인 설문자료가 최종적으로 활용되었다. 분석결과,혁신전파의 유형별 특성은 상호작용수준과 체계성만으로도 전체적인 설명과 유형화가 가능하였으며,개별 특성에 따른 유형은 ‘형식적지도형’,‘상황대응전파형’,‘농업인연계중시형’,‘체계적접근형’으로 구분되었다. 형식적지도형은 낮은 상호작용과 낮은 체계성을 통해 가장 기본적인 혁신전파활동을 유지하는 유형이며,상황대응전파형은 지역 내 실정과 작목특성,농업인과의 상호작용 방식 등이 복합적으로 작용하여,특정 형태의 상호작용수준이나 혁신전파활동으로 제한되지 않고 다변적으로 나타냐는 상황을 총괄하는 유형이다. 농업인연계중시형은 다소 낮은 체계성의 방식을 사용하나 농업인과의 높은 상호작용수준을 유지하면서 다양한 매체의 활용을 통해서 파트너쉽을 지속하는 형태이며,체계적접근형은 상호작용수준을 중시했다기 보다는 체계성이 유지하고 있는 상호작용의 발현으로 혁신전파활동을 유지하는 형태이다. 개별 유형별로 현장성,접근성,전달성,적용성에서 상황대응전파형,체계적접근형,형식적지도형,농업인연계중시형의 순으로 높게 나타났으며,개별 속성들간에는 유기적인 맥락이 존재하였다. This study differentiates innovation diffusion process of agricultural experiment station in terms of interaction and systematicity levels. The study use correspondence analysis to find relationship between interaction and system level. Then, extension and diffusion type of regional experimental station is classified based on the clustering analysis of 280 survey data obtained from farmers. The individual characteristics of 'formal extension type', 'situational reaction diffusion type', 'farmers connection type', and 'systematic approach type', all differentiated according to innovation diffusion patterns, were pre-defined. The formal extension type maintains the most basic innovation diffusion activities through low interaction and low systematicity. The situational reaction diffusion type oversees situations where the local situation, crop/plant characteristics, and methods of interaction with farmers act in a complex manner with considerable diversity., instead of being limited to specific forms of interaction levels or innovation diffusion activities, farmers connection type uses methods with relatively low system level but maintains a high level of interaction level with farmers, It also utilize partnership through the use of diverse media. The systematic approach type maintains innovation diffusion activities through interaction maintained by system level instead of focusing on interaction levels, In terms of individual types, immediacy, accessibility, referability, applicability, and satisfaction were the highest in the situational reaction diffusion type, systematic approach type, formal extension type, and farmers connection type, in the order. And there existed organic contexts among individual characteristics.

        Case Report : A Case of Fibroelastolytic Papulosis on the Neck of a Young Man

        Fibroelastolytic papulosis of the neck (FEPN) encompasses a spectrum of two disorders that were previously reported as pseudoxanthoma elasticum-like papillary dermal elastolysis (PXE-PDE) and white fibrous papulosis of the neck (WFPN). The clinical presentation of FEPN is asymptomatic to mildly pruritic whitish-yellow papules that may coalesce into cobblestone patterned plaques that resemble pseudoxanthoma elasticum (PXE). The histology is characterized by a decrease or loss of elastic fibers in the papillary dermis and this is sometimes accompanied by a minimal or nodular increase of dermal collagen fibers. We report here on a 28-year-old Korean man with asymptomatic, multiple, skin-colored to slightly yellowish, match-head sized, cobblestone-patterned papules on the neck, and these were histologically consistent with FEPN and the papules showed slightly increased dermal collagen associated with decreased and fragmented elastic fibers, elastin and tropoelastin. The pathogenesis of FEPN in this case might have been related with mild dermal inflammation, followed by fragmentation, elastolysis and increased dermal collagen. (Ann Dermatol 23(2) 193∼197, 2011)

        Epidemiologic Study of Malassezia Yeasts in Acne Patients by Analysis of 26S rDNA PCR-RFLP

        Background: Although acne is a common follicular inflammatory dermatosis, studies of the relationship between Malassezia yeasts and acne have rarely been conducted. Objective: We sought to identify Malassezia yeasts from acne patients and establish a relationship between specific types of species of Malassezia and acne. Methods: Sixty acne patients were enrolled. Each strain obtained was identified as one of eleven species by 26S rDNA PCR-RFLP. We then compared these data with those of age- and sex-matched healthy subjects. Results: Growth of Malassezia was evident in fewer patients with acne (50%) in comparison to controls (70.6%). M. restricta was dominant in patients with acne (23.9%), whereas M. globosa was most common (26.7%) in healthy controls. In the patients group, the rate was the highest (71.7%) in the twenties and, in terms of body site, the rate was the highest (60%) in the chest. In the control group, the rate was the highest (75.0%) in the thirties and in the forehead (85.0%). Conclusion: The detection rate of Malassezia yeasts was conspicuously low in the acne patients group. Statistically significant differences were observed between the patient and the control groups in the twenties and thirties, and in terms of body site, in the forehead and chest.

      • Current and Prospect of Agricultural Information System in Korea

        For the last 10 years, many institutions have tried to build more effective agricultural information system. They have devotedmost effort to build DB/NWs for agribusiness, farmers, and rural community. However, their information system has not satisfied the demands on farm. Few suggestions are raised to build successful information based agriculture and rural community. First, we should focus more on developing a farm based MIS SWs rather than the DB/NWs. Second, we should emphasize education and AS of information system ore than development of the system. Third, governments should invest more to build better information infrastructure for rural community. Only 0.5% of farmers use internet currently. If we don't meet farmer's demand on information system, they will not use the system. And then we will have no future to build information intensive agriculture in Korea. Keeping these problems in mind, we can prepare for good prospects in future. With information infrastructure expending, farms and agribusinesses use PCs and internet regularly. Using the PCs and internet will be lot easier with friendly interfaces and protocol. Various farm management programs are available through ASP. Farmers can monitor market and expected prices for their own products in real time through market monitoring system. Various type of Ecs and online trading can help agribusiness to keep their own blend and prices. Fast and consistent communication infrastructure will help the small business of farms to integrate vertically and horizontally easily. It will also makes remote conference and control easier and hence reduce socio-cultural gap between urban and rural area with remote medical operation, teaching, and cultural performance. Improved robotics and artificial intelligence will allow farmers to increase size of their farm with automated production facilities and equipments.
