Clear Cell Meningioma of Cauda Equina without Dural Attachment

We report a 17-year-old woman presented with a one-month history of lower back pain and radiating pain in the left leg. Examination revealed Lasegues sign in left leg with mild weakness of the plantar flexion of the left big toe. Magnetic resonance image revealed a well enhanced intradural lesion at the S1-2 level. Following a subtotal laminectomy of S1 and an intradural exposure, the roots of the cauda equina draped the tumor loosely without any attachment to the dura or cauda equina. The tumor was removed en bloc. The patient's recovery was uneventful. Histological examination confirmed a clear cell meningioma. Key words:Clear cell meningioma;Spinal cord neoplasm;Cauda equina.

분류층 가스화기에서 슬래그의 점도 변화는 가스화기 운전조건을 결정하는 중요한 요소이다. 본 논문에서는 국내 가스화 대상탄으로 선정된 9개 탄의 슬래그 점도를 가스화 조건에서 측정하였고 점도 변화에 영향을 미치는 슬래그내의 결정체의 크기와 종류를 관찰하였다. 시료는 고둥기술원 건식 가스화기에서 배출된 슬래그 시료를 사용하였다. 9개탄 중 Alaska Usibelli, Curragh, Drayton, Adaro, Baiduri탄 슬래그는 결정슬래그의 경향을 보여주었고, Datong탄과 Cypurus탄 슬래그는 유리슬래그이였다. 결정슬래그외 경우 $T_{cv}$ +$50^{\circ}C$를, 유리슬래그의 경우 $T_{1000}$poise/+$50^{\circ}C$를 가스화기의 최저운전온도로 정의하였을 때 Denisovsky 탄이 가장 높은 온도를, Drayton탄이 가장 낮은 온도를 필요로 하였다. 모든 슬래그가 가스화기 내화물성분인 C $r_2$ $O_3$를 포함하고 있었고, 132$0^{\circ}C$ 주위에서 $T_{cv}$ 를 갖는 결정슬래그의 경우(Alaska Usibeli, Curragh, Adaro탄) 냉각중에 형성됨을 나타내는 커다란 anorthite이 주 결정상으로 나타나 anorthite의 형성이 점도의 급격한 상승의 원인이 됨을 확인하였다. 또한 기포를 갖고 있는 Denisovsky탄의 경우 기포가 산화철의 환원에 의해 형성됨을 추정할 수 있었다.. In the entrained flow gasifiers, the slag viscosity is an important parameter determining the operation conditions. The slag viscosities of 9 coals, which were selected and tested as possible gasification feedstock in Korea, were measured in a high temperature slag viscometer under gasification conditions. The type and size of crystalline phases that were known to affect the slag viscosity behavior were also determined. The slag samples were obtained from the IEA dry-feed gasifier. The slags of Alaska Usibelli, Curragh, Kideco, Adaro, Denisovsky, Baiduri and Drayton coals showed the behavior of crystalline slags, while those of Datong and Cypurus glassy slags. When a recommended minimum operating temperature was arbitrarily defined as $T_{1000}$poise/+$50^{\circ}C$ for glassy slags and $T_{cv}$ +$50^{\circ}C$ for crystalline slags, the Drayton slag required the lowest temperature, while Denisovsky slag required the highest. All the slags contained C $r_2$ $O_3$ from the refractory. The crystalline slags with $T_{cv}$ at around 132$0^{\circ}C$ contained large anorthites as the major crystalline phase that would have caused the rapid inrease in viscosity. Denisovsky slag contained many pores which were formed by $O_2$ from F $e_{x}$O reduction..
Prevalence and Epidemiological Features of Moyamoya Disease in Korea

Objective:The objectives of the present study were to investigate the annual detection rate of patients with Moyamoya disease (MMD) and to describe the prevalence and epidemiological features of the Moyamoya patients in Korea. Materials and Methods:The authors analyzed the epidemiological data of Korean patients taken from the National Health Insurance Corporation in Korea among Moyamoya patients who were treated from 2004 until 2008. Results:Based on 2004 data, 2,539 MMD patients were treated in Korea and the prevalence rate was 5.2 per 100,000 people. There were 2,987 in 2005, 3,429 in 2006, 4,051 in 2007, and 4,517 cases in 2008, and the prevalence rates per 100.000 people were 6.3, 7.0, 8.6, and 9.1, for those respective years. This represents an annual increase of 15% of new cases during this period. In 2008, 466 people were newly diagnosed with MMD, representing an incidence rate of 1 per 100,000 persons. The gender ratio was 1,547 men (34%) and 2,970 women (66%). Women had a higher incidence rate than men (1.94 times). There were two age peaks: teenagers and those in their forties. Conclusion:The present study shows that the number of Moyamoya patients in Korea is increasing. This increase could partly be explained by a recent increase in newly diagnosed cases, suggesting that a more careful consideration of the disease and better diagnostic techniques should be promoted among clinicians.
T1 Slope and Cervical Sagittal Alignment on Cervical CT Radiographs of Asymptomatic Persons

Objective : We performed a retrospective analysis of medical records and radiographic images of patients who never underwent spinal treatment including diagnosis. The objective of this study is to explain the biomechanical and physiologic characteristics of cervical alignment related to thoracic inlet angle including T1 slope changes in each individual. Methods : We reviewed the cervical CT radiographs of 80 patients who visited ENT outpatient clinic without any symptom, diagnosis and treatment of cervical spine from January 2011 to September 2012. All targeted people were randomized without any prejudice. We assessed the data-T1 slope, Cobb’s angle C2-7, neck tilt, sagittal vertical axis (SVA) C2-7 and thoracic inlet angle by the CT radiographs. Results : The relationships between each value were analyzed and we concluded that Cobb’s angle C2-7 gets higher as the T1 slope gets higher, while the SVA C2-7 value decreases. Conclusion : We propose that the T1 slope is background information in deciding how much angle can be made in the cervical spinal angle of surgical lordotic curvature, especially severe cervical deformity.

The authors describe a patient with multiple myeloma who was treated for an acute cervical vertebra body fracture by percutaneous vertebroplasty. A 48-year-old female patient presented a severe neck pain without neurologic deficit. A myelomatous pathologic fracture of C4 was diagnosed, and percutaneous vertebroplasty was performed successfully through a left anterolateral approach. Her posterior neck pain was immediately relieved with vertebroplasty and no instability was observed over 18 months of follow-up. Various complications related to anterior lumbar interbody fusion(ALIF) have been reported in the literature. However, disseminated intravascular coagulation(DIC) after venous injury during ALIF has not been previously reported. We describe a rare case of DIC after ALIF.
