        약사의 전문성강화를 위한 국가별 평생교육제도 비교연구

        The purpose of this study is to provide the comparative study results for the purpose of the developing con- tinuing professional educational systems for Korean pharmacists. The professional continuing educational systems of the UK, USA, Japan and Korea were analyzed. General Pharmaceutical Council in UK controls the pharmacy professions for the protection of the health of the citizens in UK and certifies the schools of pharmacy and the continuing education pro-viders. The USA and Japan have the several accreditation bodies for the pharmacy education and the continuing phar-maceutical education. However, the quality assurance systems in Korea for the continuing education and specialty programs are not implemented, yet. The renewal system of the pharmacist license should be introduced like the health care providers like doctors to improve professionalism. Finally, it is recommended that all of these things should be done by a single admin-istrative authority such as Korean Accreditation Council for Pharmacy Education.

        Mancozeb의 급성노출이 마우스의 면역기능에 미치는 영향

        Mancozeb, a polymeric complex of zinc and manganese salts of ethylene bisthiocarbamate (EBDC), is used widely in agriculture as fungicides, insecticides, and herbicides. Mancozeb can be occupationally and environmentally exposed to human and has been reported to induce estrogenic activity, therein it is considered as an endocrine disrupter, We investigated the effects of acute exposure of Mancozeb on the immune function in mice. After single oral administration of Mancozeb to female ICR mice, the immunopathological parameters (body- and organ-weight, splenic cellularity hematological parameters), mitogen (Con A, PHA+IL-2, LPS)-induced splenocyte proliferation (SP) and splenic IgM plaque forming cell (PFC). WBC and splenic cellularity were decreased, but liver-, kidney-, and spleen-weight were increased when compared with control group. Splenic IgM PFC against SRBC was slightly lowered. Mitogen-induced proliferation of spleen cells from Mancozeb-treated mice was not signifcantly changed ex vivo, however, SP in vitro were significantly lowered in concentration dependent manner. These results indicate that Mancozeb could affect the immune function in mice.

        Mancozeb (MCZ), a polymeric complex of zinr and manganese salts of ethylene bisthiocarbamate, is widely used in agriculture as fungicidel, insecticides, and herbicides. MCZ can be occupationally and environmentally exposed to human and has been reported to have detrimental effects on the reproductive system, but the toxicity of MCZ on immune responses has not been systematically investigated. We investigated the effects of MCZ exposure on the activities of murine splenocytes through evaluation of splenocytes cellularity and INF-$\gamma$ synthesis. Splenocytes were examned ex vivo from mice orally treated with various doEes of MCZ for 1 day (acute exposure, 2,100, 5,000, 10,000 mg/kg) or ior 5 consecutive days per week for 4 weeks (subacute exposure,250, 1,000, 1,500 mg/kg/day) fellowed by culture for 2 days in the presence of Con A or PHA plus IL-2. Splenocytes Iron naive mice were cultured with various concentration of MCZ (50, 500, 1,000 ng/ml) in the presence of Con A or PHA plus IL-2 for 2 days in vitro. IFN-$\gamma$ production was decreased with the in vitro exposure to all concentration of MCZ. The spleen cellularity and IFN -$\gamma$ production by splenocytes from MCZ -acutely and - subacutely exposured mice were decreased in comparision with that oi control group.

        유아 혈관종에서 저용량 Propranolol의 사용 적절성 평가

        Infantile hemangiomas (IH) are the most common benign tumor during childhood. The majority are self-resolving, however, 10% of patients require treatment. Researches on theefficacy of propranolol for IH have been reported outside of Korea since 2008. This study evaluateda low-dose propranolol use on IH based on its clinical efficacy and adverse event profiles. Total of 59 children, 3-year-old or younger, diagnosed IH and took propranolol as a maintenancedosage of 2 mg/kg/day between November 2009 and July 2011, were culled from SeoulNational University Children’s Hospital using electrolyte medical records retrospectively. Forcomparison, clinical photos were taken before, during and after treatment. IH was predominantly presented in female (Male: Female=1:2.5), 39% of which occurred and wasdistinguished at birth (n=23). For the remains (n=36), 69% of children (n=25) occurred within amonth. Location of the IH was as follows: face and neck (n=44), trunk (n=4), the limbs (n=6), multiples (n=5), respectively. Mean age of treatment initiation was 5-month-old (range 1.3-16)and the total duration of treatment was an average of 8 months (range 2.3-22). 22% of patientsshowed a rebound growth when the medication was discontinued and restarted propranolol,receiving relatively shorter courses; a mean of 4 months (range 1 week - 11 months). Based on theinitial diagnosis, more than 50% of the reduction in the size of lesion was exhibited at 48patients, followed by four with less reduction and no changes in seven. Total 59% of patients(n=36) displayed improvement within a month such as a regression in size, softening in the lesionand faded in color. 22 patients experienced minor but reportable side effects to propranolol,including rash (n=8), gastro-esophageal reflux (n=7), hypotension (n=3), somnolence (n=2), andothers (respiratory distress, n=1, hypothermia, n=1), respectively. In this study, the clinical efficacy of low-dose propranolol use on IH was confirmed. Due to thelimited data from Seoul National University Children’s Hospital, prospective studies with morepatients are needed and the guideline for duration and dosage of propranolol should be furtherelaborated.

        Mancozeb, a polymeric complex of zinc and manganese salts of ethylene bisthiocarbamate (EBDC), is used widely in agriculture as fungicides, insecticides, and herbicides Mancozeb can be occupationally and environmentally exposed to human and has been reported to induce estrogenic activity, therein it is considered as an endocune disrupter After female TCR mice were treated Mancozeb of ally at the doses of 250, 1,000 and 1,500 mg/kg/day for consecutive 30 day, we investigated the effects of Mancozeb on the immunopathological parameters (body-, thymus-, spleen-, liver- and kidny-weight, splenic cellularity, hematological parameters) and mitogen (Con A, LPS)-induced splenocyte proliferation (SP) Liver- and kidney- weight wete increased , but body- and thymus-weight, number of splenocytes and WBC were decreased, when compared with control group When splenocytes isolated from the mice exposed to Mancozeb for 30 days were cultured in presence of mitogens, the SP against Con A was significantly and dose-dependently decreased and the SP against LPS was also slightly decreased Our present results indicate that Subacute exposure of Mancozeb to mice might show immunotoxic effect.

        노인의 퇴원 시 당뇨병 약제 강화 현황 및 단기 임상결과에 미치는 영향 분석

        This study aimed to assess diabetes medication intensification at discharge in older adults hospitalized for non-glycemic reasons, examining its shortterm benefits and risks. Methods: A retrospective review of electronic medical records from Boramae Medical Center (September 1, 2020, to August 31, 2022) was conducted. Medication intensification was defined as either increasing the dosage of existing diabetes medications, adding insulin or initiating new medications. We compared glycated hemoglobin (HbA1c) levels 90 days post-discharge and the incidence of blood glucose-related emergency department visits or diabetesrelated unplanned readmission within 90 days post-discharge between patients with and without medication intensification. Results: Out of 1,278 patients, 480 (37.6%) underwent medication intensification at discharge. Factors associated with intensification included longer hospital stays, consultations with endocrinologist, higher HbA1c at admission, frequent hyperglycemic events, and changes in steroid or immunosuppressant use. The intensification group showed a significant reduction (8.6% to 7.0%) in HbA1c 90 days post-discharge compared to the non-intensification group (6.9% to 6.9%, p < 0.001). However, there was no significant impact on postdischarge emergency visits or unplanned readmissions related to blood glucose (aOR 0.34; 95% CI 0.07–1.53). Conclusion: A third of older adults admitted for nonglycemic issues was discharged with intensified diabetes medications, leading to improved short-term glycemic control but did not significantly affect diabetesrelated unplanned readmissions or emergency visits.       • 서울시 은평뉴타운 모기의 발생소장 및 모기 유충 발생원의 공간생태학적 특성

        This study was conducted to elucidate the occurrence area and the density of mosquito larvae in Eunpyeong Newtown, Seoul, Korea. Sampling of mosquito larvae was carried out 2 times using a dipping method at 130 points in august, 2014. A total of 1977.6 individuals were collected. Aedini larvae such as Stegomyia albopicta (Skuse) (=Aedes albopictus) or Hulecoeteomyia koreica (Edwards) (=Oclerotatus koreicus) were the most frequently collected than Culex spp. and the Anopheles spp. Mosquito larvae were surveyed in the street inlet (a rain drip box). The positive correlation of the larval density was found with the street inlet point with leaf mold and the depth of water. In the result of spatial statistics analysis, all points larvae collected were clustered and the tendency of spatial autocorrelation was apparent, but points over 20 individuals were dispersed. This result can be assumed that the mosquito larvae are able to occur to stagnant water in all places of this area. To control to mosquito occurrence, we think integrated methods are needed such as biological, ecological, chemical, physical control methods in various aspects.
