Intestinal Anastomosis by Use of a Memory-shaped Compression Anastomosis Clip (Hand CAC 30): Early Clinical Experience

        이학윤,Jin-Hee Woo,Si-Young Park,Nam-Wook Kang,Ki-Jae Park,Hong-Jo Choi 대한대장항문학회 2012 Annals of Coloproctolgy Vol.28 No.2

Purpose: The safety and the efficacy of the compression anastomosis clip (Hand CAC 30) have been demonstrated by animal studies. This study was designed to evaluate the clinical validity of the Hand CAC 30 in enterocolic side-to-side anastomosis after colonic or enteric resections. Methods: A non-randomized prospective data collection was performed for patients undergoing a side-to-side anastomosis using the Hand CAC 30. Eligibility criteria for the use of the Hand CAC 30 were for anastomoses between the colon and the ileum or between two small bowels. The primary short-term endpoint was the rate of anastomotic leakage. Other clinical outcomes, including intra- and postoperative complications, length of operation time and hospital stay, and the clip elimination time were recorded. Results: A total of 63 patients (male, 36) underwent an enteric or right-sided colonic resection followed by a side-to-side anastomosis using the Hand CAC 30. Laparoscopic surgery was performed in 36 patients, in whom one patient who underwent a laparoscopic right hemicolectomy was converted to an open procedure (1/32, 3.1%). One patient with ascending colon cancer showed postoperative anastomotic leakage and died of co-morbid ischemic heart disease. There were no other surgical mortalities. The exact date of expulsion of the clip could not be recorded because most patients were not aware of clip elimination. No patients manifested clinical symptoms of anastomotic stricture. Conclusion: Short-term evaluation of the Hand CAC 30 anastomosis in patients undergoing enterocolic surgery proved it to be a safe and efficacious alternative to the standard hand-sewn or stapling technique. A study on pressurizer cutting scenario for radiation dose reduction for workers using VISIPLAN

        이학윤,김선일,송종순 한국원자력학회 2022 Nuclear Engineering and Technology Vol.54 No.7

The operations in the design lifecycle of a nuclear power plant targeted to be decommissioned lead to neutron activation. Operations in the decommissioning process include cutting, decontamination, disposal, and processing. Among these, cutting is done close to the target material, and thus workers are exposed to radiation. As there are only a few studies on pressurizers, there arises the need for further research to assess the radiation exposure dose. This study obtained the specifications of the AP1000 pressurizer of Westinghouse and the distribution of radionuclide inventory of a pressurizer in a pressurised water reactor for evaluation based on literature studies. A cutting scenario was created to develop an optimal method so that the cut pieces fill a radioactive solid waste drum with dimensions 0.571 m 0.834 m. The estimated exposure dose, estimated using the tool VISIPLAN SW, in terms of the decontamination factor (DF) ranged from DF-0 to DF-100, indicating that DF-90 and DF-100 meet the ICRP recommendation on exposure dose 0.0057 mSv/h. At the end of the study, although flame cutting was considered the most efficient method in terms of cutting speed, laser cutting was the most reasonable one in terms of the financial aspects and secondary waste

A Study on the Construction of Cutting Scenario for Kori Unit 1 Bio-shield considering ALARA

        이학윤,이민호,양기태,안준열,송종순 한국원자력학회 2023 Nuclear Engineering and Technology Vol.55 No.11

Nuclear power plants are subjected to various processes during decommissioning, including cutting, decontamination, disposal, and treatment. The cutting of massive bio-shields is a significant step in the decommissioning process. Cutting is performed near the target structure, and during this process, workers are exposed to potential radioactive elements. However, studies considering worker exposure management during such cutting operations are limited. Furthermore, dismantling a nuclear power plant under certain circumstances may result in the unnecessary radiation exposure of workers and an increase in secondary waste generation. In this study, a cutting scenario was formulated considering the bio-shield as a representative structure. The specifications of a standard South Korean radioactive waste disposal drum were used as the basic conditions. Additionally, we explored the hot-to-cold and cold-to-hot methods, with and without the application of polishing during decontamination. For evaluating various scenarios, different cutting time points up to 30 years after permanent shutdown were considered, and cutting speeds of 1–10nullm2/h were applied to account for the variability and uncertainty attributable to the design output and specifications. The obtained results provide fundamental guidelines for establishing cutting methods suitable for large structures.

      • Look Around You! A Linguistic Landscape Project in Study Abroad Context in Korea

        이학윤(Hak yoon Lee),최범용(Bum yong Choi) 국제한국어교육문화재단 2018 국제한국어교육 Vol.4 No.1

(조지아주립대학교, 에모리대학교) This study explores how the concept of Linguistic Landscape (LL) is employed as a pedagogical tool in Korean study abroad program. Linguistic landscape is the visibility and salience of languages on public and commercial signs in a given territory or region (Landry and Bourhis, 1997, p.23), and it is a fast evolving research area. However, few studies are found in Korean language education. By illustrating how the concept of LL is applied to Korean as a second language context, this study fills this gap. This study is grounded on how the concept of regional competence (Watson et al. 2013) is relevant to a holistic view of linguistic landscape that is applied in the students’ analysis of different signs in Korea. 11 learners in a study abroad program in Korea participated in this project, and they investigated the complexity of the linguistic environment and multilingualism in Korea while they were in the study abroad program. Then they shared their pictures and analyses on Facebook as a platform for prompt exchanges of ideas. Focusing on the issues of translation, audience and context, and language selection, we analyze 1) students’ final presentation, 2) interaction in Facebook, and 3) reflection papers for this study.
