Endometriosis is a relatively common gynecologic disease. Thoracic endometriosis syndrome occupies half of various types of external endometriosis, however it is very rare condition. We experienced a case of 40-year-old woman who presented with recurrent episodes of spontaneous right-sided pneumothorax that occurred during her menstrual periods. After the localization of the disease site by means of chest radiography and high resolution computed tomography of the thorax, thoracoscopic assisted diaphragm endometriosis resection and suture were performed. After pathologic diagnosis, a hormonal treatment with gonadotropin-releasing hormone (GnRH) agonist was also conducted. We report this case with a brief review of the concerned literatures. 모체태아의학 : 제왕절개 후 발생한 괴사성 근막염 1예

Necrotizing fasciitis (NF) is a rare, rapidly progressive and potentially fatal soft-tissue infection characterized by widespread severe infection of the deep soft tissue, including fascia. Predisposing conditions for NF include diabetes, malignancy, obesity, and chronic liver disease. Patients with suspected NF should be empirically and immediately managed with broad-spectrum antibiotics covering the commonly suspected organisms. And surgical debridement is the mainstay of treatment of NF. We experienced a very rare case of NF with 33-year old healthy woman who presented with high fever, erythema, edema, and pustule on upper abdomen one day after cesarean section. NF was strongly suspected and immediate surgical intervention and broad spectrum antibiotics were used. We report it with a brief review of literatures. 탈북여성 및 결혼이주여성 건강 실태 조사

Purpose: This study attempts to understand the current health conditions of the North Koreanwomen defectors and the marriage immigrant women by conducting medical examinations to themin order to prepare better healthcare programs for the women. Methods: From July 2011 to February 2013, complimentary medical examinations were conductedfor eight times to the North Korean women defectors and the marriage immigrant women underthe support of International Aid Korea (IAK) and seven obstetrics and gynecology hospitals andclinics. The number of total examinee was 219 composed of 138 North Korean women defectorsand 81 marriage immigrant women. The study items included routine gynecological examinations,BMD (bone mineral density), test of hemoglobin level, hepatitis B antigen/antibody, Pap. smear,and sexually transmitted infections (syphilis, HIV, gonorrhea, chlamydia, trichomonas, HPV). Results: Anemia was diagnosed for 32.8% of the North Korean women defectors and 67% of themarriage immigrant women. The hepatitis B antigen positive rate was 11.8% for the North Koreanwomen defectors and 6.7% for the marriage immigrant women while the hepatitis B antibody positiverate was 80.3% for the North Korean women defectors and 67% for the marriage immigrantwomen. In the result of BMD, 48.6% of the North Korean women defectors and 46.9% of the marriageimmigrant women had osteopenia or osteoporosis. The result of Pap smear revealed abnormalfindings of 4.1% for the North Korean women defectors and 4.3% for the marriage immigrantwomen. HPV positive rate was 29.1% for the North Korean women defectors and 22.4% for themarriage immigrant women. In the tests for sexually transmitted infection, 4.5% of the NorthKorean women and 8.2% of the marriage immigrant women were positive to chlamydia and 4.5%of the North Korean women defectors and 2% of the marriage immigrant women were positive totrichomonas. Conclusion: There are 18,776 North Korean women defectors and 206,369 marriage immigrantwomen who are living in South Korea. Most of them are socially and economically disadvantagedand belong to vulnerable class who are in a lack of access to appropriate medical care. This was a fundamental study to understand the current health conditions of the North Korean women defectorsand the marriage immigrant women. Further studies with larger population and more developedresearch frameworks will provide not only meaningful health statistics of the North Koreanwomen defectors and the marriage immigrant women, but also give better direction for effectivehealth support programs for the underprivileged women. 한국은 1945년 남과 북으로 분단된 이후 독립적인경제적 성장을 이루면서 사회 제반 시설에 차이를 보이고 있고 의료 환경에서도 예외는 아니다. 남북은 같은인종으로 유전적 조건이 유사하고, 기후 및 지리적 조건에 크게 차이가 없음에도 불구하고 건강 실태 조사에서 차이를 보이고 있다. 또한 한국은 단일민족 국가임을 강조했던 과거와는 달리 놀라운 속도로 다문화 국가의 반열에 올라서고 있다. 탈북여성 및 결혼이주여성은대부분은 낮은 사회경제적 위치에 처해 있으며 의료 취약 계층으로 분류할 수 있다. 하지만 보건 의료 시스템과 사회적 인식이 인구 구조 변화의 속도를 따라가지못하는 실정이다. 본 연구는 탈북여성 및 결혼이주여성의 건강 상태를파악하기 위한 기초 조사이다. 향후 더 많은 대상과 더욱 체계적인 연구 틀을 만들어 조사한다면 탈북여성 및결혼이주여성의 건강과 관련한 유의한 통계자료 마련과더불어 이를 바탕으로 한 효율적인 탈북여성 및 결혼이주여성의 건강 지원사업의 방향을 제시하는데 도움이될 것이다.

목적: 본 연구는 탈북여성 및 결혼이주여성들을 대상으로 건강검진을 시행하여 이들의 건강 실태를 파악하여 여성건강 지원방안의 방향 설정에 도움이 되고자 한다. 방법: 2011년 7월부터 2013년 2월까지 인터네셔날 에이드 코리아 및 7개소의 산부인과 병·의원의 지원을 받아 총 8차에 걸쳐 탈북여성과 결혼이주여성을 대상으로 무료 건강검진을 시행하였다. 검진 대상자는 총 219명으로 탈북여성 138명, 결혼이주여성 81명이었다. 검진은 일반적인 산부인과 검진과 더불어 빈혈검사, B형간염 항원/항체 검사, 자궁경부암세포진 검사, 골밀도 검사, 성매개감염 검사 (매독, 에이 즈, 임질, 클라미디아, 트리코모나스, 인유두종 바이러스)를 하였다. 결과: 빈혈은 탈북여성의 32.8%, 결혼이주이성의 67%에서 진단되었다. B형간염 항원 양성률은 탈북여성에서 17.6%, 결혼이주이성에서 6.7%였으며, B형간염 항체 양성률은 탈북여성에서 32.8%, 결혼이주이성에서 67%였다. 골밀도 검사에서 탈북여성의 48.6%, 결혼이주이성 의 44.9%에서 골감소증 및 골다공증 소견을 보였다. 자궁경부암세포진 검사 결과 탈북여성의 4.1%, 결혼이주이성의 4.3%에서 비정상 소 견을 보였으며, HPV 검사는 탈북여성의 29.1%, 결혼이주이성의 23.4%에서 양성 소견을 보였다. 성매개감염의 경우 클라미디아 양성은 탈북여성의 4.5%, 결혼이주여성의 8.2%, 트리코모나스 양성은 탈북여성의 4.5%, 결혼이주여성의 2%에서 나타났다. 결론: 우리나라에는 2014년 6월 기준 18,776명의 탈북여성이, 2013년 1월 기준 206,369명의 결혼이주여성이 거주하고 있다. 이들 대부분 은 낮은 사회경제적 위치에 처해 있으며 의료 취약 계층으로 분류할 수 있다. 본 연구는 탈북여성 및 결혼이주여성의 여성 건강 상태를 파악하기 위한 기초 조사이다. 향후 더 많은 대상과 더욱 발전된 연구 틀을 만들어 조사한다면 탈북여성 및 결혼이주여성의 건강과 관련 한 유의한 통계자료 마련과 더불어 이를 바탕으로 한 효율적인 여성건강 지원사업의 방향을 제시하는데 도움이 될 것이다.

괴사성 근막염은 드물지만 빠르게 진행하며 높은 치사율을 보이는 연부조직 감염으로 근막을 포함한 심부 연부조직을 침범한다. 위험인 자로는 당뇨, 비만, 악성 종양, 만성 간질환 등이 있다. 의심되면 적절한 항균제의 즉각적인 투여가 필요하며, 수술적 괴사조직 제거가 주요치료로 사망률을 의미 있게 감소시킨다. 저자들은 평소 건강하던 33세 여성에서 제왕절개수술 1일 후 갑자기 발생한 고열, 홍반, 부종, 상복부의 농포를 주소로 전원된 환자에서 괴사성 근막염이 의심되어 조기 복합항생제 투여와 수술적 괴사조직의 제거로 치료한 매우 드문예를 경험하여 문헌고찰과 함께 보고하는 바이다. 난소갑상선종 1예

Struma ovarii is a rare, monodermal and highly specialized teratoma, composed entirely or predominantly (>50%) of thyroid tissue. Presenting symptoms are not specific. Despite containing thyroid tissue, only 5% of struma ovarii have features of hyperthyroidism. Therefore, preoperative diagnosis of struma ovarii is difficult. Recently, the authors experienced a case of struma ovarii found in a young woman who presented with known pelvic mass and dysmenorrhea. A transabdominal ultrasonography and computed tomography detected a 16-cm sized multiloculated mass in pelvic cavity. She underwent laparoscopic unilateral ovarian wedge resection. The final histopathologic diagnosis was struma ovarii of the mature cystic teratoma. Therefore, we report this rare case with a brief review of the literature.
