        Occupational Health Could be the New Normal Challenge in the Trade and Health Cycle: Keywords Analysis Between 1990 and 2020

        This brief report aims to establish the keyword content of studies on occupational health and safety-the key framework of the world of work in the trade and health domain. Data were collected from the SCOPUS database, focusing on articles on occupational health and safety and related keywords, with an emphasis on abstracts and titles. Data were analyzed and summarized based on keywords included from the MeSH database. There were 24,499 manuscripts in the domain and 1,346 (5.40%) occupational health-related keywords, including those that overlapped. The most frequently referenced occupational health-related keyword was "occupational health" (452 articles), followed by "occupational safety" (141 articles). There were fewer keywords on occupational health in the trade and health literature. As the world of work has been prioritized because of the recent new normal of work life since the COVID-19 pandemic, examining the focus of occupational health priorities within the global perspective is crucial.

        제조업 사업장의 안전보건활동이 안전보건수준 인식에 미치는 영향

        근로자의 건강을 보호하기 위해서는 안전보건수준을 향상시키는 것이 필요하다. 사업장의 안전보건수준은 다양한 안전보건활동을 통해서 향상될 수 있다. 사업장의 안전보건활동은 법적 규제에 대응하는 활동과 경제적 합리성에 근거해서 이루어지는 활동으로 구분될 수 있다. 본 연구에서는 제조업체의 다양한 안전보건활동 중에서 사업장의 안전보건수준에 대한 인식에 가장 큰 영향을 미치는 활동이 무엇이며, 위험도 수준에 영향을 미치는 활동이 무엇인지를 알아보았다. 본 연구에 활용된 자료는 2009년 산업안전보건연구원이 실시한 산업안전보건동향조사 자료이며, 이 중에서 제조업체 응답 자료만 선별하여 분석에 활용하였다. 사업장의 다양한 안전보건활동을 범주화하기 위하여 요인분석을 한 결과 사업주의 일반적인 안전보건경영활동, 근로자 건강관리 활동, 화학물질 관리, 보호구 및 유해위험기계기구관리, 및 정보전달과 의사소통 등의 5개 하위 활동으로 분류되었다. 5개로 분류된 안전보건활동은 모두 사업장의 안전보건활동 수준에 대한 인식에 통계적으로 유의한 영향을 미쳤으며, 이 중에서 사업주의 일반적인 안전보건경영활동의 영향이 가장 큰 것으로 나타났다. 반면 위험도에 대한 인식은 5%의 유의수준에서 보호구 및 유해위험기계기구 관리만 통계적으로 유의한 영향을 미치는 것으로 나타났다. 결국안전보건수준에 대한 인식과 위험도에 대한 인식이 서로 다른 차원으로 구성되고 있음을 보여주는 것이다. 본 연구는 단면조사 자료를 이용하였기 때문에 인과관계에 대한 해석에 한계를 지니고 있으며, 안전보건활동의 다양성을 충분히 포괄하지 못한 제한점이 있다. In order to protect workers` health it is needed to improve the level of occupational safety and health. The level of workplace safety and health can be improved through a variety of activities of occupational safety and health. Workplace safety and health activities can be divided into two categories such as response to the legal and regulatory activities and economically rational activities for profit making. Author investigated which activities have the greatest effect on awareness of workplace safety and health levels and workplace risk level. Materials utilized for this study was data from 2009 Occupational Safety and Health Trend Survey conducted by Occupational Safety and Health Research Institute. Using factor analysis the various activities of workplace safety and health were classified into five sub categories such as employer`s general safety and health management activities, employee health-related activities, management of chemicals, machinery protection and personal protective equipment management, and information delivery and communication. All of the activities of safety and health has had a significant impact on perceived level of workplace safety and health. Among five categories of activities, the employer`s general health and safety management showed the largest impact. Whereas awareness of risk was influenced by only activity of personal protection equipment and hazard machinery at 5% significance level. After all perceived level of health and safety and risk awareness are recognized differently by safety and health managers in workplace. This study has some limitation. Cross sectional survey data cannot be interpreted as causal relationship and surveyed activities about workplace safety and health cannot cover all kinds of activities.

        상업교사 교육을 위한 산업안전보건 과목의 필요성과 교육과정 개발에 관한 연구

        The purpose of this study lies in analyzing the current status of occupational safety and health education at business-oriented specialized vocational high schools and, through this, developing a curriculum regarding the occupational safety and health subject at business teacher training institutions. In order to achieve this, first, analyzed through the text mining technique were three types of textbooks for the subject “Successful Vocational Life,” which are representative textbooks used in occupational safety and health education at specialized vocational high schools. In addition, the subject “Labor Human Rights and Occupational Safety and Health” to be created newly through the Draft Plan for 2022 Revied Curricula for Vocational High Schools was analyzed, and Vocational High School Field Practice Occupational Safety Manual produced by the Ministry of Education and the Korea Certified Public Labor Attorneys Association (KCPLAA) was analyzed together as well. Second, analyzed were surveys of 50 current teachers at specialized vocational high schools and 51 prospective business teachers on the current status of the operation/management of occupational safety and health education at business-oriented specialized vocational high schools and perceptions of, demands regarding, and ideas for improving the new establishment of a curriculum regarding occupational safety and health education at teacher training institutions. Based on the results of the surveys, this study presents the following research results. First, this study developed a curriculum for the occupational safety and health subject to be applied to business teacher training institutions. The curriculum for the occupational safety and health subject consists of goals, contents, teaching and learning methods, and evaluations and, through them, describes the main contents of the occupational safety and health subject that business teacher training institutions must implement. Second, a syllabus for the occupational safety and health subject was created in addition, thus presenting a plan for concrete use. In the end, this study is significant for investigating the necessity of the occupational safety and health subject, which is currently the object of public attention, developing a new curriculum reflecting its characteristics, and also presenting a syllabus, which is a concrete execution plan for implementing the curriculum.

        Occupational Health: Meeting the Challenges of the Next 20 Years

        Background: The industrial revolution that took place in the United Kingdom (UK) between 1760 and 1830 led to profound social change. Occupational medicine was concerned with the diagnosis, treatment, and prevention of occupational diseases, that is, diseases directly caused by exposure to workplace hazards. A similar pattern of development has occurred globally. Methods: A review of relevant literature. Results: The international conceptualization and development of occupational health occurred during the 20th century. A new paradigm for occupational health has emerged that extends the classical focus on what might be termed "health risk management" that is, the focus on workplace hazards and risk to health to include the medical aspects of sickness absence and rehabilitation, the support and management of chronic noncommunicable diseases, and workplace health promotion. Conclusion: The future strategic direction for occupational health will be informed by a needs analysis and a consideration of where it should be positioned within future healthcare provision. What are the occupational health workforce implications of the vision for occupational health provision? New challenges and new ways of working will necessitate a review of the competence and capacity of the occupational health workforce, with implications for future workforce planning.

        산업간호사를 대상으로 한 구강보건교육의 효과 평가

        The purpose of this study was to evaluate effects of oral health education for occupational health nurses. The subjects were 300 occupational health nurses which participated in continuing education of Korean Association of Occupational Health Nurses. Oral health education contents consisted of basic knowledge about oral health, prevention of periodontal disease, oral health care for workers, and oral health program for workers. In order to evaluate the effects of oral health education, we performed questionnaire surveys before and after the education regarding their perceived oral health status and concern for oral health, knowledge about prevention of periodontal disease, attitude about oral health promotion, and needs for implementation of oral health promotion program. The data were analyzed by paired t-test to compare the change of knowledge and attitude according to the education. Linear regression analysis was carried out to assess the factors related to the improvement of their knowledge and attitude. The findings indicated that oral health knowledge and attitude of occupational health nurses were significantly improved by oral health education. A factor of the improvement of knowledge and attitude was concern for oral health. And they would like to be provided primarily oral health education for occupational health nurses. Finally, this study suggested that oral health education for occupational health nurses had significantly effects on improving oral health knowledge and attitude.

        우리 나라 보건관리대행서비스 평가 연구

        Group occupational health service programme started in 1990 is one of the measures to cope with limited human and financial resources in occupational health. The programme has expanded rapidly to include 52 institutions, private as well as public, all over the country. In spite of its potential impact on health of employees and practice of occupational health in small and medium sized industries, comprehensive evaluation in terms of quality has not been tried. This study has aims to develop the criteria to assess the quality of newly developed group occupational health service programme, and to investigate the quality of institutions, and finally to develop policies for the quality improvement. 1) Criteria development : By defining occupational health services, in particular for small and medium sized industries, as one of the primary health care, followings are included as core elements of qualitative occupational health programme; accessibility, continuity, comprehensiveness, technical quality, intersectoral collaboration, emphasis on preventive services, community participation, and adequacy. Again each element is divided into five major components of national health system infrastructure developed by the World Health Organization; development of health resources, organized arrangement of resources, delivery of health care, economic support, and management. In turn, each component is categorized into three aspects of quality assessment, structure, process and outcome. Expert panel selected several criteria for each category to evaluate the programme. Criteria were modified according to each group of interviewees, to produce two sets of questionnaire, one for chief operating officer and another for nurses in the institutions, and the chief operating officer and workers in the workplace. 2) Subject : Of all 52 institutions, 25 voluntarily participated in the survey. At individual institution, chief operating officer and practicing nurses were interviewed in depth. After intensive education for interviewees, every interviewees, every interview was performed with standardized guideline and questionnaire. The quality of the 'Group occupational health service programme' was found to be lower than expected. Especially in continuity, comprehensiveness, technical quality, community participation and adequacy, lower quality in structural aspect was commonly identified throughout all the institutions,. Quality in terms of accessibility and continuity highly varied among institutions., To improve quality of the programme, more comprehensive and systematic programme such as accreditation has to be introduced. In addition, human resources, governmental fund and information systems for individual workers are to be developed. As a long range plan, integration of occupational health services into the national health systems and pooling of financial resources and planned allocation should be considered.

        일부(一部) 사업장(事業場)의 산업보건(産業保健) 사업(事業)에 있어서의 근로자(勤勞者) 참여(參與)에 관한 조사(調査) 연구(硏究)

        The concept of workers' participation in occupational health was introduced to Korea recently in relation to primary health care in occupational health. But there is confusion and debate about workers' participation concept. The purpose of this study was to review the concepts of workers' participation and to conduct evaluation the workers' participation status in occupational health and safety, 394 workers and 54 employers (5.6%) in north area of Kyunggi-Do, were selected and interviewed with a questionnaire by a trained interviewer from August to September 1990. In general, the concept of workers' participation is based on industrial domocratisation and Declaration of Human Rights which had been powerful ideologies in labor movement. Contrary to workers' participation, community participation is rooted in the Health Rights. So, it is necessary to consider concept of workers' participation to improve participation. The results of survey were as follows. 1. Most of companies (71.75) carried out occupational health education to workers in study area. 2. The Occupational Safety and Health Committee (OSHC) were set up in 24.1% among the study companies, and 72.7% of workers among respondents thought that OSHC was helpful to workers health. 3. The workers signed his name to personal health report in 43.1% and the rate of participation in occupational environment examination was 54.9%. 4. The workers prefer the OSHC (39.3%), owner (35.1%) and union (25.8%) as a occupation health organisation, but owners prefer OSHC (54.5%), manager (43.2%) and union (2.3%). 5. Among the factors of the general characteristics, the existence of labor union was a major determinant of workers' attitude and level of workers' participation. As we have seen, most levels of workers' participation are low in occupational health. The variable of existence of labor union among the factors was a ma determinant of workers attitude and level of workers' participation. Therefore, in order to promote workers health, it is necessary to ponder long deeply on occupational health care system under the viewpoint of workers' participation.

        산업안전보건관리자 특성과 화학물질 유해성 정보전달의 관련성

        Objectives: The aim of the present study was undertaken to investigate the association between communication for chemical hazard information and characteristics of occupational safety and health managers. Subjects and Methods: We surveyed 78 occupational safety and health managers (64 male and 14 female) in 78 chemical manufacturing plants. Data were obtained using a self-reported questionnaire about size and type of company, products, communication system for chemicals and work-related characteristics of occupational safety and health managers. All analyses in this study were performed using SPSS program 12.0. Results: 64.1% of the study participants were occupational health managers aged 39.3 years on average and were graduated from college and university around 90%. 30.0% and of them were majoring in chemistry (engineering chemistry), 18.0% occupational health, 16% nursing and 30.0% others. Occupational safety managers were aged 39.4 years on average, 42.9% of them were majoring in chemistry (engineering chemistry), 21.4% environmental engineering, 10.7% occupational safety and 25% others. 86% of occupational health managers and 71.4% of occupational safety managers were classified as office job. Over 94% of the hazards information for chemicals were delivered by occupational safety and health managers, but about 28.2% workers preferred outside experts who have a profound knowledge about occupational safety and health and understandable, Occupational safety and health managers and workers had difficulties in understanding toxicological information, hazards identification, stability/reactivity, composition/information on ingredients, physical/chemical properties and ecological information. On multiple logistic regression analysis for the 16 heading of material safety data sheet, content of material safety data sheet was significantly associated with education level (odds ratio=0.286, 95% confidence interval=0.105-0.780). The hazard identification (odds ratio=3.947, 95% confidence interval=1.092-14.271) and toxicological information (odds ratio=0.841, 95% confidence interval=0.705-0.998) were significantly associated with type of occupation. Conclusions: This finding implies that the education level, type of occupation and speciality of occupational safety and health managers may affects hazards information delivery.

        How to Define the Content of a Job-Specific Worker's Health Surveillance for Hospital Physicians?

        Background: A job-specific Worker's Health Surveillance (WHS) for hospital physicians is a preventive occupational health strategy aiming at early detection of their diminished work-related health in order to improve or maintain physician's health and quality of care. This study addresses what steps should be taken to determine the content of a job-specific WHS for hospital physicians and outlines that content. Methods: Based on four questions, decision trees were developed for physical and psychological job demands and for biological, chemical, and physical exposures to decide whether or not to include work-related health effects related to occupational exposures or aspects of health reflecting insufficient job requirements. Information was gathered locally through self-reporting and systematic observations at the workplace and from evidence in international publications. Results: Information from the decision trees on the prevalence and impact of the health- or work-functioning effect led to inclusion of occupational exposures (e.g., biological agents, emotionally demanding situations), job requirements (e.g., sufficient vision, judging ability), or health effects (e.g., depressive symptoms, neck complaints). Additionally, following the Dutch guideline for occupational physicians and based on specific job demands, screening for cardiovascular diseases, work ability, drug use, and alcohol consumption was included. Targeted interventions were selected when a health or work functioning problem existed and were chosen based on evidence for effectiveness. Conclusion: The process of developing a job-specific WHS for hospital physicians was described and the content presented, which might serve as an example for other jobs. Before implementation, it must first be tested for feasibility and acceptability.

        The association between occupational stress level and health-related productivity loss among Korean employees

        OBJECTIVES: Occupational stress management is particularly important for successful business operations, since occupational stress adversely affects workers’ health, eventually lowering their productivity. Therefore, this study aimed to investigate the correlation between occupational stress and health-related productivity loss (HRPL) among Korean workers. METHODS: In 2021, 1,078 workers participated in a web-based questionnaire survey. HRPL was measured using the Work Productivity and Activity Impairment Questionnaire, and occupational stress was measured using the Korean Occupational Stress Scale-Short Form. The occupational stress level was divided into tertiles (low, intermediate, and high), and the low occupational stress group was used as the reference group. Using a generalised linear model, differences in labour productivity loss according to the level of occupational stress were tested after adjusting for demographic characteristics such as age, gender, education level, household income, occupation, and underlying medical conditions. RESULTS: Non-parametric regression analysis of HRPL according to occupational stress showed a direct association between occupational stress and HRPL. A statistically significant difference was observed in HRPL between participants with intermediate and high occupational stress and those with low occupational stress. CONCLUSIONS: Our results support the hypothesis that high occupational stress is associated with decreased labour productivity.
