Malignant peripheral nerve sheath tumor (MPNST) is defined as a malignant tumor arising from cells of the peripheral nerve sheath. The patients with neurofibromatosis type 1 (NF1) have the highest probability of developing MPNST. Even with wide surgical excision and adjuvant chemoradiation, the prognosis of MPNST is generally poor. We experienced a case of MPNST in a 38-year-old man with a long-standing NF1. He had a painless, infraauricular mass of 1 month duration. The patient underwent excision of the mass followed by total parotidectomy and modified neck dissection. Permanent pathologic sections confirmed MPNST. We report this case with a review of the literature.

고온용 원관형 히트파이프의 열적 특성에 관한 실험 및 모사

High-temperature cylindrical stainless steel-sodium heat pipes were fabricated and tested for transient and steady-state operations. Total length of the heat pipe was 1 m and the diameter was 25.4 ㎜. The thermal performance of each heat pipe was tested up to 2-㎾ thermal load (heat flux of 42 ㎾/m²). Thermal performance was investigated experimentally by changing internal wick structure as a function of the input thermal load and working fluid charge ratio. Thermal resistance and average heat transfer coefficients in evaporator and condenser sections were analyzed and discussed. Based on the experimental results, a commercial numerical tool was employed to simulate the characteristics of the high temperature heat pipe. Calculated temperature distributions of the heat pipe were compared with those of experiments. Effective thermal conductivity of a heat pipe was introduced in the modeling or performance estimation of which the values varied from 1,206 to 3,090 W/m ㆍ depending on the input thermal load.
항산화제 투여 후 총 항산화지수 및 사이토카인 변화와 스텐트 재협착에 미치는 영향

목적: 관상동맥질 환자에서 항산화 비타민 C와 E, 항산화제 probucol 병합요법이 스텐트 시술 후 혈중 P-selectin, chemokine, 사이토카인의 농도변화와 스텐트 재협착에 미치는 영향을 연구하였다. 방법: 관상동맥 질환으로 일반 스텐트 시술을 시행한 총 90명의 환자를 항산화제를 복용하지 않은 대조군과 비타민 C와 E, Probucol을 병합 투여한 군으로 나누었으며 이 중 6개월 후 추적관상동맥 조영술을 시행한 환자 71명을 대상으로 비교하였다. 항산화제 치료 전과 6개월 후 혈장내 전체 항산화지수, P-selectin, MCP-1, IL-6과 IL-10을 측정 비교하였고, 6개월 후 관상동맥 조영술을 시행하여 재협착 유무를 조사하였다. 결과: 스텐트 시술 전 임상적 소견, 위험인자, 생화학지표 등은 양 군 간에 유의한 차이가 없었으며 전체 항산화지수, P-selectin, MCP-I, IL-6, IL-10도 양 군 간 유의한 차이가 없었다. 6개월 후 추적검사에서 전체 항산화 지수 P-selectin, MCP-I, IL-6 및 IL-10 역시 양 군 간 유의한 차이가 없었고, 재협착 발생률도 양 군 간 유의한 차이가 없었다. 결론: 관상동맥 질환에서 일반 스텐트 시술 직후 비타민 C와 E, Probucol 병용투여는 혈장내 항산화 상태와 심혈 관의 죽상경화 진행에 관여하는 여러 가지 화학물질의 농도와 스텐트내 재협착 예방에 영향을 미치지 못했다. Background: Oxidative stress might be a role in atherosclerosis and increased intake of antioxidant appear to be protective and modify neointimal formation. An antioxidant and probucol prevents endothelial dysfunction and low density lipoprotein oxidation and also inhibits the secretion of cytokine by macrophages. We aimed 1) to study the effects of antioxidant (Vitamin C, E and probucol) supplementation on serum level of antioxidant status (TAS), P-selectin, MCP-1, IL-6 and IL-10 and 2) to investigate the effects of antioxidant intake on in-stent restenosis. Methods: Total 90 patients were assigned to control or antioxidant group (probucol; 500 mg, vitamin C; 1,000 mg, vitamin E; 400 mg). We performed follow up coronary angiography in 35 patients of antioxidant group and 36 patients of control group after 6 months of coronary bare metal stent implantation. We counted the stenotic lesions more than 50% of implanted stent lumen as a restenosis by quantitative coronary angiography. The serum levels of total antioxidant status, P-selectin, MCP-1, IL-6 and IL-10 were measured. Results: The serum levels of total antioxidant status was not elevated in antioxidant group. Antioxidant supplementation did not change the serum levels of P-selectin, MCP-1, IL-6 and IL-10. The 6-month angiographic in-stent restenosis rate was 27% versus 30% (p=NS) with an associated late loss of 0.76±1.01 mm versus 0.91±1.00 mm (p=NS) for antioxidant group and control group. The serum levels of total antioxidant status did not correlate with the restenosis or late loss after stent implantation. Conclusions: Vitamin C, E and probucol did not elevate the serum level of antioxidant status and could not prevent in-stent restenosis after bare metal stent implantation.(Korean J Med 71:158-165, 2006)
Largest Coding Unit Level Rate Control Algorithm for Hierarchical Video Coding in HEVC

In the new video coding standard, called high efficiency video coding (HEVC), the coding unit (CU) is adopted as a basic unit of a coded block structure. Therefore, the rate control (RC) methods of H.264/AVC, whose basic unit is a macroblock, cannot be applied directly to HEVC. This paper proposes the largest CU (LCU) level RC method for hierarchical video coding in a HEVC. In the proposed method, the effective bit allocation is performed first based on the hierarchical structure, and the quantization parameters (QP) are then determined using the Cauchy density based rate-quantization (RQ) model. A novel method based on the linear rate model is introduced to estimate the parameters of the Cauchy density based RQ model precisely. The experimental results show that the proposed RC method not only controls the bitrate accurately, but also generates a constant number of bits per second with less degradation of the decoded picture quality than with the fixed QP coding and latest RC method for HEVC.

<P>Variable-wavelength photodetectors are fabricated by a selective growth of Zn<SUB><I>x</I></SUB>Cd<SUB>1−<I>x</I></SUB>Se alloy nanowires on patterned Au catalysts thus forming nanowire air-bridges between two Pt pillar electrodes. From the composition-dependent linear changes of bandgap energies and lattice parameters, Zn<SUB><I>x</I></SUB>Cd<SUB>1−<I>x</I></SUB>Se nanowires are found to be perfectly alloyed in the entire range of Zn composition without any phase separation and have a structural transition from zinc blende to wurtzite at 0.31 < <I>x</I> < 0.72. The spectral responses of Zn<SUB><I>x</I></SUB>Cd<SUB>1−<I>x</I></SUB>Se detectors show that the cut-off wavelength can be continuously tuned within the visible spectrum of light extending from red to blue. In on-off switching operations, all of the detectors can be reversibly switched between the low and the high conductivity state but the CdSe detector has slower response and recovery behaviors than ZnSe and Zn<SUB>0.72</SUB>Cd<SUB>0.28</SUB>Se ones, which is explained by adsorption and photodesorption of O<SUB>2</SUB> on the surface of CdSe nanowires.</P> <P>Graphic Abstract</P><P>Wavelength-controlled photodetectors are fabricated by selectively growing Zn<SUB><I>x</I></SUB>Cd<SUB>1−<I>x</I></SUB>Se alloyed nanowires on patterned Pt pillar electrodes, in which nanowire/nanowire junctions act as the conducting path.
