        한국인 전상환자의 악안면결손에 대한 임상적연구

        Maxillofacial war injures is specific representative of severe hard and soft tissue defect. This type of injuries were different from the trauma because it may be fatal. The purpose of this study was to evaluate the injury type base on the retrograde medical record in the 104 patients from Feb. 1991 to Aug. 1992 in Korea Veterans Hospital. The obtained results were as follows. 1. Among 104 cases, 51 cases(49.0%) were classified as mandibular defects only, and 53 cases (51.0%) were classified as maxillary with mandibular defect (combined). 2. The etiologic factors of injury were gunshot, artillery and grenade or shell: 33 cases, 14cases, and 10 cases respectively in Korea War, 19 cases, 5 cases, and 8 cases respectively in Vietnam War. 3. In 57 cases of mandibular partial defect, the angle area showed the highest frequencies, 18 cases(31.6%). 4. In 42 cases of mandibular segmental defect, the area between the 1st molar and the ascending ramus showed the highest frequencies. 17 cases(40.4%), and almost all cases were unilateral defect (40 cases, 95.0%) 5. Reconstruction method performed for segmental mandibular defect were wire or plate(15 cases, 35.7%) and soft tissue closure only(12 cases, 28.7%), respectively.

        동종 탈회골의 크기가 유도골 형성에 미치는 영향에 관한 실험적 연구

        As early as 1889, treatment of ostemyelitis was reported using xenogeneic demineralized bone. In 1965, Urist discovered that demineralized long bone fragment, even when implanted in nonskeletal tissue, would stimulate osteogenesis. The clinical use of demineralized bone of Oral and Maxillofacial surgery is not new. The demineralized bone implants were used for 1) interposition within osteotomy gaps, cystic detects, alveolar clefts ; 2) augmentation, over intact bone surfaces ; 3) construction of new bone within soft tissue. Demineralized bone grafts invokes a induced osteogenesis which is transformation of host cells into osteoblasts. Demineralized bone has identified several factors that modulate the osteogeneic response : sterilization method, recipient age, particle size etc. Especially, pulverization of bone matrix may enhance its osteoinductive properties, to allow rapid, efficient bridging of large defects. the purpose of the present report was to describe the potential efficacy of demineralized allogeneic bone powder of skull of rabbits as a particle size ; 212 ㎛, 710㎛, 1 mm each other. Microscopic finding in our experimental studies shown that 710㎛ demineralized bone powder is the most potent osteogenic response, and then 212㎛, 1mm size. Densitometric analysis shown that density of all group was continue to increase until 4 weeks after operation, and then continue to decrease.

        냉동 건조 탈회 동종골 이식후 골막이 골 형성 과정에 미치는 영향에 관한 실험적 연구

        Periosteum in general is described as a specialized fibrous membrane of mesenchymal origin consisting of two basis layers : outer fibrous layer consists of irregularly arranged dense connective-tissue with fibroblasts, and inner osteogenic or cambial layer is composed of more loosely arranged fibers, greater vascularity and flatted spindle-shaped pre-osteoblasts. This periosteum may serve in controlling bone growth, especially mandibular growth has been emphasized. But, the periosteum enwrapping the facial skeleton have been studied for many years leaving a controversy in opinion regarding the function of these structures. We evaluated the bone formation activity of te periosteum in allogeneic bone grafts which bones are made of freeze-dried preparation preoperatively. We made the calvarial bone defects, 5×7mm sized, amd grafted with allogeneic bone in rats, which a half of specimens has dissected the overlying periosteum and a rest intacted. After bone grafting, we evaluated the capacity ofbone formation of periosteum, 1, 2, 4, 6, 8 weeks postoperatively. There are subtle differences of bone formation during early healing period after demineralized allogeneic bone grafting between control groups with periosteum and experimental groups without periosteum.

        오죽잎차와 오죽죽순차의 성분 분석 및 항산화 효과

        Nutritional components of Ojuk leaf tea and Ojuk shoot tea prepared from the leaves and shoots of black bamboo (Phyllostachys nigra Munro) by tea manufacturing process were evaluated. In addition, the extraction yield of water soluble components from these teas in the general tea brewing condition (water extraction in 80oC for 10 min.) and the contents of polyphenol and flavonoid were compared with not only the dried raw materials, but also green tea and mate tea. Finally, offline and online scavenging activities against 2,2`-azino-bis(3-ethylbenzothiazoline- 6-sulphonic acid) (ABTS) and di(phenyl)-(2,4,6 trinitrophenyl)iminoazanium free radicals were investigated to evaluate the antioxidant activity and explore the components showing ABTS free radical scavenging activity from tea infusion. These results demonstrated that these teas from black bamboo contain various nutritional components and can be used as traditional tea beneficial to human health.

        안면신경 마비 환자에 있어서의 측두근 및 근막피판을 이용한 안면근 기능 회복 증례보고

        Generally, the totally paralyzed face can never be made normal by any of the current methods of reconstruction. Careful selection of patients based on sound judgment of what can and cannot be achieved by the proposed surgical technique is paramount to a successful operation and a satisfied patient. The results are related to time of delayed between injury and repair : the shorter the delay the better are the results. The objectives in correcting facial paralysis are to achieve normal appearance at rest ; symmetry with voluntary motion ; control of the ocular, oral, and nasal sphincter; symmetry with involuntary emotion and controlled balance when expressing when expressing emotion ; and no significant functional deficit secondary to the reconstructive surgery. It must be employed a number of concepts, for treatment of the paralyzed face by surgeon, depending on the cause, time interval, and wound characteristics, as well as the availability of and necessity for neuromuscular substitution. Nerve grafts, crossovers, muscle transfers, free muscle and nerve-muscle grafts, micronuerovascular muscle transfers, and regional muscle transpostion are the principal methods being developed. We applied the tmporal musle transposition for reanimation of unilatrally paralyzed faces for long times on two patients. The results of muscle transposition can be enhanced by the patient's learning to activate the transposed muscle by voluntary effort, and are best in patients who are motivated to learn the necessary motor-sensory coordination techniques.

      • Use of Electroacupuncture at ST36 to Inhibit Anaphylactic and Inflammatory Reaction in Mice

        <P><I>Objective:</I> Electroacupuncture (EA) has been used to treat myalgia, allergy and gastroenteropathy in Korea. To determine whether EA can treat anaphylactic and inflammatory reactions, the effect of EA was investigated in a murine model. <I>Methods:</I> EA stimulation of the ST36 acupoint was performed for 10 min. Using a passive cutaneous anaphylaxis (PCA) model, the antianaphylactic effects of EA were examined. Interleukin-6 and tumor necrosis factor-α were measured using the ELISA method. The level of nuclear factor (NF)-κB/RelA protein and NF-κB DNA-binding activity was determined using the Western blot analysis and the transcription factor enzyme-linked immunoassay method. <I>Results:</I> EA inhibits PCA and β-hexosaminidase release, IL-6 secretion on the PCA, and in addition, EA reduces NF-κB DNA-binding activity. <I>Conclusion:</I> These results indicate that EA may possess antianaphylactic and antiinflammatory properties.</P><P>Copyright © 2007 S.       • The improving effects of Saengmaeksan on ulcerative colitis

        Saengmaeksan (SMS) is a well known Korean traditional herbal prescription, which consists of 3 different herbs, Radix Ophiopogonis, Radix Ginseng, and Fructus Schisandrae. SMS has been commonly used in various disease including respiratory system and cardiovascular System in Korea. The objective of this study is to find out the improving effects of SMS against Dextran Sulfate Sodium (DSS)-induced ulcerative colitis. SMS reduced clinical signs of DSS-induced colitis, including body weight loss, shorten colon length, and increased disease activity index. The results showed that SMS significantly inhibited the activation of nuclear factor-${\kappa}B$ p65 in the colon tissues of DSS-treated mice. In addition, we observed that result showed that the levels of IL-6 in plasma were increased in DSS treated group compared to those of the normal group, but these increased levels were reduced by administration with SMS. Taken together, these findings suggest that SMS has improving effects on DSS-induced ulcerative colitis, which may explain its beneficial effect in the regulation of chronic intestinal inflammation.

        Using Etomidate and Midazolam for Screening Colonoscopies Results in More Stable Hemodynamic Responses in Patients of All Ages

        Background/Aims: Recent studies have demonstrated that etomidate is a safe sedative drug with noninferior sedative effects. In our recent study, we revealed that etomidate/midazolam was more hemodynamically stable than propofol/midazolam in elderly patients undergoing colonoscopies. We aimed to investigate whether compared with propofol/midazolam, etomidate/midazolam causes fewer cardiopulmonary adverse events with noninferior efficacy for screening colonoscopies in patients of all ages. Methods: In this single-center, randomized, double-blind study, we prospectively enrolled 200 patients. The patients were divided into etomidate and propofol groups. The primary outcome was the occurrence of cardiopulmonary adverse events. The secondary outcomes were the proportion of patients with fluctuations in vital signs (oxygen desaturation and transient hypotension), adverse events interrupting the procedure, and sedation-related outcomes. Results: Adverse cardiopulmonary events were more common in the propofol group than the etomidate group (65.0% vs 51.0%, respectively; p=0.045). Forty-six patients (46.0%) in the propofol group and 29 (29.0%) in the etomidate group experienced fluctuations in their vital signs (p=0.013). The proportions of patients experiencing adverse events that interrupted the procedure, including myoclonus, were not significantly different between the two groups (etomidate: 20.0% vs propofol: 11.0%; p=0.079). Both groups had similar sedation-related outcomes. Multivariate analysis revealed that compared with the propofol groups, the etomidate group had a significantly lower risk of fluctuations in vital signs (odds ratio, 0.427; 95% confidence interval, 0.230 to 0.792; p=0.007). Conclusions: Compared with using propofol/midazolam, using etomidate/midazolam for screening colonoscopies results in more stable hemodynamic responses in patients of all ages; therefore, we recommend using etomidate/midazolam for colonoscopies in patients with cardiovascular risk factors.

        유도 결합 플라즈마를 이용한 TiN 박막의 식각 특성

        This study described the effects of RF power, DC bias voltage, chamber pressure and gas mixing ratio on the etch rates of TiN thin film and selectivity of TiN thin film to SiO₂ with BCl₃/Ar gas mixture. When the gas mixing ratio was BCl₃(20%)/Ar(80%) with other conditions were fixed, the maximum etch rate of TiN thin film was 170.6 nm/min. When the DC bias voltage increased from ?50 V to ?200 V, the etch rate of TiN thin film increased from 15 ㎚/min to 452 ㎚/min. As the RF power increased and chamber pressure decreased, the etch rate of TiN thin film showed an increasing tendency. When the gas mixing ratio was BCl₃(20%)/Ar(80%) under others conditions were fixed, the intensity of optical emission spectra from radical or ion such as Ar(750.4 ㎚), Cl?(481.9 ㎚) and Cl²?(460.8 nm) was highest. The TiN thin film was effectively removed by the chemically assisted physical etching in BCl₃/Ar ICP plasma.

      • Proinflammatory cytokine IL-1 β polymorphisms in sudden sensorineural hearing loss

        <P>The cause and pathogenesis of sudden sensorineural hearing loss (SSNHL) remain unknown. IL-1β is one of the most powerful inflammatory cytokines. The aim of this study was to evaluate the relationships between interleukin-1 β (IL-1β) gene polymorphisms (−511 C/T and +3953 C/T) in patients with SSNHL. One hundred two patients affected by SSNHL and 595 controls were genotyped for IL-1β gene polymorphisms. The polymorphisms were analyzed by polymerase chain reaction amplification and DNA fragment separation via electrophoresis. Compared to controls, the IL-1β (+3953) T allele increased the relative risk of SSNHL in subjects with IL-1β (−511) TT genotype (<I>p</I> = 0.022, OR = 9.111, 95% CI = 1.441-57.618). In this study, polymorphisms in the IL-1β −511 and IL-1β +3953 loci were assessed for evidence of association with SSNHL. From this assessment, a significant difference in carriage of both the IL-1β −511 T allele and the IL-1β +3953 T allele was observed between SSNHL and controls. This suggests that the IL-1β −511 and +3953 loci may play an important role in the etiopathogenesis of SSNHL.</P>
