        Background: Complementary and alternative medicine (CAM) is any practice that has healing effects, but is not based on evidence demonstrated by scientific method. Recently, CAM has been used in various diseases including acne. However, there have been no studies on CAM for acne in Korea. Objectives: The purpose of this study was to analyze the use of CAM in acne patients. Methods: A total of 159 patients with acne were enrolled on the study, and filled out a questionnaire about use of CAM. Results: Overall 87.4% (139/159) of the patients reported the previous or current use of at least one more type of CAM. Cosmetics for acne (100, 22.9%) was most frequently used, followed by diet therapy (81, 18.5%), spa and bath therapies (77, 17.6%), health food supplement (67, 15.3%), skin care shop (64, 14.6%), oriental medicine (38, 8.7%), and aromatherapy (9, 2.1%). The most common reason for using CAM was ``wish to try everything`` (28.6%), and the most common source of information was internet (40.5%). The therapeutic effect of CAM was best with diet therapy (32.1%). The most common side effect of CAM was aggravation of symptoms. The most common monthly cost for CAM was between 50,000 and 100,000 won/person. Conclusion: As our results, we can predict that the use of various types of CAM for acne will become more common. Therefore, dermatologists need to study about benefits and adverse effects of CAM for acne.

        The Longitudinal Trend of Cardiac Surgery in Korea from 2003 to 2013

        Background: The purpose of this study was to investigate longitudinal changes of the utilization of operational and surgical medical care inside and outside a metropolitan area over 10 years, analyzing the residential areas of patients and the locations of medical facilities for major cardiovascular surgery. Methods: Data analysis was conducted by classifying the addresses of patients and the locations of medical care facilities of metropolitan cities and provinces, using data from the National Health Insurance Corporation from January 2003 to December 2013. Results: There is serious concentration of major heart surgery to medical facilities in Seoul; this problem has not improved over time. There were differences in percentages of surgical procedures performed in the metropolitan areas according to major diseases. In the case of Busan and Daegu provinces, at least 50% of the patients underwent surgery in medical facilities in the city, but there are other regions where the percentage is less than 50%. In the case of provinces, the percentage of surgical procedures performed in medical facilities in Seoul or nearby metropolitan cities is very high. Conclusion: Policies to strengthen the regional capabilities of heart surgery and to secure human resources are required to mitigate the concentration of patients in the capital area. Many regional multi-centers must be designated to minimize unnecessary competition among regional university hospitals and activate a win-win partnership model for medical services.

        Comparison of the Regulatory Models Assessing Off-Site Radiological Dose due to the Routine Releases of Tritium

        삼중수소의 환경방출에 따른 주민선량의 규제이행을 위해 개발된 NEWTRIT모델, AIRDOS-EPA 모델, NRC 모델의 평가 방법을 고찰하고, 활용 가능한 국내 특성자료를 사용하여 예측결과를 비교하였다. 이들 모델 중에서 가장 최근에 개발된 NEWTRIT 모델만이 tritiated water (HTO) 방출에 따른 organically bounded tritium (OBT)의 영향을 고려한다. 평가결과 삼중수소의 환경방출로 인해 모든 가능한 경로로부터 받게 되는 총 피폭선량은 AIRDOS-EPA 모델의 예측결과가 NEWTRIT모델과 NRC모델에 비해 각각 1.03배, 2.46배 높은 결과를 나타냈다. 이러한 결과로부터 NRC모델로 예측되는 피폭선량이 실제 주변주민이 받을 수 있는 피폭선량을 과소평가할수 있다고 이해해서는 안될 것이다. 왜냐하면 삼중수소의 환경내 거동에 대한 불확실성은 매우 크기 때문에 규제이행을 위한 수학적 모델과 관련 변수 값은 극히 보수적 가정에 근거하기 때문이다. NEWIRIT 모델로 예측된 식품섭취에 의한 피폭선량에서 우리나라의 주식인 곡류의 상대적으로 많은 섭취로 OBT는 HTO와 거의 대둥한 수준의 영향을 나타내었다. 삼중수소의 환경방출에 따른 총 피폭선량에서 NRETRIT 모델은 AIRDOS-EPA 모델과 유사한 예측결과를 나타내지만, NEWTRIT 모델은 식품섭취에 따른 OBT의 영향을 고려함으로써 환경으로 방출된 HTO 거동의 현상적 이해 둥에 있어서 보다 의미가 있다고 판단된다. Methodologies of NEWTRIT model, NRC model and AIRDOS-EPA model, which are off-site dose assessment models for regulatory compliance from routine releases of tritium into the environment, were investigated. Using the domestic data, if available, the predictive results of the models were compared. Among them, recently developed NEWTRIT model considers only doses from organically bounded tritium (OBT) due to environmental releases of tritiated water (HTO) . A total dose from all exposure pathways predicted from AIRDOS-EPA model was 1.03 and 2.46 times higher than that from NEWTRIT model and NRC model, respectively. From above result, readers should not have an understanding that a predictive dose from NRC model may be underestimated compared with a realistic dose. It is because of that both mathematical models and corresponding parameter values for regulatory compliance are based on the conservative assumptions. For a dose by food consumption predicted from NEWTRIT model, the contribution of OBT was nearly equivalent to that of HTO due to relatively high consumption of grains in Korean. Although a total dose predicted from NEWTRIT model is similar to that from AIRDOS-EPA model, NEIIfTRIT model may be have a meaning in the understanding of phenomena for the behavior of HTO released into the environment.

        Peroxiredoxin‐1, a possible target in modulating inflammatory cytokine production in macrophage like cell line RAW264.7

        <P><B>ABSTRACT</B></P><P>Peroxiredoxin (PRX), a scavenger of H<SUB>2</SUB>O<SUB>2</SUB> and alkyl hydroperoxides in living organisms, protects cells from oxidative stress. Contrary to its known anti‐oxidant roles, the involvement of PRX‐1 in the regulation of lipopolysaccharide (LPS) signaling is poorly understood, possible immunological functions of PRX‐1 having been uncovered only recently. In the present study, it was discovered that the PRX‐1 deficient macrophage like cell line (RAW264.7) has anti‐inflammatory activity when stimulated by LPS. Treatment with LPS for 3 hrs resulted in increased gene expression of an anti‐inflammatory cytokine, interleukin‐10 (IL‐10), in PRX‐1 knock down RAW264.7 cells. Gene expression of pro‐inflammatory cytokines IL‐1β and tumor necrosis factor‐ α (TNF‐α) did not show notable changes under the same conditions. However, production of these cytokines significantly decreased in PRX‐1 knock down RAW264.7 cells with 12 hrs of stimulation. Production of IL‐10 was also increased in PRX‐1 knock down RAW264.7 cells with 12 hrs of stimulation. We predicted that higher concentrations of IL‐10 would result in decreased expression of IL‐1β and TNF‐α in PRX‐1 knock‐down cells. This was confirmed by blocking IL‐10, which reestablished IL‐1β and TNF‐α secretion. We also observed that increased concentrations of IL‐10 do not affect the NF‐κB pathway. Interestingly, STAT3 phosphorylation by LPS stimulation was significantly increased in PRX‐1 knockdown RAW264.7 cells. Up‐regulation of IL‐10 in PRX‐1 knockdown cells and the resulting downregulation of proinflammatory cytokine production seem to involve the STAT3 pathway in macrophages. Thus, down‐regulation of PRX‐1 may contribute to the suppression of adverse effects caused by excessive activation of macrophages through affecting the STAT3 signaling pathway.</P>

        Development of Prediction Equation of Diffusing Capacity of Lung for Koreans

        Background: The diffusing capacity of the lung is influenced by multiple factors such as age, sex, height, weight, ethnicity and smoking status. Although a prediction equation for the diffusing capacity of Korea was proposed in the mid-1980s, this equation is not used currently. The aim of this study was to develop a new prediction equation for the diffusing capacity for Koreans. Methods: Using the data of the Korean National Health and Nutrition Examination Survey, a total of 140 nonsmokers with normal chest X-rays were enrolled in this study. Results: Using linear regression analysis, a new predicting equation for diffusing capacity was developed. For men, the following new equations were developed: carbon monoxide diffusing capacity (DLco)=-10.4433-0.1434${\times}$age (year)+0.2482${\times}$heights (cm); DLco/alveolar volume (VA)=6.01507-0.02374${\times}$age (year)-0.00233${\times}$heights (cm). For women the prediction equations were described as followed: DLco=-12.8895-0.0532${\times}$age (year)+0.2145${\times}$heights (cm) and DLco/VA=7.69516-0.02219${\times}$age (year)-0.01377${\times}$heights (cm). All equations were internally validated by k-fold cross validation method. Conclusion: In this study, we developed new prediction equations for the diffusing capacity of the lungs of Koreans. A further study is needed to validate the new predicting equation for diffusing capacity.

        Pathogens of peritoneal dialysis peritonitis: Trends from a single-center experience over 15 years

        Background: Concerns are increasing about the emergence of pathogens with antibiotic resistance in peritoneal dialysis (PD) peritonitis. We investigated the current pathogen trends and risk factors in PD peritonitis. Methods: We conducted a retrospective study analyzing data from 643 patients who maintained PD over 3 months between January 2001 and December 2015. The isolated pathogens from PD peritonitis were compared between period A (2001-2008) and period B (2009-2015). Results: Among 643 PD patients, 252 patients experienced one or more episodes of PD peritonitis (total 308 episodes) during the median follow-up of 66 months. In both periods, gram-positive bacteria were the dominant pathogens (22.2% vs. 53.8%, P < 0.01). Gram-negative bacteria showed an increasing tendency in period B, but without statistical significance (17.0% vs. 23.7%, P = 0.15). The culture-negative rate was improved from 57% in period A to 18% in period B (P < 0.01). There was no increase in the prevalence of resistant pathogens such as methicillin-resistant Staphylococcus epidermidis (MRSE), Staphylococcus aureus (MRSA) and extended-spectrum beta-lactamase (ESBL)-producing Escherichia coli between periods A and B. Preserved residual renal function was associated with a lower risk of PD peritonitis (odds ratio, 0.53; 95% confidence interval, 0.31-0.88; P = 0.01). Conclusion: Over the past two decades, the pathogens of PD peritonitis have not significantly changed in Korea. Gram-positive organisms remained dominant, with S. epidermidis being the most common pathogen. Resistant bacteria such as MRSE, MRSA, ESBL-producing Gram-negative bacilli did not increase, but should be monitored.

        Forced Expression of Programmed Death-1 Gene on T Cell Decreased the Incidence of Type 1 Diabetes

        Programmed death-1 (PD-1) is a co-inhibitory receptor of the CD28/CTLA-4 family which is expressed on activated T cells and inhibits T cell activation after binding to PD-1 ligands. In animal models, PD-1 regulates autoimmune disease and induces tolerance in pancreas. In this study the effects of PD-1 on type 1 diabetes were examined using PD-1 transgenic mice (Tg). The incidence of autoimmune diabetes induced by multiple low dose of streptozotocin (STZ) was reduced in PD-1 Tg mice. Although the expression of CTLA-4, PD-1 and FoxP3, which are inhibitory molecules of activated T cells, is reduced only on STZ injected wild type (WT) mice, CD4, CD8 and regulatory T cell populations were not changed in all experimental groups. When splenocytes were re-stimulated in ex vivo, the production of IL-2 and IFN-${\gamma}$ and the T cell proliferation were increased in all STZ injected mice, but the increment rate was less in PD-1 Tg groups. Interestingly, macrophages were observed in splenocytes of STZ injected PD-1 Tg at somewhat lower level than macrophage in diabetic wild type mice. In this research, we found out that total numbers of T cell in the experimental groups are not changed, but T cell function is changed, and FoxP3 expression is decreased in pancreas and spleen of diabetesinduced groups. Macrophage frequency might also affects on type 1 diabetes. Although more experimental evidence needs to be provided, these results suggest that ligation of PD-1 and PD-L1/2 may have an effect on macrophages as well as does T cells.
