Melamine-induced nephrotoxicity is closely associated with crystal formation in the kidney caused by combined exposure to melamine (Mel) and cyanuric acid (CA). However, there are few dosage-finding studies for toxicological evaluation of chronic co-exposure to Mel and CA. The objective of this study was to investigate the possible mechanism by which a Mel and CA mixture lead to renal toxicity in rats. Mel and CA were co-administered to rats via oral gavage for 50 days. Nephrotoxicity was determined by measuring blood urea nitrogen (BUN) and serum creatinine (sCr) levels. Relative kidney weights were significantly increased in rats after co-exposure to Mel+CA (63/6.3 or 630/6.3 mg/kg) mixtures. BUN and sCr levels were significantly increased after Mel and CA co-exposure. Taken together, significant increase in KIM-1, NGAL, and calbindin levels were observed in the urine of rats exposed to Mel+CA (63/6.3 or 630/6.3 mg/kg) compared with the corresponding control group. Histological analysis revealed epithelial degeneration and necrotic cell death in the proximal tubules of the kidney after co-exposure to Mel+CA (63/6.3 or 630/6.3 mg/kg). Our data suggest that Mel-mediated renal toxicity may be influenced by CA concentrations in Mel-contaminated milk or foods.

Purpose: To evaluate the effect of adjuvant external beam radiation therapy (EBRT) on local failure-free survival rate (LFFS) for papillary thyroid cancer (PTC) invading the trachea. Materials and Methods: Fifty-six patients with locally advanced PTC invading the trachea were treated with surgical resection. After surgery, 21 patients received adjuvant EBRT and radioactive iodine therapy (EBRT group) and 35 patients were treated with radioactive iodine therapy (control group). Results: The age range was 26-87 years (median, 56 years). The median follow-up period was 43 months (range, 4 to 145 months). EBRT doses ranged from 50.4 to 66 Gy (median, 60 Gy). Esophagus invasion and gross residual disease was more frequent in the EBRT group. In the control group, local recurrence developed in 9 (9/35, 26%) and new distant metastasis in 2 (2/35, 6%) patients, occurring 4 to 68 months (median, 37 months) and 53 to 68 months (median, 60 months) after surgery, respectively. Two patients had simultaneous local recurrence and new distant metastasis. There was one local failure in the EBRT group at 18 months after surgery (1/21, 5%). The 5-year LFFS was 95% in the EBRT group and 63% in the control group (p = 0.103). In the EBRT group, one late grade 2 xerostomia was developed. Conclusion: Although, EBRT group had a higher incidence of esophagus invasion and gross residual disease, EBRT group showed a better 5-year LFFS. Adjuvant EBRT may have contributed to the better LFFS in these patients.

A psoas compartment block has been used to provide anesthesia for orthopedic surgical procedures and analgesia for post-operative pain. Currently, this block is advocated for relieving pain in the lower extremity and pelvic area resulting from various origins. We report a case of a 69-year-old male patient who had gait abnormality with posterior pelvic and hip pain, which were both aggravated by hip extension. From the magnetic resonance image, the patient was found to have a laterally herniated intervertebral disc at the L2/3 level, which compressed the right psoas muscle. This was thought to be the origin of the pain, so a psoas compartment block was performed using 0.25% chirocaine with triamcinolone 5mg, and the pain in both the pelvis and hip were relieved.

<P>We report the synthesis of KNbO<SUB>3</SUB> nanowires (NWs) with a monoclinic phase, a phase not observed in bulk KNbO<SUB>3</SUB> materials. The monoclinic NWs can be synthesized via a hydrothermal method using metallic Nb as a precursor. The NWs are metastable, and thermal treatment at ∼450 °C changed the monoclinic phase into the orthorhombic phase, which is the most stable phase of KNbO<SUB>3</SUB> at room temperature. Furthermore, we fabricated energy-harvesting nanogenerators by vertically aligning the NWs on SrTiO<SUB>3</SUB> substrates. The monoclinic NWs showed significantly better energy conversion characteristics than orthorhombic NWs. Moreover, the frequency-doubling efficiency of the monoclinic NWs was ∼3 times higher than that of orthorhombic NWs. Background OAT is a key enzyme that converts ornithine into proline, an essential precursor of collagen. We previously reported that OAT expression is increased in IPF lung and inhibition of OAT attenuate bleomycin-induced lung fibrosis. EMT is the main process for fibroblast production during lung fibrosis. We investigate the role of OAT in lung fibrosis especially focused on the EMT pathway and its clinical implication in IPF. Methods OAT knockdown A549 alveolar epithelial cell line was treated with TGF-β1. Morphological changes and expression of EMT-related markers, including E-cadherin, N-cadherin, and α-smooth muscle actin were evaluated. Expressions of Smad and non- Smad mediators and EMT- related transcription factors were measured. We measured the OAT level from bronchoalveolar lavage fluid (BALF) in IPF and controls. Results OAT knockdown inhibited the TGF-β1 induced change of the EMT phenotype. OAT knockdown attenuates the TGF-β1 induced increase in the phosphorylation of Smad 2 and 3 as well as that of ERK and p38 mitogen-activated protein kinase mediators. In addition, OAT knockdown reduced TGF-β1 induced expression of EMT transcription factors; Twist, Slug, and Snail 1. OAT levels in IPF BALF are reversely correlated with FVC (% pred.) and DLCO (% pred.). Conclusions Inhibition of OAT attenuates BLM induced lung fibrosis via suppressing of TGF-β1 induced EMT pathways. OAT lung level is associated with lung function decline in IPF. Taken together, regulation of OAT could be a therapeutic target and surrogate marker in fibrotic lung disease. [Grant :NRF-2019R1A2C100635]

Kaposi`s sarcoma (KS) is a locally aggressive endothelial tumor belonging to the family of vascular sarcoma. Four variants have been described: classic KS, endemic African KS, KS associated with immunosuppressive therapy, and AIDS-related KS. Up to recently, there are some controversies whether KS is a reactive proliferative disorder or a true malignant neoplasm and spontaneous remission of KS has been rarely reported. In previous literatures, spontaneous remission of KS has been reported in immune compromised patients. However, spontaneous remission of classic KS is uncommon. Herein, we report a case of classic KS which showed spontaneous remission, suggesting that KS may be a reactive disorder. A 77-year-old man presented with multiple purpuric nodules on the trunk and extremities for 3 months. No mucosal involvement was noted. Except for the history of angina pectoris and hypertension, he had no remarkable history such as immunosuppression. Laboratory findings including anti-HIV antibody were also unremarkable. Histopathological examination revealed slit like vascular spaces infiltrating the entire dermis and the vascular spaces were lined by spindle endothelial cells. With positive results of HHV-8 stain, the diagnosis of classic KS was made. However, at the 3-month follow-up while in the process of staging work-up, all skin lesions were spontaneously regressed without any specific treatment.
