This study is conducted to established integrated principal framework for the effects of the Hospital Information System(HIS) specially Physician Order Entry system(POE), Laboratory Information System(LIS), and Picture Archiving and Communication System(PACS) on the performance of medical institute. We had reviewed and analyzed the literature of POE, LIS, and PACS, and then formulated principal framework. In conclusion, the implementation of HIS could improve the satisfaction of medical staff and reengineer the work process, and those leaded improving the quality of service, and that also leaded increasing the number of patients and case mix of disease, and then those leaded increasing the performance of medical institute.

A pathology laboratory tool is developed to take the effective laboratory far medical students. The pictures of glass-slides used far the tool are digital data acquired by computerized 3 Charge-Coupled Device(CCD) camera attached research microscope. A graphic program is used for correction of image size, background color, and sharpness of the acquired original pictures. The students can easily find an exact picture by indices composed of number of glass-slides and diagnoses. The tool is suitable for review by computer monitor and reveals good quality for printing. Since the proper admixture of the pictures used fur tool and the newly acquired pictures is applied for the local area network(LAN)-based examinations, an effective laboratory is performed. I believe that this tool serve the students to understand the morphologic characters and to secure the time for the study of pathogenesis related to morphologic changes.

A Local Area Network-based examination is used for the effective examination of medical learning for medical students. This system is designed to execute the text with multimedia data. Three tests were performed. Each student used personal computer connected to Local Area Network(LAN) in the computer laboratory. The descriptive and multiple choice questions including related gross and microscopic images were delivered through Internet Explorer. Automatic scoring and analysis of the multiple choice problems were performed by Common Gateway Interface(CGI) program of Perl language. It reduced time to mark examination paper and to analyze the items. This method was successful to overcome the limitations inherent to the traditional evaluation methods. I believe that this LAN-based examination is one of the effective methods to evaluate the medical learning for the medical students.

In endometrial carcinomas, one of the most difficult problems in clinical management has been to identify women with low stage disease who have a substantially increased risk of developing recurrence. Numerous studies to find the prognostic indicator of endometrial carcinomas were performed, but little is known about the relationship between the overexpression of epidermal growth factor receptor(EGFR) and findings of flow cytometric DNA analysis. The immunohistochemical reactivity of antibodies against EGFR and flow cyromertic DNA analysis were examined in 19 endometrial carcinomas, and these finding were compared with histopathological findings and each other. Thirteen of 19 cases were immunohistochemical positive for EGFR(68%). The overexpression of EGFR did not correlate with histological grade, muscle invasion, DNA ploidy, DNA index, S-phase fraction, proliferative index. The cases of high DNA index(1.89 and 1.95) were histological grade 2 and 3 with deep muscle invasion. In this study, the overexpression of EGFR in endometrial carcinomas failed to show correlation with histopathologial findings and results of flow cytometric DNA analysis. But the high content of DNA is related to high histologic grade and deep muscle invasion, therefore it may present important prognostic information especially to patient with low clinical stage.

DNA analysis in gestation trophoblastic diseases was performed by flow cytometry in formalin-fixed, paraffin-embedded tissue. Hydatidiform moles were 15 case, invasive mole 6 cases, choriocarcinoma 14 cases (including 3 cases of merastatic choriocaricinoma) and mormal product of conception 9 cases as control. Hydatidiform moles were aneulploid(all multiploid). Choriocarcinomas were aneuploid in 3 cases(27%) out of 11 cases. Metastatic choriocarcinomas were all aneuploid ncluding I tetraploid and 2 multiploid. Normal products of conerption were all doploid. Hydatidiform mole were more aneuploidy and choriocarcinomas more diploidy. S-phase fraction, G_2M, and proliferative indec were mot significant between each gestational trophoblastic diseases. Cases of aneuploid were higher S-phase Fraction and proliferative index than those of diploid. Cases of diploid in gestational tropholbastic diseases and hydatidiform moles were higher S-phase fraction and proliferative index than those of normal product of conception, repectively These findings suggest that aneuploid, increased S-phase fraction and increased proliferative index were helpful for the diagnosis of gestational trophobalstic disease.

<P><B>Abstract</B></P> <P>Extensively drug-resistant (XDR) <I>Acinetobacter baumannii</I> strains have emerged rapidly worldwide. The antibiotic resistance characteristics of XDR <I>A. baumannii</I> strains show regional differences; therefore, it is necessary to analyze both genomic and proteomic characteristics of emerging XDR <I>A. baumannii</I> clinical strains isolated in Korea to elucidate their multidrug resistance. Here, we isolated new sequence type of XDR <I>A. baumannii</I> clinical strain (KAB03) from Korean hospitals and performed comprehensive genome analyses. The strain belongs to new sequence type, ST451. Single nucleotide polymorphism (SNP) analysis with other types of <I>A. baumannii</I> strains revealed that KAB03 has unique SNP pattern in the regions of <I>gyrB</I> and <I>gpi</I> of MLST profiles. <I>A. baumannii</I> KAB03 harbours three antibiotic resistance islands (AbGRI1, 2, and 3). AbGRI1 harbours two copies of Tn<I>2006</I> containing <I>bla</I> <SUB>OXA-23</SUB>, which play an important role in antibiotic resistance. AbGRI2 possesses aminoglycoside resistant gene <I>aph(3′)-Ic</I> and class A β-lactamase <I>bla</I> <SUB>TEM</SUB>. AbGIR3 has macrolide resistant genes and aminoglycoside resistant gene <I>armA</I>. <I>A. baumannii</I> KAB03 harbours mutations in <I>pmrB</I> and <I>pmrC</I>, which are believed to confer colistin resistance. In addition, proteomic and transcriptional analysis of KAB03 confirmed that β-lactamases (ADC-73 and OXA-23), Ade efflux pumps (AdeIJK), outer membrane proteins (OmpA and OmpW), and colistin resistance genes (PmrCAB) were major proteins responsible for antibiotic resistance.
