Testosterone Deficiency and Nocturia: A Review

Nocturia causes lack of sleep and excessive daytime somnolence, reducing overall well-being, vitality, productivity, and mental health. Nocturia is significantly associated with testosterone deficiency, lower urinary tract symptoms (LUTS), and sleep disorders. The development of LUTS is commonly associated with testosterone deficiency in elderly men, and recent studies have suggested that testosterone has an ameliorative effect on nocturia. In hypogonadal men with nocturia, a negative feedback cycle can arise, in which testosterone deficiency leads to the development of nocturia, and nocturia contributes to the decline in testosterone levels. Therefore, patients with nocturia should receive appropriate treatment in order to improve their quality of life. Nocturia is generally treated by restricting nighttime water intake, as well as by the administration of medications, such as alpha-1 blockers, anticholinergic drugs, and desmopressin. Testosterone replacement therapy (TRT) is used worldwide as a treatment for many hypogonadal conditions. TRT represents an alternative treatment option for nocturia in hypogonadal men. However, limited information is currently available regarding the effects of TRT on nocturia in hypogonadal men, and further studies are required to reach more definitive conclusions.
TT Virus Infection Among Blood Donors and Patients with non-B, Non-C Liver Diseases in Korea

Background/Aims : A novel virus, designated the TT virus (TTV), was isolated from the serum of a patient with posttransfusion hepatitis of unknown etiology, in Japan. Subsequently, TTV was suggested to be a causative agent in a proportion of cases with cryptogenic hepatitis in Japan. This study aimed to elucidate the significance of TTV infection in cases with cryptogenic liver disease in Korea, a neighbor of Japan. Methods : The prevalence of TTV infection was studied in 120 patients with liver diseases, including 85 patients diagnosed as having non-B, non-C liver diseases. As controls, 220 blood donors were also examined. TTV DNA was detected by polymerase chain reaction, and the sequence was analyzed by phylogenetic analysis. Results : Fourteen (14.0%) of 100 accepted blood donors, 23 (19.2%) of 120 rejected blood donors, and 15 (17.6%) of 85 patients with non-B, non-C liver diseases were positive for TTV DNA. The prevalences of TTV infection among these groups were not significantly different. Phylogenetic analysis suggested the existence of four major genotypes of TTV The proportions of each genotype among patients with non-B, non-C liver diseases were not different from those among accepted blood donors. Conclusions : TTV exists in Korea, but the prevalence among patients with non-B, non-C liver diseases was almost the same as that among blood donors. TTV may not be the main causative agent of cryptogenic liver disease in Korea. The relationship between non-B, non-C liver diseases and TTV genotype remains unclear, although TTV can be classified into four genotypes. (The Journal of Hepatology 30(3):389-93, 1999)
Prevalence and risk factors of hepatits C virus infection among Koreans in rural area of Korea

In a Korean rural area with a high incidence of liver cancer, a molecular epidemiological study was conducted to estimate the prevalence of hepatitis C virus (HCV) infection and to determine the distribution of HCV genotype and transmission routes of HCV infection, The study population is those who volunteered to participate in a health survey in 1993 and were over 10 years of age. Those who were anti-HCV-positive and 20 of their spouses were re-examined in 1995 and tested for HCV genotype. A phylogenetic tree was constructed after sequencing analysis of core and envelope regions from the subjects with genotype 2a without a blood transfusion history. The age- and sex-standardized prevalence of anti-HCV was 5.52%. In a multivariate analysis, transfusion history was not associated with anti-HCV-pos-itivity, but a history of acupuncture (adjusted odds ratio = 2.2, 95% confidence interval = 1.0, 4.7) and a history of surgical operation (adjusted odds ratio = 2.0, 95%, confidence interval = 1.0, 4.1) were associated. The prevalence of HCV genotype 2a was the highest, and genotype 1b was less frequent. The phylogenetic tree showed strong homology among our samples with Japanese HCV strains. The present study suggests that there is a highly endemic area of HCV infection in Korea and that this endemicity is probably associated with a parenteral source of HCV infection other than blood transfusion. The study also suggests that some of the HCV infection was spread through non-sterilized or non-disposable acupuncture needles in this HCV endemic
