Multivisceral organ transplantation is the final treatment option for children that have difficulty in absorbing sufficient enteral nutrition. Postreperfusion syndrome (PRS) is frequently seen in solid organ transplantation, and has a negative effect on graft and/or recipient outcome. Here, we describe a 30- month-old male toddler with chronic intestinal pseudoobstruction treated with multivisceral organ transplantation, including of the stomach, duodenum, pancreas, spleen, jejunum, ileum, and colon, but not the rectum. Immediately before multivisceral graft reperfusion, the recipient was pretreated with an intravenous bolus of phenylephrine (2.0 μg/kg) with increasing norepinephrine infusion. Pretreatment with phenylephrine and norepinephrine to prevent PRS reduced the severity and duration of severe hypotension and metabolic disturbance after graft reperfusion. As prolonged severe hypotension during graft reperfusion is associated with adverse outcomes, timely pretreatment with vasopressors before graft reperfusion is required to attenuate the development of severe PRS in pediatric multivisceral organ transplantation.
Visuomotor anomalies in achiasmatic mice expressing a transfer-defective Vax1 mutant

In binocular animals that exhibit stereoscopic visual responses, the axons of retinal ganglion cells (RGCs) connect to brain areas bilaterally by forming a commissure called the optic chiasm (OC). Ventral anterior homeobox 1 (Vax1) contributes to the formation of the OC, acting endogenously in optic pathway cells and exogenously in growing RGC axons. Here, we generated Vax1AA/AA mice expressing the Vax1AA mutant, which is incapable of intercellular transfer. We found that RGC axons cannot take up Vax1AA protein from the Vax1AA/AA mouse optic stalk (OS) and grow slowly to arrive at the hypothalamus at a late stage. The RGC axons of Vax1AA/AA mice connect exclusively to ipsilateral brain areas after failing to access the midline, resulting in reduced visual acuity and abnormal oculomotor responses. Overall, our study provides physiological evidence for the necessity of intercellular transfer of Vax1 and the importance of the bilateral RGC axon projection in proper visuomotor responses.
Optimum Design on Medical Screw Threads with Osseointegration

Medical screws are widely used in Orthopedics and Dental implants. The demanding property of medical screws is mechanical stability and biocompatibility. Though these properties are met, patients are still suffering and get revision surgery. This research focuses on Osseointegration of medical screws. Osseointegration; the structural connection between medical screw threads and living bone is more than just a natural phenomenon but a great indicator of surgery success. Current medical screws do not perfectly satisfy the osseointegration feature. The aim of this research is to analyze current Ti-based alloys of medical screws through ANSYS computer simulation, find which property is crucial in osseointegration, and make the optimized design of medical screw threads. This research concluded that when using innovative alloys and this optimized design, patients can reach osseointegration faster and reduce bone deletion volume.

Aims: In radiotherapy (RT) for hepatocellular carcinoma (HCC), little is known about pretreatment response prediction. Hypoxia of a tumor makes central necrosis, which is shown as consistent central enhancement defect (CED) in dynamic CT. We hypothesized that CED in pretreatment dynamic CT is a predictive factor of local failure after RT, in HCC. The purpose of this study was 1) to compare outcomes of RT in HCC with or without CED, and 2) to analyze blind reliability test for CED detection. Methods: We retrospectively reviewed 392 patients who underwent RT for HCC at a single center, from January 2010 to October 2010. Among them, we excluded the patients who were not eligible for the present analysis as follows: 1) patients who had infiltrative HCCs with vascular invasion; 2) patients who received combined therapy with other treatment within 3 months; or 3) follow up (among survivors) periods were less than 2 years. Finally, a total of 202 patients were included. Tumor characteristics on pretreatment dynamic CT, RT dose, and outcomes were measured. With treatment outcomes blinded, presence of CED was decided by 5 physicians’ agreements, and inter-observer reliability was tested. With tumor size and RT dose matched, 66 patients with or without CED were assessed by matched-pair analysis. Dose-response relationship was analyzed in both groups. Results: Median follow up duration was 30.5 months (range 5.6 - 64.7), and median RT dose (equivalent 2 Gy dose (EQD2)) was 88 Gy (range, 31.3 - 125 Gy). CED was present in 20.8% of all patients. Local control rate (LCR) and overall survival (OS) were significantly worse in patients with CED (both p< 0.001). On multivariate analysis, local recurrence was correlated with tumor size, RT dose, and CED (odd ratios = 4.1, 10.2, 45.5, respectively). In matched-pair analysis, LCR was 26.4% and 79.2%, and OS was 39.4% and 66.7%, in patients with or without CED, respectively, at 2 years (both p< 0.001). Inter-observer reliability of CED detection was 89.5% (p< 0.001). Hypofractioned RT with EQD2 > 70 Gy showed significantly better LCR in both patients with and without CED (p< 0.001, p = 0.039, respectively). Conclusions: In patients with HCC, a CED on pretreatment dynamic CT is a potent predictive factor for negative clinical outcomes, with good inter-observer reliability. To increase treatment outcomes in patients with HCC with CED, high-dose hypofractionated RT or other alternative treatment modalities should be considered.

토양 수분함량 변화에 따른 용적전기전도도 변화와 포화침출액전기전도도의 상관관계

간척농지 염농도 관리를 위해서 염농도 자동계측기 설치가 확대되고 있다. 자동계측기에서는 용적전기 전도도(Bulk electrical conductivity, EC<SUB>b</SUB>)가 측정되는데, EC<SUB>b</SUB>는 토양 수분에 크게 영향을 받는다. 따라서, EC<SUB>b</SUB>를 토양 수분함량에 독립적인 포화침출액 전기전도도(Saturated-paste extract EC, EC<SUB>e</SUB>)로 환산할 필요가 있다. 본 연구에서는, 실내 토조실험을 통해 사양토와 미사질 양토를 이용하여 EC<SUB>b</SUB>와 EC<SUB>e</SUB>의 상관관계를 구명하였다. 토조에 세 가지 수준의 염농도로 조정된 토양을 충진하고 염농도 센서(Teros12)를 설치한 후 수분을 과포화 상태로 조정하여 각각의 토양에 대해 약 2주간 실험을 수행하였다. 사양토의 EC<SUB>e</SUB>는 4.5, 7.0, 10.5 dS m<SUP>-1</SUP> 였고, 미사질양토의 EC<SUB>e</SUB>는 4.3, 7.8, 11.4 dS m<SUP>-1</SUP> 였다. 시간이 경과함에 따라 토양 수분 함량은 증발에 의해 약 0.2 cm³ cm<SUP>-3</SUP>로 감소하였다. 수분 함량이 과포화에서 약 0.3 cm³ cm<SUP>-3</SUP> 수준으로 감소하는 동안 EC<SUB>b</SUB>는 염 농축에 의해 증가하였지만, 그 이후부터는 수분 함량이 감소함에 따라 이온 활동도가 감소하여 EC<SUB>b</SUB>도 감소하였다. EC<SUB>b</SUB>가 EC<SUB>e</SUB>에 가장 근접하게 측정되는 수분함량은 토양 염농도 수분별로 상이하였는데, 사양토의 경우 ECe는 4.5, 7.0, 10.5 dS m<SUP>-1</SUP>에서 각각 0.39, 0.39, 0.28 cm³ cm<SUP>-3</SUP> 였고, 미사질양토의 경우에는 ECe는 4.3, 7.8, 11.4 dS m<SUP>-1</SUP>에서 각각 0.48, 0.62, 0.50 cm³ cm<SUP>-3</SUP> 였다. 해당 수분 함량에서 EC<SUB>b</SUB>와 EC<SUB>e</SUB>는 매우 강한 상관관계를 보였는데, 회귀식은 사양토와 미사질양토가 각각 EC<SUB>b</SUB>=EC<SUB>e</SUB> 0.80 x + 1.3, EC<SUB>b</SUB>=EC<SUB>e</SUB> –0.27 x² + 4.7 x - 9.8 였다. 본 연구결과는 염농도 자동계측기에서 측정하는 EC<SUB>b</SUB>가 토양 수분함량이 높을 때는 염농도 희석, 그리고 수분함량이 낮을 때는 이온 활동도 감소에 의해 크게 변하는 것을 보여준다. 따라서, EC<SUB>b</SUB>가 EC<SUB>e</SUB>와 유사한 토양 수분 함량 조건에서만 EC<SUB>b</SUB>를 직접 활용할 수 있는 것으로 나타났다. 따라서, 향후 염농도 자동계측기의 활용도를 높이기 위해서는 토양수분함량에 따른 EC<SUB>b</SUB> 변화를 정량화하여 토양수분함량의 영향을 제거할 수 있는 방법 개발이 필요하다.

기술의 발전은 사회에 많은 영향을 끼친다. 메타버스 시대가 다가오며 긍정적인 영향과 동시에 그에 따른 부작용도 발생하고 있다. 이에 과학기술정보통신부는 메타버스 윤리 원칙 초안을 마련하며 사용자들이 지향할 가치와 실천 방안을 안내하고 있다. 본 연구의 목적은 메타버스에서 발생하는 윤리적 문제 사례들을 분석하여 그 해결책을 제시하는 것이다. 이를 위해 현재 발생하고 있는 사례들을 조사하여 성격에 따라 6가지 영역으로 분류하였다. 1) 성적 자율성, 2) 폭력, 3) 금융, 4)보안과 정보, 5)정체성, 6)중독과 과의존 영역의 사례들을 조사한 후 윤리적으로 갖는 의미와 현재 시행되고 있는 해결책들을 살펴보고 그 한계점들을 다루었다. 마지막으로 대안책을 제시함으로 메타버스 상의 윤리적 문제가 최소화되며 우리 사회에 안착되도록 기여하고자 한다. The development of technology has a lot of influence on society. As the metaverse era approaches, positive effects and side effects are occurring at the same time. In response, the Ministry of Science and ICT is preparing a draft of the Metaverse Ethics Principles and guiding users on values and practical measures to pursue. The purpose of this study is to analyze cases of ethical problems occurring in metaverse and present solutions. To this end, the cases currently occurring were investigated and classified into six areas according to their characteristics. 1) sexual autonomy, 2)violence, 3)finance, 4)security and information, 5) identity, 6)addiction and overdependence, we looked at ethical implications and current solutions and addressed their limitations. Finally, by suggesting alternative measures, we intend to contribute to minimizing ethical problems on the metaverse and settling in our society.
