      • Long second toe: its nail and skin changes

        Background: Nail can be affected by trauma, infection, and systematic disorders. In particular, the toe nails are exposed to the repeated and cumulative minor traumas, which usually result in thickened, splitted, and discolorated nails. Considering these, it can be inferred that the long second toe can be related to various nail and skin changes by repeated minor traumas such as friction in shoes, but the study or report on this topic hasn’t been done yet. Objectives: The purpose of this study was to introduce various nail and skin changes on long second toe. Methods: We analyzed the kinds of nail/skin changes of 50 patients with long second toe, enrolled at Pusan National University Hospitals (Busan and Yangsan) from 2013 to 2018. Results: There was no significant difference in the incidence of left and right toe and toenail, and 7 (14%) patients presented bilateral distribution. The most common nail change on long second toe was melanonychia (23, 46%), followed by subungual hematoma (15, 30%), onycholysis (2, 4%), subungual hyperkeratosis (2, 4%), onychomadesis (1, 2%), and retronychia (1, 2%). The most common skin change was corn (9, 18%), followed by paronychia (2, 4%). Twenty (20/50, 40%) patients also had other toe or toenail changes. Among them, the most common site was big toe/nail (13, 62%), and the most common change was subungual hematoma (8, 42%). Conclusion: Through this study, the dermatologists can recognize and diagnose the nail and skin changes of long second toe.

      • Epigenetic regulation in bladder cancer: development of new prognostic targets and therapeutic implications

        <P>Epigenetic and genetic alterations contribute to cancer initiation and progression. Epigenetics refers to heritable alterations in gene expression without DNA sequence changes. Epigenetic changes include reversible alterations in DNA methylation, chromatin modification, nucleosome positioning, and non-coding RNA profiles. In bladder cancer, many epigenetic changes have been reported to exhibit correlation with cancer progression and malignances. In particular, both DNA hypermethylation and hypomethylation have been reported to be associated with higher bladder cancer rates or advanced tumor stages. The association of changes in histone modifying enzyme expression with bladder cancer stages has been extensively studied. MicroRNA (miRNA) expression also has been studied in bladder cancer diagnosis and disease progression. The use of epigenetics for the diagnosis of and therapeutic target screening in bladder cancer is an emerging and rapidly evolving field. The reversible nature of epigenetic changes may facilitate additional therapeutic options in the future.</P>

      • Dermoscopic findings of metastatic skin cancers: a single center experience

        Background: Though skin metastasis is rare, it can seriously affect the prognosis and management. The role of dermoscopy in primary skin cancers are established well, but its role in skin metastasis hasn’t been clear. Objectives: This study was performed to elucidate the dermoscopic patterns of metastatic skin cancers. Methods: We analyzed the dermoscopic patterns and clinical data of 46 patients with metastatic skin cancers at Pusan National University Hospitals (Busan and Yangsan) from 2005 to 2018. Results: Mean age and disease free interval were 62.0±14.2 years and 29.2±41.2 months, respectively. The most common primary origin of cancer was breast (11/46, 23.9%), followed by lung (8/46, 17.4%), and upper gastrointestinal tract (8/46, 17.4%). Any dermoscopic patterns was not found in 8 cases, who showed subcutaneous nodules without surface change. Most common dermoscopic pattern was vascular pattern (30/46, 65.2%), followed by homogenous structureless pattern (16/46, 34.8%). Most common vascular pattern was serpentine and arborizing vessel (23/46, 50%), followed by dotted vessel (7/46, 15.2%). There was no significant difference in dermoscopic vascular pattern among cancers, because the number of subjects was insufficient. Conclusion: Based on our study, when vascular patterns such as serpentine and arborizing vessel or dotted vessel are seen on dermoscopy within a nodule in a patient already diagnosed with cancer, metastatic skin cancer should be strongly considered.

        또래학습을 통한 또래교사와 또래학습자의 인지구조 변화 -초등 도형영역에 대하여-

        본 연구는 또래교수 활동을 통하여 변화된 도형의 개념 및 성질, 도형간의 관계에서 인지구조를 관찰하고 어떻게 정교화 해 가는지에 대해 또래교사와 또래학습자의 인지 구조의 차이를 알아보았다. 연구 문제를 해결하기 위해 보은의 S초등학교 5학년 학생 6명을 대상으로 또래교수와 학습자 3팀을 선정하고 도형영역에 대한 사전·사후검사를 실시하고 자료를 분석하였으며 심층면담을 실시하여 인지구조를 분석하였다. 또래교사와 또래학습자 모두 도형영역에서 인지구조가 변화되었으며 특히 또래교사의 인지구조의 변화가 더욱 뚜렷이 나타났다. 따라서 또래교수활동은 체계적인 계획하에 일관성 있는 교수활동 뿐만이 아니라 학습활동에 더욱더 많은 교사의 지속적인 노력과 지원이 필요하다. The purpose of the study is finding the effective teaching and learning methods on the concepts of figures through exploring the change of students` cognitive structures before and after the peer teaching activities. The difference of the peer teacher`s and student`s cognitive structures was investigated for the activities. Three teams, six students of 5th grade, were selected from the S elementary school in Boyeon. To figure out the students` cognitive structures, pre and post in-depth interviews were conducted and analyzed. Both peer teachers` and learners` cognitive structures were changed. Peer teachers` cognitive structures were changed more positively than peer learners. A consistent systematic planation and continuous teacher support and effort are needed for the activities.

      • Morphological Changes of Bones and Joints with Rheumatoid Arthritis and Osteoarthritis

        Arthritis is a common disease in aged people, and is clinically divided into rheumatoid arthritis (RA) and osteoarthritis (OA). Although common symptoms such as pain are present, the underlying pathological mechanisms are slightly different. Therefore, the objectives of the present study were to compare joint damage induced by RA and OA by analyzing the major morphological and molecular differences, and to propose a suitable therapeutic intervention based on the pathophysiological conditions of bones and joints. For the RA animal model, 8-week-old DBA1/J mice were immunized with bovine type II collagen emulsified in complete Freund's adjuvant (CFA). Normal C57BL/6 mice (over 2 years of age) were used for OA. The clinical arthritis score was calculated using a subjective scoring system, and paw thicknesses were measured using calipers. The serum TNF ${\alpha}$ level was analyzed using an ELISA kit. Micro-CT was used to identify pathological characteristics and morphological changes. In collagen-induced RA mice, there were increased ankle joint volumes and clinical scores (p<0.01). The concentration of TNF ${\alpha}$ was significantly increased from 3 to 7 weeks after immunization. Micro-CT images showed trabecular bone destruction, pannus formation, and subchondral region destruction in RA mice. OA among aged mice showed narrowed joint spaces and breakdown of articular cartilage. This study suggests that a careful therapeutic intervention between RA and OA is required, and it should be based on morphological alteration of bone and joint.

        자율주행자동차 운행을 위한 행정규제 개선의 시론적 고찰 -자동차, 운전자, 도로를 중심으로-

        현재 자율주행자동차는 상용 단계가 아니다. 향후 상용화의 관건은 기술적인 것 보다는자율주행자동차의 운행에 대비한 법제에 있다고 할 것이다. 자동차의 운행은 다양한 이해관계를 발생시키는 것이기에 자동차 사고시 책임법제는 물론이고, 운행의 기반이 되는 자동차·운전자·도로 법제도 필요하다. 향후 자율주행자동차 시대에는 ``사람`` 에서 ``자동차`` 로그 주도권이 이동될 것으로 예상이 된다. 따라서 현재 사람 중심의 규제방식을 자동차 중심의 규제방식으로 어떻게 변화시킬 것인가 하는 문제가 중요하다. 자동차의 규제방식은 그 성격상 대물적 행정행위 측면에서 근본 성격의 변화는 없을 것으로 보이고, 다만 자율주행자동차의 특성에 맞는 다양한 규제방식이 필요하다. 특히, 사고시의 책임 소재와 관련하여, 「자동차관리법」이 중요한 역할을 할 것으로 기대된다. 운전자에 대하여는, 오로지 운전자 위주의 면허제도와 교통규칙 등 규제방식이 차량을대상으로 그 중점이 이동하는 방식으로 변화할 가능성이 있다고 할 것이다. 도로에 대하여는, 현재 보다는 도로의 차선, 표지, 신호기 등 도로시설이 차량에 미치는영향이 훨씬 증대될 것으로 예상이 된다. 특히 이러한 점을 고려하여 제반요소를 새로운도로 설치기준으로 반영되어야 할 것이다. 그 외에도, 자동차, 사람, 도로 간의 정보의 교환과 네트워크의 보안 등의 중요성이 증대될 것으로 예상이 되는바, 이는 자동차 운행의 제4의 요소가 될 것이다. 이런 상황에서 우리나라도 선제적으로 자율주행자동차의 운행을 위한 자동차법제, 도로법제, 운전자법제의 개편에 대한 논의가 본격적으로 이루어져야 한다. Autonomous vehicles has not reached the commercial phase. The key of commercialization will be the legal system rather than technical issues. Since the operation of car incurs various interest, it is necessary to prepare road laws of the car driver as well as responsibility laws in automobile accidents. Therefore, the importance lies on how to move a direction from the ``people`` to ``auto`` in future autonomous driving car era. The regulation on the vehicle is administrative action against a thing so the principles of it is not expected to change. However, various regulation systems for operation of autonomous vehicles are required. In particular, the automotive management laws will play an important role in the accidents regarding responsibilities. For the driver, the focus of the regulations such as driver`s license and traffic rules is likely to move from the driver to the vehicle. For the road, the impact of road infrastructure such as the traffic lane, signs, signals on the vehicle will increase significantly compared to now. In particular, new installation standards for road autonomous driving car should be reflected. In addition, for the vehicle operation it is expected that the importance of the network security and the communication between vehicles, people and roads would be highly increase. Those will be the fourth element of operation of automobile. In conclusion, discussion on the reform of the legal system for autonomous vehicles should begin in earnest.

      • Seismic stratigraphic reconstruction of Plio-Quaternary depositional sequences on the continental shelf of Korea Strait

        <P><B>Abstract</B></P> <P>The Korea Strait is a seaway connecting the East Sea with the East China Sea. The Plio-Quaternary stratigraphy and sedimentation on the Korea Strait shelf was studied using a dense network of high-resolution, single-channel seismic reflection profiles. The shelf sequences above the acoustic basement consist of seven depositional sequences separated by erosional unconformities corresponding to falls in sea level. Individual sequences are characterized by well-stratified and complex sigmoid-oblique prograding reflection configurations, reflecting a history of upbuilding and outbuilding in response to repetitive transgressive and regressive events driven by sea level changes. During the regression to lowstand of sea level, lowstand prograding wedges were formed seaward along the present shelf margin, whereas during the highstand of sea level, as in the present condition, highstand deposits were deposited in the inner shelf along the coast. However, as the subsequent regression began, the inner-shelf highstand deposits were severely eroded out and/or formed a condensed section. Volumetrically, the forced regressive and lowstand systems tracts are the most important components of the Korea Strait. Consequently, the Plio-Quaternary sequence in this area consists of a succession of stacked progradational lowstand wedges, mainly formed during the regression and lowstand of sea level.</P>

        In this paper, we propose a method, an optimized architecture of a device with an image signal process of a field unit to minimize the flick phenomenon that happens in direction of a color temperature at a color tone change. The proposed IIR loop filter has an optimized architecture and reduced hardware compared with previous filters. In order to achieve the optimization for the hardware complexity. It is designed by time-multiplexing architecture. The proposed IIR loop filter is synthesized by suing the STD90 0.35um cell library.

        원문 : 좌업 시간과 알코올 섭취가 30대 남자 사무직 근로자의 혈압과 혈관탄성에 미치는 영향

        조봉오, 이인녕, 정균근, 장창현. 좌업 시간과 알코올 섭취가 30대 남자 사무직 근로자의 혈압과 혈관탄성에 미치는 영향. 운동과학, 제23권 3호, 241-248, 2014. 본 연구의 목적은 일과 중 대부분을 좌업근무 형태로 업무를 수행하고, 스트레스로 인해 음주가 잦고, 운동이 부족한 30대 남자 사무직 음주자 15명을 대상으로 좌업 시간과 알코올 섭취가 혈압과 혈관탄성에 미치는 영향을 분석하여 사무직 근로자에게 건강관리에 필요한 정보를 제공하는데 있다. S대학 운동 생리학 실험실에서 사전 인적 사항과 설문을 작성하고, 신장, 체중, 채혈, 혈압, 혈관탄성, BMI를 측정하였다. 알코올 섭취 횟수, 섭취량, 좌업 시간, 그리고 음주 섭취 전과 후 및 시간대별 (10분, 40분, 60분) 혈압과 혈관탄성의 변화를 측정 분석한 결과를 요약하면 다음과 같다. 1) 평균 좌업시간이 9.87시간, 음주횟수 및 음주량은 월 평균 9.13회와 12.47잔 이였다. 2) 알코올 섭취횟수 및 섭취량에 따른 BMI, TC, LDL-C, HDL-C, 혈압과 혈관탄성 비교에서 알코올 섭취횟수에서 좌상지 혈관탄성은 통계적으로 유의한 차이를 나타냈다(p<.05). 3) 알코올 섭취 전, 후 비교 분석 결과 우상지와 좌상지의 혈관탄성에서 통계적으로 유의한 차이가 나타났다(p<.05). 4) 알코올 섭취 후 시간대별 우상지 혈관탄성은 통계적으로 유의한 차이를 나타냈다(p<.05). 이상의 결과를 종합해 볼 때, 좌업 시간이 혈압과 혈관탄성에 미치는 영향은 통계적으로 유의하게 나타나지 않았고, 알코올 섭취는 혈관탄성에 통계적으로 유의한 차이를 나타냄을 알 수 있었다. 따라서 이는 직장생활의 스트레스를 음주로 해소하는 경향이 있음을 보여주며 건강에 부정적인 영향을 주고 있음을 알 수 있었다. 또한, 신체가 음주에 적응되어 실험 시 섭취한 적은 양(1/2병)은 1회 12.47잔에 비해 혈압과 혈관탄성에 큰 영향을 미치지 못한 것으로 사료된다. The purpose of this study was to examine effects of duration of sedentary office work and alcohol intake on blood pressure and vascular compliance in male subjects who were office workers aged between 30 to 39 and frequently drink alcoholic beverages to cope with their stress without exercising for health. The study was to provide health-related information necessary for white-collar workers. We collected data of demographic information, height, weight, blood sample, blood pressure, vascular compliance, and BMI from 15 subjects in S University`s Exercise Physiology Laboratory, and analyzed the frequency and amount of alcohol intake, duration of sedentary work, and changes in blood pressure and vascular compliance at diffident time points (10, 40, and 60 minutes post drinking) from baseline. I t was found that the average time of sedentary work was 9.87 hours, drinking 9.13 times a month on average, 12.47 glasses of alcohol beverages per drink. In comparison of BMI, TC, LDL-C, HDL-C, blood pressure and vascular compliance upon the frequency and amount of alcohol intake, the upper left limb`s vascular compliance had a statistically significant difference (p<.05). In the result of the comparative analysis of pre-and post-drinking, vascular compliance was significantly different in the upper right and left limbs (p<.05). Vascular compliance in the upper right limb after drinking significantly changed (p<.05). As a result, the duration of sedentary work had no statistically significant difference in blood pressure and vascular compliance. T he vascular compliance after drinking h ad a statistically significant difference. This suggested that the result was an index that shows the trend of the office workers releasing their work stress by drinking alcoholic beverages, and that drinking had negative impacts on them. In addition, a small amount of intake (a half bottle) at the test time, after the body was adjusted to drinking, compared with 12.47 glasses per intake, had no great impacts on blood pressure and vascular compliance.
