        A Concept Analysis of Gratitude in Patients Based on Rodgers' Evolutionary Method

        This article presents a concept analysis of the gratitude experience of diseased patients. The Rodgers' Evolutionary Method was used for conducting the analysis. A search of CINAHL, MEDLINE, PsycARTICLES, and Springer databases was conducted using "gratitude or appreciation or thanks" and "patient or illness" as a key word, 22 final articles were selected. Three critical attributes of gratitude in patients were identified: positive emotions, acceptance of the current status, and a driving force to plant the will of life. In addition, two antecedents of gratitude in patients were identified: interactions with people or the environment, and the perception of a favorable stimuli or help. Two consequences of gratitude in patients were identified: an increased compliance in implemented treatment, and an enhancement of trust relationship. The concept analysis describes diseased patients' gratitude. This paper will become the basis for future clinical research related to diseased patients' gratitude.

        Stimulatory heterotrimeric GTP-binding protein augments cisplatin-induced apoptosis by upregulating Bak expression in human lung cancer cells

        <P>The present study aimed to investigate the effect of the stimulatory heterotrimeric GTP-binding (Gs) protein signaling system on cisplatin-induced apoptosis of lung cancer cells and its underlying mechanism as an attempt to develop a novel strategy to improve the therapeutic efficacy of cisplatin. Overexpression of the constitutively active alpha subunit of Gs (GalphasQL) in A549 human lung cancer cells increased cisplatin-induced apoptosis, and knockdown of Galphas with small hairpin RNA decreased the percentage of apoptotic cells. GalphasQL increased the expression of the proapoptotic proteins B-cell leukemia/lymphoma-2 genes (Bcl-2) homologous antagonist killer protein (Bak) and Bcl-2 associated X protein (Bax), and decreased the expression of the antiapoptotic proteins Bcl-2 and Bcl-Xlong protein. Knockdown of Bak blocked the augmentative effects of GalphasQL. GalphasQL decreased the degradation rate of the Bak protein, and increased Bak mRNA transcript levels. GalphasQL increased Bak-luciferase activity in a protein kinase A and cyclic AMP response element-dependent manner. GalphasQL also augmented cisplatin-induced apoptosis of H1299 human lung cancer cells that lack functional p53. From this study, it is concluded that Galphas augments cisplatin-induced apoptosis of lung cancer cells partially through upregulating Bak expression by increasing transcription and by decreasing the rate of protein degradation.</P>

        A cluster of lung injury cases associated with home humidifier use: an epidemiological investigation

        <P><B>Background</B></P><P>In April 2011 a tertiary hospital in Seoul, Korea reported several cases of severe respiratory distress of unknown origin in young adults.</P><P><B>Methods</B></P><P>To find the route of transmission, causative agent and patient risk factors of the outbreak, an investigation of the epidemic was initiated. Clinicopathological conferences led to the suspicion that the cases related to an inhalation injury. An age- and sex-matched case–control study was therefore performed to examine the inhalation exposure of the patients to various agents.</P><P><B>Results</B></P><P>Of the 28 confirmed cases, 18 agreed to participate. A total of 121 age- and sex-matched controls with pulmonary, allergic or obstetric disease were selected. All patients and controls completed questionnaires with questions about exposure to various inhalants. The crude ORs for patient exposure to indoor mould, humidifier use, humidifier detergent use and insecticide use were 4.4 (95% CI 1.5 to 13.1), 13.7 (95% CI 1.8 to 106.3), 47.3 (95% CI 6.1 to 369.7) and 3.9 (95% CI 1.3 to 11.7), respectively. However, when considered concurrently, indoor mould and insecticide use lost statistical significance. Moreover, humidifier use was ruled out as the cause because of a lack of biological plausibility and the weak strength of the association. This suggested that humidifier disinfectant was the cause of the outbreak. This information led the Korean government to order the removal of humidifier detergents from the market. In the years following the ban, no additional cases were detected.</P><P><B>Conclusions</B></P><P>Epidemiological evidence strongly suggests that the lung injury outbreak was caused by humidifier detergent use at home.</P>

        Effects of Brown Rice Extract Treated with Lactobacillus sakei Wikim001 on Osteoblast Differentiation and Osteoclast Formation

        Phytic acid (myo-inositol hexakisphosphate) or phytate is considered an anti-nutrient due to the formation of precipitated complexes that strongly reduces the absorption of essential dietary minerals. In this study, brown rice with reduced phytate was made by inoculation with Lactobacillus sakei Wikim001 having high phytase activity. The effects of brown rice extract treated with L. sakei Wikim001 (BR-WK) on osteoblast differentiation and osteoclast formation were investigated. The proliferation of SaOS-2 cells was measured by the MTT assay. Treatment with BR-WK increased cell proliferation by 136% at a concentration of 100 ㎍/mL. The Alkaline phosphate activity in SaOS-2 cells was 129% higher when BR-WK was processed at a concentration of 100 ㎍/mL. The proliferation of bone marrow macrophages decreased by nearly 60% in response to treatment with BR-WK. In addition, BR-WK reduced the number of tartrate-resistant acid phosphatase-positive (TRAP+) multinucleated cells from bone marrow macrophages. These results indicate that BR-WK stimulates bone formation through its positive action on osteoblast differentiation and function and furthermore, decreases osteoclast differentiation.

      • Optimized Search-and-Compute Circuits and Their Application to Query Evaluation on Encrypted Data

        <P>Private query processing on encrypted databases allows users to obtain data from encrypted databases in such a way that the users' sensitive data will be protected from exposure. Given an encrypted database, users typically submit queries similar to the following examples: 1) How many employees in an organization make over U.S. $100000? 2) What is the average age of factory workers suffering from leukemia? Answering the questions requires one to search and then compute over the relevant encrypted data sets in sequence. In this paper, we are interested in efficiently processing queries that require both operations to be performed on fully encrypted databases. One immediate solution is to use several special-purpose encryption schemes simultaneously; however, this approach is associated with a high computational cost for maintaining multiple encryption contexts. Another solution is to use a privacy homomorphic scheme. However, no secure solutions have been developed that satisfy the efficiency requirements. In this paper, we construct a unified framework to efficiently and privately process queries with search and compute operations. For this purpose, the first part of our work involves devising several underlying circuits as primitives for queries on encrypted data. Second, we apply two optimization techniques to improve the efficiency of these circuit primitives. One technique involves exploiting single-instruction-multiple-data (SIMD) techniques to accelerate the basic circuit operations. Unlike general SIMD approaches, our SIMD implementation can be applied even to a single basic operation. The other technique is to use a large integer ring (e.g., Z(2)t) as a message space rather than a binary field. Even for an integer of k bits with k > t, addition can be performed using degree 1 circuits with lazy carry operations. Finally, we present various experiments performed by varying the considered parameters, such as the query type and the number of tuples.</P>

        백과사전 질의응답 시스템은 사용자의 자연어 질문과 검색 대상 문서인 백과사전 내용의 의미를 파악하기 위한 고정밀 자연어 처리 기술이 요구된다. 이러한 고정밀 자연어 처리 기술을 위한 중요한 언어자원을 제공하기 위하여 한국어 명사와 동사로 구성되는 대규모 어휘개념망을 구축하였다. 한국어 어휘개념망은 명사와 동사의 상하위 관계를 주요 계층구조로 하여 다양한 한국어 어휘 기초 자료를 바탕으로 구축되었다. 구축된 규모는 일반 명사 약 6만 어휘와 동사 약 2만 어휘를 포함한다. 이 논문에서는 어휘개념망을 구축하기 위한 방법과 과정을 소개하고 지금까지 구축된 어휘개념망의 특성에 대해 기술하며, 백과사전 질의응답 시스템에서 어떻게 활용되는지 시스템 구성요소의 예를 들어서 설명한다. 또한 현재 구축된 어휘개념망의 성능 평가를 위해 일반 코퍼스에 대한 커버리지 측정 결과를 기술한다.

      • Averaged head phantoms from magnetic resonance images of Korean children and young adults

        <P>Increased use of mobile phones raises concerns about the health risks of electromagnetic radiation. Phantom heads are routinely used for radiofrequency dosimetry simulations, and the purpose of this study was to construct averaged phantom heads for children and young adults. Using magnetic resonance images (MRI), sectioned cadaver images, and a hybrid approach, we initially built template phantoms representing 6-, 9-, 12-, 15-year-old children and young adults. Our subsequent approach revised the template phantoms using 29 averaged items that were identified by averaging the MRI data from 500 children and young adults. In females, the brain size and cranium thickness peaked in the early teens and then decreased. This is contrary to what was observed in males, where brain size and cranium thicknesses either plateaued or grew continuously. The overall shape of brains was spherical in children and became ellipsoidal by adulthood. In this study, we devised a method to build averaged phantom heads by constructing surface and voxel models. The surface model could be used for phantom manipulation, whereas the voxel model could be used for compliance test of specific absorption rate (SAR) for users of mobile phones or other electronic devices.</P>
