Back Mouse 증후군 : 증례보고

Back mouse 증후군은 요천추부위에 존재하는 압통을 가지는 섬유-지방의 피하결절로 인해 발생하는 요통을 일컫는 질환으로 요천추부 추간판 탈출증, 신경근 병변 등으로 오진을 할 수 있는 질환이다. 저자들은 54세 여자 환자로 20여년 전부터 발생한 요통과 무릎통증을 주소로 내원하여 이학적 검사와 초음파 검사를 통해 통증을 유발하는 피하지방과 유사한 에코를 보이는 줄기를 가진 결절을 관찰 후 반복적 주사 치료로 증상 호전을 경험하였기에 문헌고찰과 함께 보고하는 바이다. Back mouse syndrome is a disease causing low back pain and painful, fibrous fatty nodules found in the lumbosacral area. It often could be misdiagnosed as lumbosacral disk herniation, radiculopathy etc. We describe 1 patient in 54 years-old who was diagnosed as back mouse syndrome complaining of low back pain with right knee pain and then improved the symptoms after repetitive lidocaine injection.

급성 알콜성 근병증 1예

급성 알콜성 근병증은 알콜 중독자 1%정도에서 발생하는 드문 병으로 종대성 근육통 등의 증상 발현 후 수 일내에 진행하며 횡문근 융해로 인한 치명적인 신 손상을 유발할 수 있어 빠른 진단과 처치를 필요로 한다. 20년간 매일 소주 3병정도를 마셨던 46세 남자 환자가 양측 어깨 및 상지에 심한 근육통을 주소로 내원하였다. 이학적 검사에서 도수근력은 상지 및 하지 모두 good grade이었으나, 근 수축 시 심한 통증을 호소하였으며 통증으로 인해 지구력이 상당히 감소되어 있었다. 양쪽 어깨 및 팔에 종대성 근육통(시각상사척도 8)이 있었으나 관절운동범위는 모두 정상이었다. 혈액 검사 소견에서 혈청 CK 2287 U/L, Myoglobin 437 ng/ml, AST 135 U/L, ALT 31 U/L, BUN 24 mg/dl, creatinine 1.0 mg/dl이었다. 근전도 소견에서는 침 근전도에서 대부분의 근위부 근육에서 비정상 자발전위가 관찰되었고, 작은 진폭의 지속시간이 짧은 다상성 운동 활동 전위가 관찰되었다. 좌측 상완 이두근에서 시행한 근 생검에서 급성 알코올 근병증으로 진단되었다. 내원 1주일 후 종대성 근육통(시각상사척도 8점)이 대퇴부까지 진행되었으며, 침상 안정 및 대증 치료를 시행하였고, 내원 1개월 후 환자는 통증을 호소하지 않았으며, 정상 독립보행 및 독립적으로 일상생활동작을 수행할 수 있는 상태로 퇴원하였다. 저자들은 알콜성 말초 신경병증을 가진 환자에서 발생한 급성 알콜성 근병증 1예를 경험하였기에 보고하는 바이다. Upper limbs and all lower limbs of manual muscle test were Good grade in physical examination, but to core contraction serious pain did complaint and endurance by ache was decreased fairly. There was myalgia (VAS 8) for stalk both shoulder and arm and range of motion of all joint were normal. Serum CK 2287 U/L, myoglobin 437 ng/ml, AST 135 U/L were increased in laboratory finding, but serum phosphate and potassium were within normal limits. Needle electrodiagnostic findings at most proximal muscle of upper extremity in electrodiagnostic study is abnormal spontaneous activity at rest and low amplitude, short duration, low amplitude polyphasic motor unit action potential on minimal volition. The finding of muscle biopsy at left biceps brachialis was revealed alcoholic myopathy. Edematous myalgia (VAS 8) was gone to thigh after 1 week. Enforced bed rest and supportive care, and patient did not complain pain after 1 month, and he discharged in state that can achieve normal gait pattern and independent activities of daily living.

Strength Development Characteristic of Cementless Mortar for Repair of Concrete

<P>Repair materials for concrete structures are often required to exhibit high bond strength at a concrete substrate, and it typically consists of ordinary Portland cement (OPC) incorporating expensive admixtures at a low water-to-binder ratio. Meanwhile, cementless mortar employs alkali-activation of cementless and pozzolanic precursors such as ground granulated blast-furnace slag (GGBFS) and fly ash (FA). The final product develops strength relatively faster than OPC, and its strength can be flexibly talyored by controlling the type and dosage of the activator. The present study investigates the strength development characteristic of cementles mortar for use in repair of concrete structures. Independent variables include mix proportions and curing conditions, which were chosen to optimize the performance of the cementless mortar.</P>

생맥산의 식이가 Streptozotocin으로 유도된 당뇨 Rat의 혈당과 혈청 성분 함량에 미치는 영향

Streptozocin으로 당뇨를 유발시킨 rat에서 생맥산의 식이가 식이효율(FER), glucose, creatinine, cholesterol, triglycetide, HDL-cholesterol, 동맥경화지수(AI) 등에 미치는 영향을 조사하였다. 식이효율은 정상군이 5.94%로 가장 높았고, 당뇨대조군은 0.58%로 정상군에 비해 현격한 감소를 보였다. DA군과 DB군은 각각 3.65%, 3.38%로 다른 투여군보다 상대적으로 높은 수준의 식이효율을 보였다. 혈당변화를 측정한 결과 투여 전에 비해 DA군은 41%, DB군은 24.8%, DC군은 31.2%, DDL군은 18.6%, DDH군은 25.5%의 혈당 감소효과를 보였다. 생맥산 투여 전 creatinine 함량은 정상군에 비하여 당뇨 유발군에서 상승을 보였으며, DB군은 투여 후 33.3%의 유의적인 감소(p<0.05)를 보였고, 그외의 군(DA, DC, DDL, DDH)에서는 유의적인 변화가 없었다. 혈청 내 cholesterol 수치의 유의적인 상승을 보인 당뇨 유발군과 비교하여 생맥산을 14일간 투여한 모든 군에서 cholesterol 수치의 유의적인 감소를 보였다. 당뇨 유발군에서 유의적인 상승을 보인 혈청 triglyceride는 DC군에서 수치가 감소하였지만 유의적인 차이는 없었다. HDL-Chlolesterol은 DA, DB, DC군에서는 유의적인 변화는 나타나지 않았다. 반면 DDL군과 DDH군에서의 HDL-chlolesterol은 투여 전과 후 각각 41.81±2.28 ㎎/dL, 40.75±0.48 ㎎/dL에서 50.59±2.77, 49.93±1.17 ㎎/dL로 유의적인 증가를 나타냈다. Atherogenic index(AI)를 측정한 결과 투여 전에는 당뇨 유발군의 AI 수치가 정상군과 비교 시 모든 그룹에서 상승하였으나, 투여 14일 후 측정결과 DA, DB, DC군에서 정상군에 가깝게 수치가 낮아진 것을 확인했다. 또한 DDL군과 DDH군은 유의적인 AI지수의 감소를 나타냈으며 압출성형백삼을 이용한 생맥산의 식이가 동맥경화의 예방에 상당한 효과가 있는 것으로 판단된다. 이상의 결과를 종합해 보면 STZ로 유도된 당뇨 rat에서 생맥산의 식이는 FER의 개선에 상당한 효과를 보이며, 고혈당을 억제시키고 정상수준의 혈당으로 회복하는데 영향을 미치는 것으로 여겨진다. 또한 발효시킨 백삼을 이용한 생맥산 투여군에서는 신장 기능의 지표인 creatinine의 함량을 낮추고 지질수준의 감소를 보여 당뇨의 증상을 완화시키고 동맥경화증과 같은 혈액 순환기계 질환의 예방에 효과적인 건강보조식품으로 사용될 수 있을 것으로 사료된다. This study was carried out to investigate the effect of saengmaec-san on the level of blood glucose and serum components in streptozocin (STZ)-induced diabetic rats. The experimental groups were divided into normal group (normal), diabetic control group (Dia-control), 10% saengmaec-san (white ginseng) group (DA), 10% saengmaec-san (fermented white ginseng) group (DB), 10% saengmaec-san (fermented red ginseng) group (DC), 5% saengmaec-san (extruded and fermented white ginseng) group (DDL), 10% saengmaec-san (extruded and fermented white ginseng) group (DDH). The body weight after induction of diabetes was 85.4% in the Dia-control group compared with the normal group. But in the DA group, the body weight showed clear sign of recovery almost normal level after administration of saengmaec-san for two weeks. The food efficiency ratios (FER) were 5.94% in the normal group. But it was significantly decreased in the Dia-control group (0.58%). All the treatment groups showed increase of FER compared with the Dia-control group. The level of blood glucose was significantly increased in the STZ-induced diabetes groups but it was decreased in all the treatment groups after administration for 2 weeks. Serum creatinine level were significantly higher in the STZ-induced diabetes groups and after administration of saengmaeg-san for 2 weeks while the level of serum creatinine was decreased 33.3% in the DB group. After administration of saengmaec-san for two weeks, serum total cholesterol level were significantly lower in all treatment groups than the first day of the total cholesterol level. The level of serum triglyceride was increased in all the treatment groups compared with the first day of triglyceride level. The level of serum HDL-cholesterol, after STZ-induced diabetes, was decreased in all treatment groups but particularly in the DDL and DDH groups increased HDL-cholesterol level compared with the first day of the saengmaec-san administration. Compared with the beginning of experiment, Atherogenic index (AI) were significantly decreased in all treatment groups than the Dia-control group and showed clear sign of recovery almost normal level. These results suggested that the saengmaec-san could be developed as an antidiabetic agent.

Because of the environmental matters the importance of the city railroad is as time goes by increasing. The case of obstacle of the power equipment which supplies electric power to city railroad will occur social and economical enormous loss. Thus, I studied on the preventing method in advance which makes it possible for us to maintain facilities efficiently. The main check points of the power facilities are voltage, current, humidity, partial discharge, move current. These points are gathered by sensor and transmitting to data acquisition device. These data are used to check equipment status in real time. In this paper I described in brief test process and results of the detection system.
