      • A case of basal cell carcinoma of the nail bed

        Basal cell carcinoma is widely regarded as the most common malignant skin tumor and usually seen in head and neck. Ralely, nail bed and nail folds can be atypical sites for BCC. The initial diagnosis of chronic paronychia lead to delay in diagnosis so that taking history and adequate biopsy are essential in making timely diagnosis. Previous study reported that the BCC of nail bed was more often on the fingers than on the toes and had a slight predilection to occur in men. Ulceration was the most common presentation of nail unit BCC, which was noted in more than one-half of patients. Mohs micrographic surgery with second intension healing is effective to make excellent cosmetic results. The incidence of metastasis is extremely rare and same as other BCCs, which has been reported with incidence rates of 0.0028% to 0.55%. A 55-year-old man presented with ill-defined 0.8 cm sized pigmented lesion on left thumb nail bed without trauma history. Duration of the lesion was 1 month. Specimen showed that basophilic cells formed budding tumor proliferations extending from the undersurface of the epidermis to the dermis. HMB45 and S-100 showed negative results. He has no any other past history and laboratory study showed normal findings. The patient was finally diagnosed as BCC of the nail bed and referred to plastic surgeon for wide excision and full thickness skin graft. Herein we report a relatively rare case of BCC of the nail bed.

      • Cytoplasmic TrkA Expression as a Screen for Detecting <i>NTRK1</i> Fusions in Colorectal Cancer <sup>1</sup> <sup>2</sup>

        <P><I>NTRK1</I> gene fusions, the targets of multikinase inhibitors, are promising therapeutic targets for colorectal cancer (CRC). However, screening methods for detecting <I>NTRK1</I> gene fusions in CRC tissues have not been reported. In this study, we investigated the potential use of immunohistochemistry (IHC) for detecting <I>NTRK1</I> gene fusions. We performed and compared IHC with fluorescence in situ hybridization (FISH) in 80 CRC patients. TrkA immunostaining was observed to be both membranous and cytoplasmic and was scored semiquantitatively using staining intensity and proportions. The tumors were observed to be <I>NTRK1</I> gene fusion-positive when ≥20 out of 100 nuclei in FISH. A significant correlation between the IHC and FISH results for determination of the <I>NTRK1</I> gene fusions was observed. We measured the cytoplasmic TrkA expression, which showed an area under the receiver operating characteristic (ROC) curve of 0.926 (range: 0.864-0.987, 95% CI, <I>P</I> = .001). By choosing 4.5 (sum of the intensity and proportion scores of cytoplasmic TrkA expression) as the cut-off value for the positive and negative <I>NTRK1</I> gene fusion groups, the sensitivity and specificity for predicting lymph node metastasis were 100 and 83.8%, respectively (<I>P</I> = .001). Specifically, high cytoplasmic TrkA expression (sum of intensity and proportion scores >4) was associated with the presence of <I>NTRK1</I> gene fusions (<I>P</I> < .0001, r = 0.528). Taken together, our data showed that IHC for TrkA can be used as an efficient screening method for detecting <I>NTRK1</I> gene fusions in CRC.</P>

        Resistance Mechanisms and Clinical Features of Fluconazole-Nonsusceptible <i>Candida tropicalis</i> Isolates Compared with Fluconazole-Less-Susceptible Isolates

        <P>We investigated the azole resistance mechanisms and clinical features of fluconazole-nonsusceptible (FNS) isolates of Candida tropicalis recovered from Korean surveillance cultures in comparison with fluconazole-less-susceptible (FLS) isolates. Thirty-five clinical isolates of C. tropicalis, comprising 9 FNS (fluconazole MIC, 4 to 64 mu g/ml), 12 FLS (MIC, 1 to 2 mu g/ml), and 14 control (MIC, 0.125 to 0.5 mu g/ml) isolates, were assessed. CDR1, MDR1, and ERG11 expression was quantified, and the ERG11 and UPC2 genes were sequenced. Clinical features of 16 patients with FNS or FLS bloodstream isolates were analyzed. Both FNS and FLS isolates had> 10-fold higher mean expression levels of CDR1, MDR1, and ERG11 genes than control isolates (P values of< 0.02 for all). When FNS and FLS isolates were compared, FNS isolates had 3.4-fold higher mean ERG11 expression levels than FLS isolates (P = 0.004), but there were no differences in those of CDR1 or MDR1. Of all 35 isolates, 4 (2 FNS and 2 FLS) and 28 (8 FNS, 11 FLS, and 9 control) isolates exhibited amino acid substitutions in Erg11p and Upc2p, respectively. Both FNS and FLS bloodstream isolates were associated with azole therapeutic failure (3/4 versus 4/7) or uncleared fungemia (4/6 versus 4/10), but FNS isolates were identified more frequently from patients with previous azole exposure (6/6 versus 3/10; P = 0.011) and immunosuppression (6/6 versus 3/10; P = 0.011). These results reveal that the majority of FNS C. tropicalis isolates show overexpression of CDR1, MDR1, and ERG11 genes, and fungemia develops after azole exposure in patients with immunosuppression.</P>

        This paper reviews the R&D results achieved in Korea<br/> Electrotechnology Research Institute (KERI) in a decade or so. KERI has developed pulsed power technology and its applications mostly based on power electronics. KERI’s R&D activity covers components, military systems, and industry applications.<br/> <br/> High voltage switching means developed in KERI are a rotary arc gap switch (11kV, 400kAp), a vacuum rotary arc gap (20kV, 150kAp), an inverse pinch switch (20kV, 150kAp), and 10kV semiconductor switch stacks made of thyristors and IGBTs. The IGBT and thyristor stacks consists of from ten to twelve 1.2kV discrete devices and<br/> are operated by only one active circuit located in the ground side. Some ferromagnetic opening switch schemes are also researched on trial bases. <br/> <br/> KERI has developed several pulsed power supplies responding to industry needs, of which applications are electrostatic precipitators, high voltage capacitor chargers, polluted gas treatment systems. The investigated pulsed power applications are rock fragmentation system, plasma ion implantation (PSII), food process, and fine metal powder fabrication system using wire-exploding process.

        <i>Rubus occidentalis</i> analgesic effect in a rat model of incisional pain

        <P><B>Abstract</B></P> <P><B>Background</B></P> <P>The purpose of this study was to evaluate the analgesic effect of <I>Rubus occidentalis</I> extract (ROE) in a rat model of incisional pain. The involved mechanisms and proinflammatory cytokine response were also examined.</P> <P><B>Materials and methods</B></P> <P>To investigate the analgesic effect, rats were intraperitoneally administered with normal saline or various doses of ROE before or after a plantar incision. To evaluate the involved mechanism, rats were intraperitoneally administered yohimbine, dexmedetomidine, prazosin, naloxone, atropine, or mecamylamine after a plantar incision; ROE was then administered intraperitoneally. The mechanical withdrawal threshold (MWT) was tested with von Frey filaments at various time points. To determine the inflammatory response, serum levels of interleukin (IL)-1β or IL-6 were measured.</P> <P><B>Results</B></P> <P>The MWTs significantly increased at 15 min after postincisional administration of 300 mg/kg ROE when compared with those in the control group. This elevation was observed for up to 45 min. Overall, MWTs increased in proportion to ROE dosage; however, ROEs administered before the incision produced no significant change in the MWT. The analgesic effect of ROE was significantly antagonized by mecamylamine, naloxone, and yohimbine, and agonized by dexmedetomidine. Administration of ROE inhibited the postincisional increase in serum IL-1β and IL-6.</P> <P><B>Conclusions</B></P> <P>Intraperitoneal administration of ROE after surgery induces antinociceptive effects in a rat model of postoperative pain, and its effects on mechanical hyperalgesia may be associated with α<SUB>2</SUB>-adrenergic, nicotinic cholinergic, and opioid receptors.</P>

      • A rare case of mucinous nevus on the buttock

        Mucinous nevus is a rare benign entity classified as a cutaneous mucinosis or connective tissue nevus. The principal site is known as trunk and clinically presented as asymptomatic growing grouped papuloplaques with unilateral distribution at birth or early adulthood. The band-like amorphous mucin deposition in the upper dermis is regarded as its own histologic features. A 11-year-old boy presented with asymptomatic multiple papuloplaques on left side of buttocks. Skin biopsy specimens from his lesion of left buttock showed orthokeratosis, acanthosis and slight papillomatosis in epidermis and also amorphous bluish materials in papillary dermis. These materials were proven as mucin by staining of alcian blue. In this case, the patient showed verrucous lesions on left side of buttock near the anus which is known as uncommon sites. We firstly suspect this findings as condyloma accuminatum, but it was ruled out by negative findings of HPV-PCR and band-like deposition of mucin in upper dermis without abnormal findings such as large vacuolated epithelial cells Therefore, we emphasize the importance of skin biopsy including staining of mucin to confirm mucinous nevus near the anus which can be misdiagnosed as condyloma accuminatum. Herein, we report a rare case of mucinous nevus on left buttock area.

      • A rare case of primary cutaneous plasmacytosis on left mandibular area

        Cutaneous plasma cell infiltration can be seen in various disorders, including amyloidosis, lupus erythematosus, scleroderma, plasma cell neoplasm and infectious diseases such as syphilis and deep fungal infections. Above all, primary cutaneous plasmacytosis is regarded as a rare benign proliferation of plasma cells which can present as reddish papuloplaques or nodules without other systemic involvement. A 44-year-old man presented with asymptomatic irregular surfaced erythematous nodule on left mandibular area. Skin biopsy specimens showed multiple plasma cells and lymphoid cell infiltration in entire dermis. There were no fungal organisms identified in PAS and GMS stainings. Immunohistochemical stainings showed positive for CD 138 but no light chain restriction on Kappa and lambda light chains suggesting of no monoclonality. He has no evidence of other organ involvement or infection including VDRL in laboratory tests. Multiple myeloma was ruled out by normal findings of calcium level, renal function, level of complete blood counts and imaging of bones. Extramedullary plasmacytoma was excluded because of no monoclonality. He was finally diagnosed with primary cutaneous plasmacytosis based on histological findings and was treated with surgical excision. Herein we report a rare case of primary cutaneous plasmacytosis on left mandibular area.

      • Microsatellite Typing and Resistance Mechanism Analysis of Voriconazole-Resistant <i>Aspergillus flavus</i> Isolates in South Korean Hospitals

        <P>A recent surveillance study in South Korea revealed that 14% (7/50) of <I>Aspergillus flavus</I> clinical isolates had a voriconazole minimum inhibitory concentration of ≥4 μg/ml. Of seven non-wild-type (non-WT) isolates, six ear isolates from four hospitals shared the same microsatellite genotype.</P><P>A recent surveillance study in South Korea revealed that 14% (7/50) of <I>Aspergillus flavus</I> clinical isolates had a voriconazole minimum inhibitory concentration of ≥4 μg/ml. None of the non-WT isolates showed <I>cyp51</I> mutations associated with azole resistance. However, the mean expression levels of efflux pump (<I>MDR2</I>, <I>atrF</I>, and <I>mfs1</I>) and target (<I>cyp51A</I>) genes exhibited significant differences between non-WT and other isolates.</P>

      • Construction of a Verification System for Paper Result

        The result of various R&D activities such as paper, patent and program are used to evaluate the performance of the research. The authenticity of the result has to be guaranteed in order to use it as an evaluation factor and in the case of a paper, whether it has actually been published or not is an important factor. A system for checking the authenticity of a paper is designed in this paper. The major components of the paper are automatically check and manual confirmation is processed through the search for the document similarity. The system aims to increase the effectiveness of verifying a paper.
