        최근 네덜란드의 건축은 세계 건축계에서 두각을 나타내고 있으며, 현대 건축사에 일 획을 긋는 등 중요한 역할을 하고 있다. 특히 우리에게 널리 알려진 Rem Koolhaas나 MVRDV등은 국내의 프로젝트에도 적극적으로 참여하여 그들의 진가를 잘 보여주고 있다. 이런 네덜란드 건축계와의 건축문화교류를 통해 그들이 일궈놓을 수 있는 독창성과 건축문화의 배경을 이해하고 우리의 건축문화를 위한 초석을 놓고저, 우리 협회는 지난 8월에 3명의 대표단을 보내 사전 조사를 시행한 바 있다. 그결과, Berlage Institute와 네덜란드 건축문화원과는 공감이 갈수 있는 건축적 이슈들을 국제토론의 장으로 이끌어 내는데 공조하기로 합의하고, 이의 후속 작업으로 Berlage Institute의 학장인 Rob Doctor교수와 네덜란드 건축문화원(Netherlands Architecture Institute, NAI)의 원장인 Ole Bouman 이 네덜란드의 건축정책을 소개하는 글을 게재하기로 했다. 이는 한국 건축사로서 현재 네덜란드에 유학중인 이은경씨의 도움에 의해 준비 되었고, 번역까지 도움을 주었다. 이은경씨의 도움에 진심으로 감사드린다.

      • A Practical Method of Prediction of Resistance for Displacement Vessels

        The prediction of the total resistance of a ship is generally based on considering it to be a simple sum of the viscous resistance and the wave resistance. An experimental approach for predicting full-size ship resistance on this basis is practical but obviously has the deficiency that a model has to be built for each prototype of interest and the resulting tank tests are time consuming. On the other hand, purely theoretical calculations of the wave resistance, using, for example, the Michell theory, require relatively little computer time and give an excellent portrayal of the overall variation of the vessel resistance as a function of forward speed. Unfortunately, there are sufficient differences between this theory and the measured results to make this method impractical for design purposes. The proposal examined here is to use a data bank of experimental resistance results to modify the theoretical predictions. It is demonstrated that the technique will produce remarkably accurate resistance predictions and can take into account the effects of the water depth, any restriction of canal or river width, as well as the prismatic coefficient, and other geometric parameters.

        Low-cost Hardware that Accelerates Frequent Item Counting with an FPGA

        In this paper, hardware acceleration using a field programmable gate array is proposed to provide low development–cost and high-performance stream processing hardware. This research is proposed as an enhancement to the software-based application for frequent item counting (FIC) and to contribute to hardware-based FIC for hardware/software co-design. We design an experiment by taking advantage of high-level synthesis (HLS) and the heterogeneous Computing Oriented Development Environment (hCODE), an open source platform providing a methodology and a tool for scalable and portable Internet Protocol design. The proposed scheme considers optimization techniques offered by HLS compilers, such as the pipeline technique, loop unrolling, and memory partition. Our implementation shows that the proposed scheme achieves a better overall performance than a software scheme, and more importantly, introduces fast and low development costs for hardware accelerators.

        Acupressure versus dilution of fentanyl to reduce incidence of fentanyl-induced cough in female cancer patients: a prospective randomized controlled study

        Background: Fentanyl-induced cough (FIC) is a transient condition with a reported incidence of 18% to 65% depending on the dose and route of administration of fentanyl. Nonpharmacological methods to prevent FIC are more cost-effective than medications. Dilution of fentanyl has a proven role in the prevention of FIC. Acupressure can also prevent FIC because it has a proven role in the treatment of cough. Methods: This study included 225 female patients with an American Society of Anesthesiologists physical status of I or II who were randomly divided into 3 groups of 75 patients each. Patients in the control group received undiluted fentanyl at 3 μg/kg, patients in the acupressure group received undiluted fentanyl at 3 μg/kg with acupressure, and patients in the dilution group received diluted fentanyl at 3 μg/kg. Coughing was noted within 2 min of fentanyl administration. The severity of FIC was graded as mild (1–2 coughs), moderate (3–4 coughs), or severe (≥5 coughs). The timing of coughs was also noted. Results: The incidence of FIC was 12.7% in the control group, 6.8% in the dilution group, and 1.3% in the acupressure group. The difference in the incidence of cough was statistically significant (P = 0.008) between the control and acupressure groups. The difference in the severity of cough among the groups was not statistically significant. The median onset time of cough among all groups was 9 to 12 seconds. Conclusions: The application of acupressure prior to administration of fentanyl significantly reduces the incidence of FIC. Dilution of fentanyl also reduces the incidence of FIC, but the difference is not statistically significant.

        Self-reported Wheat Sensitivity in Irritable Bowel Syndrome and Healthy Subjects: Prevalence of Celiac Markers and Response to Wheat-free Diet

        Background/Aims Most patients with irritable bowel syndrome (IBS) report food-related aggravation of symptoms. Wheat/gluten is one of the most commonly incriminated. We studied the prevalence of self-reported wheat sensitivity in patients with IBS and in a healthy population from a region in India consuming mixed-cereal diets, correlated it with serological and human leukocyte antigen (HLA) markers of celiac disease, and evaluated the response to a wheat-free diet. Methods We surveyed 204 patients with IBS and 400 healthy persons for self-reported wheat sensitivity. Testing for IgA anti-tissue transglutaminase and HLA DQ2 or DQ8 was done in individuals who reported wheat sensitivity. Consenting persons with wheat sensitivity were put on wheat-free diet and monitored for symptom change. Results Twenty-three of 204 patients with IBS (11.3%) and none of the healthy subjects self-reported wheat sensitivity. Of 23 patients, 14 (60.9%) were positive for HLA DQ2 or DQ8 and none for anti-tissue transglutaminase antibody. After 6 weeks on wheat-free diet, all 19 participating patients reported clinical improvement; fewer patients had bloating, diarrhea, constipation, and easy fatigue. Conclusions Eleven percent of patients with IBS self-reported wheat sensitivity. None of them had positive celiac serology; 60.9% were positive for HLA DQ2 and DQ8, suggesting a possible genetic basis. All of them improved symptomatically on a wheat-free diet.         Gross motor dysfunction and balance impairments in children and adolescents with Down syndrome: a systematic review

        Background: Individuals with Down syndrome present with several impairments such as hypotonia, ligament laxity, decreased muscle strength, insufficient muscular cocontraction, inadequate postural control, and disturbed proprioception. These factors are responsible for the developmental challenges faced by children with Down syndrome. These individuals also present with balance dysfunctions.Purpose: This systematic review aims to describe the motor dysfunction and balance impairments in children and adolescents with Down syndrome.Methods: We searched the Scopus, ScienceDirect, MEDLINE, Wiley, and EBSCO databases for observational studies evaluating the motor abilities and balance performance in individuals with Down syndrome. The review was registered on PROSPERO.Results: A total of 1,096 articles were retrieved; after careful screening and scrutinizing against the inclusion and exclusion criteria, 10 articles were included in the review. Overall, the children and adolescents with Down syndrome showed delays and dysfunction in performing various activities such as sitting, pulling to stand, standing, and walking. They also presented with compensatory mechanisms to maintain their equilibrium in static and dynamic activities.Conclusion: The motor development of children with Down syndrome is significantly delayed due to structural differences in the brain. These individuals have inefficient compensatory strategies like increasing step width, increasing frequency of mediolateral center of pressure displacement, decreasing anteroposterior displacement, increasing trunk stiffness, and increasing posterior trunk displacement to maintain equilibrium. Down syndrome presents with interindividual variations; therefore, a thorough evaluation is required before a structured intervention is developed to improve motor and balance dysfunction.

        Per-oral cholangioscopy via a gastric access loop for the management of recurrent hepatolithiasis

        This is a case report of the successful management of recurrent large intrahepatic stones using per-oral cholangioscopy via a gastric access loop created surgically during Roux-en-Y hepaticojejunostomy (HJ). A 55-year-old male presented with acute cholangitis. Radiodiagnostic imaging suggested hepatolithiasis in the left hepatic duct. He had experienced similar events on three occasions in the past, for which preliminary conventional endoscopic retrograde cholangiopancreatography and percutaneous transhepatic biliary drainage approaches proved only supportive. Hence definitive surgical treatment was performed with the patient`s consent during his third admission in the form of cholecystectomy, intraoperative retrieval of hepatolithiasis using a rigid ureteroscope, and Roux-en-Y HJ with gastric access loop formation. The gastric conduit facilitated prompt and convenient endoscopic access to enter the intrahepatic ducts and achieve complete ductal clearance using cholangioscopy-guided laser lithotripsy of the large intrahepatic stones. The patient remained asymptomatic at subsequent follow-up visits.

        <P>Proteins expressing postsynaptic density (PSD)-95/Drosophila disk large (Dlg)/zonula occludens-1 (ZO-1) (PDZ) domains are commonly involved in moderating receptor, channel, and transporter activities at the plasma membrane in a variety of cell types. At the apical membrane of renal proximal tubules (PT), the type IIa NaP(i) cotransporter (NaP(i)-IIa) binds specific PDZ domain proteins. Shank2E is a spliceoform of a family of PDZ proteins that is concentrated at the apical domain of liver and pancreatic epithelial cell types and is expressed in kidney. In the present study, immunoblotting of enriched plasma membrane fractions and immunohistology found Shank2E concentrated at the brush border membrane of rat PT cells. Confocal localization of Flag-Shank2E and enhanced green fluorescent protein-NaP(i)-IIa in cotransfected OK cells showed these proteins colocalized in the apical microvilli of this PT cell model. Shank2E co-immunoprecipitated with NaP(i)-IIa from rat renal cortex tissue and HA-NaP(i)-IIa coprecipitated with Flag-Shank2E in cotransfected human embryonic kidney HEK cells. Domain analysis showed that the PDZ domain of Shank2E specifically bound NaP(i)-IIa and truncation of the COOH-terminal TRL motif from NaP(i)-IIa abolished this binding, and Far Western blotting showed that the Shank2E- NaP(i)-IIa interaction occurred directly between the two proteins. NaP(i)-IIa activity is regulated by moderating its abundance in the apical membrane. High-P(i) conditions induce NaP(i)-IIa internalization and degradation. In both rat kidney PT cells and OK cells, shifting to high-P(i) conditions induced an acute internal redistribution of Shank2E and, in OK cells, a significant degree of degradation. In sum, Shank2E is concentrated in the apical domain of renal PT cells, specifically binds NaP(i)-IIa via PDZ interactions, and undergoes P(i)-induced internalization.</P>
