        Inhalation of a foreign body into the respiratory passage is a frequent in children and sometimes fatal accident. It can produce different symptoms and signs, which may be confused with esophageal foreign body. On occasion, it can be not only followed by some pulmonary complications, but also it is often delayed to be removed Recently, we experienced a bronchial foreign body, which was misdiagnosed a esophageal foreign body. It was successfully removed via ventilating bronchoscope, after esophagoscopy and reevaluation of a foreign body in operating room. We report this case with review of the literacture.

      • 國際政治體制와 國際的 安定性

        International system is a political environment in the international society. It indicates the direction of foreign policies of nations compromising the system as well as it shapes the patterns of relations among nations. Thus, international system is a dynamic pattern of international politics and it is anarchic, decentralized and informally organized. The purpose of this article is to examine the relations between the international system and international stability. Many scholars have established some hypotheses about this matter. For example, Karl Deutsch and David Singer contend that as the system moves away from bipolarity toward multipolarity, the frequence of war should be expected to diminish. On the contrary, Kenneth Waltz argues that the bipolar system is more stable than the multipolar system. But Richard Rosecrance criticizes the both arguments concerning bipolarity and multipolarity and concludes that the most desirable type of international system would exhibit both bipolar and multipolar characteristics. He calls it "a system of bi-multipolarity" And another scholar, as Ronald Yalem, saw an emerging tripolar world(United States, Soviet Union, and China) and such a world has greater possibility for conflict than any other systems on account of its potential conflict. On the one hand, these hypotheses are useful to analyze the structure of international system, on the other hand, they are turned out to be not "absolutely right." For example, according to the outcome of William R. Thompson's analysis, the argument of Waltz is right but that of Deutsch and Singer is false. And the outcome of my analysis is quite different from that of Thompson's analysis. In this respect, the relations between international system(types of polarity) and international stability are not relevant directly. The international instability such as wars among nations is not affected by the structure of system but by the foreign policies of major powers, their expansionistic trends and the conflicts of interests among nations.

      • 3차원적 측두골 전산화 단층촬영을 이용한 중이질환에 대한 임상적 고찰

        Study of the complex anatomy and pathology of the temporal bone has traditionally used permitted analysis in only two dimensional computed tomography. Recent technological advances in clinical application of computed tomography have made 3-dimensional reconstruction from consecutive axial tomography possible. This new method improves the assessment and therapy of patients undergoing surgical procedures for disease involving temporal bone. Three dimensional computed tomography was performed in 20 patients, involved the middle ear and temporal bone. 5 examples are demonstrated here and we concluded that this new method improves the display of the location and state of pathology and affords accurate therapeutic and surgical planning.

      • 진네트정(세푸록심 250 mg)에 대한 베아세프정의 생물학적 동등성

        ABSTRACT-The bioequivalence of “Bearcef tablet”(Daewoong Pharmaceutical Co.), containing 250 mg cefuroxime (as cefuroxime axetil), in reference to“Zinnat tablet”(Glaxo Wellcome Korea Co.) was evaluated in 16 normal volunteers (age 21∼29 yrs) following the oral administration. After one tablet was administered, blood was taken at predetermined time intervals and the concentration of the drug in plasma was quantitated with an HPLC method. AUC, C_max and T_max were calculated and statistically analyzed for the bioequivalence of the two products. The results showed that the differences in AUC, C_max and T_max between two products were 6.92%, 3.49% and 5.26%, respectively. The powers for AUC, C_max and T_max were >90%, >90% and 89.0%, respectively. Confidence intervals for these parameters were all within ±20%. Judging based on the above results, “Bearcef tablet”is regarded to be bioequivalent to “Zinnat tablet”.

      • 의약품중 잔류유기용매 시험법 확립에 관한 연구

        Headspace-GC(딘5-GC)를 이용하여 ICH 잔류웅매 가이드라인 class 1, 2에 속하는 28종의 잔류유기웅매 분석방법을 연군한 결과 SPB-5, BB-WAX column을 동시 이용하여 각각을 확인, 정량할 수 있었다.또한 염의 참가효과, 평형시간,평형온도가 headspace분석에 미치는 영향을 검토한 결과 평형 시간 30분,평형온도 85℃,Na₂SO₄Ig을 첨가하여 시험할 때 최적 부넉조건을 나타내었다. 회수율은 1,1-achloroethene(68.3%)를 제외하고 90.9 ~ 114.5%로 패체적으로 양호하였다. 따로 HS-GCJECD, DB-524 column을 이용하여 9종의 잔류유기용매에 대하여 각각의 검랸선을 작성하고 검출한계를 구하였다. 실제 의약품 원료 10종및 제제 5종에 적용시킨 결과 모두 불검출되었으며 본 시험법은 의약품중의 잔류유기용매 분석에 적용할 수 있는 방법이라고 판단된다. The headspace-fC/FID(HS -GC/FID) method was performed for test method developnlent of residu;31 organic solvents in pfrarmaceuticals. Using SPB-S and BB-WAXcolumn, 28 kinds of solvents iri ICH residual sof)·ents guidetine class 1, 2 couBd be individuallt-identified and quantitated. The foIBowing residoal soIYents were not detected by the headspaceinjection condition : N,N-Cmethylacetamide, N,N-dimeth)rlformamide, ethyfeneglycol, formamide,2-methoryethanot, N-rnethylpyrrolidone, suBforane. The effects of the addition of salts,equilibration time, and equilibration temperature on headspace analt·sis were invesugated. Theoptimum conditions weiFe obtained with addtion of Ifa2s04 19 as a salt, simultaneously. thetime and temperature of equitibration were 30min and 8i'f, respectivety. The recovery havefound between 90.9 and 114.5% except 1,1-dichloroethene(fi8.3%). Using DB-624 column &HS-GC/:ECD method, 9 kinds of residual solvents could be individually identified andfuantitated. This HS-GC method can be applied to test the residua3 organic solvent in thepharmaceuticals.

      • 중이 수술후 청력 변화에 대한 임상적 고찰

        statistical analysis of bone conduction loss in chronic otitis media has not been exactly reported until now. Authors studied the relationship between the preoperative bone conduction hearing level and age, nature of middle ear and temporal bone pathology, pneumatization of mastoid in 97 ears of chronic uppurative otitis media performed middle ear surgery at Konkuk University Hospital from March 1992 to December 1992. The results were as following: The mean bone conduction threshold was 16.2±12.4dB. In the cases of ossicular defect, bone conduction loss was greater in total destruction than in partial destruction with statistical significance. Among ossicular defect, incus destruction group showed more bone conduction loss than intact incus group with statistical significance. There was no difference of bone conduction loss according to pathologic findings in middle ear cavity and mastoid antrum, but pathologic groups showed more bone conduction loss than control group with normal temporal bone pneumatization with statistical significance. Comparing pneumatization of mastoid antrum and mastoid process, there was statistically significant bone conduction loss in orders of pneumatic type, diploic type, sclerotic type, cholesteatoma cavity. There was no difference of bone conduction hearing level according to age, sex, and duration of disease process with statistical significance. The results suggest that the bone conduction threshold in chronic otitis media is influenced by artificial elevation due to conductive defect and can be improved by reconstructive surgery.Many patients being followed for chronic suppuration of the middle ear were observed to have sensorineural or mixed type hearing loss. The major cause of this bone conduction loss might be secondary to cochlear biochemical changes occuring through the round window membrane. But, clinical and statistical analysis of bone conduction loss in chronic otitis media has not been exactly reported until now.       • 안와 내벽 외향 골절의 내시경적 비내 재건술

        The orbital blowout fracture occurs when a convex object(usually a baseball and a fist) larger in diameter than orbital rim strikes the orbit, increasing intraorbital pressure and producing hydraulic forces in all directions, which explodes the bony margin. The most frequently site of fracture is the bony floor in the region of the infraorbital canal, less frequently medial orbital wall, that is, lamina papyracea. Medial orbital wall fractures are also seen with other types of facial fractures such as frontal, nasoethmoid, and maxillary injuries. With the advent of nasal endoscope, endoscopic intranasal reconstruction has substituted for external approach. We experienced a case of blowout fracture of left medial orbital wall, which was treated with endoscopic intranasal reconstruction method and report this case with the review of the literature.
