        Protective effect of yeoldahanso-tang on radiation-induced oral mucositis in mice

        Radiation-induced oral mucositis is probably the most common toxicity for head and neck cancer patients. This adverse effect potentially leads to various lesions such as erythema, ulcerations, pain, opportunistic infections, and weight loss. Although there are many experimental studies and clinical trials related to novel therapies for radiation-induced oral mucositis, we here evaluate the protective effect of orally administrated Yeoldahanso-tang (YDHS) on radiation-induced oral mucosal damage in a mouse model. YDHS is a traditional Korean herbal formula, containing 7 herbs: Pueraria thunbergiana Benth., Angelica tenuissima Nakai, Platycodon grandiflorum (Jacq.) A. DC., Raphanus sativus var., Angelica dahurica Benth. et Hooker f., Cimicifuga heracleifolia Kom., and Scutellaria baicalensis Georgi. Oral mucosal damage was induced by irradiation (17 Gy) throughout the head and neck area in male BALB/c mice. YDHS (400 or 800 mg/kg/day for consecutive 12 days) was administered orally, and the protective effect was determined by histological evaluation of the oral mucosa. We found that irradiation highly increased COX-2 expression in salivary glands and its expression was suppressed by YDHS. Histologically, the irradiated mice exhibited the epithelial thickness from the basement membrane to the stratum granulosum significantly decreased as compared to the vehicle group. YDHS-treated mice showed significantly recovered epithelial cell layer thickness compared to irradiated group. Similarly, YDHS-treated group also showed the expression of Ki-67, cell proliferation maker, in the basal layer compared to the irradiated group in both dorsal and ventral surface. From these results, we suggest that YDHS is a promising candidate for preventing or treating radiation-induced oral mucositis during radiation therapy.

        Refining prognostic stratification of human papillomavirus-related oropharyngeal squamous cell carcinoma: different prognosis between T1 and T2

        Purpose: To validate the 8th edition of the American Joint Committee on Cancer/Union for International Cancer Control (AJCC/UICC) TNM staging system for human papillomavirus (HPV)-related oropharyngeal squamous cell carcinoma (OPSCC) and investigate whether a modified classification better reflects the prognosis. Materials and Methods: Medical records of patients diagnosed with non-metastatic HPV-related OPSCC between 2010 and 2016 at a single institution were retrospectively reviewed. HPV status was determined by immunohistochemical analysis of p16 and/or HPV DNA polymerase chain reaction (PCR). We reclassified TNM stage T0-1 and N0-1 as group A, T2-3 or N2 as B, and T4 or N3 as C. Survival analysis according to 8th AJCC/UICC TNM staging and the modified classification was performed. Results: Of 383 OPSCC patients, 211 were positive for HPV DNA PCR or p16. After exclusion, 184 patients were included in this analysis. Median age was 56 years (range, 31 to 81 years). Most primary tumors were in the palatine tonsil (148 tumors, 80%). The eighth AJCC/UICC TNM classification could not differentiate between stage I and II (p = 0.470) or II and III (p = 0.209). Applying modified grouping, the 3-year overall survival rate of group A was significantly higher than that of group B and C (98% vs. 91%, p = 0.039 and 98% vs. 78%, p < 0.001, respectively). Differentiation between group B and C was marginally significant (p = 0.053). Conclusion: The 8th AJCC/UICC TNM staging system did not clearly distinguish the prognosis of stage II from that of other stages. Including the T2N0-1 group in stage II may improve prognostic stratification.

      • Enhancement of Detection Accuracy in Position-sensitive Plastic Scintillating Fiber-based Sensor Using ANN Model

        In this study, we evaluate artificial neural network (ANN) models that estimate the positions of gamma-ray sources from plastic scintillating fiber (PSF)-based radiation detection systems using different filtering ratios. The PSF-based radiation detection system consists of a single-stranded PSF, two photomultiplier tubes (PMTs) that transform the scintillation signals into electric signals, amplifiers, and a data acquisition system (DAQ). The source used to evaluate the system is Cs-137, with a photopeak of 662 keV and a dose rate of about 5 μSv/h. We construct ANN models with the same structure but different training data. For the training data, we selected a measurement time of 1 minute to secure a sufficient number of data points. Conversely, we chose a measurement time of 10 seconds for extracting time-difference data from the primary data, followed by filtering. During the filtering process, we identified the peak heights of the gaussian-fitted curves obtained from the histogram of the time-difference data, and extracted the data located above the height which is equal to the peak height multiplied by a predetermined percentage. We used percentage values of 0, 20, 40, and 60 for the filtering. The results indicate that the filtering has an effect on the position estimation error, which we define as the absolute value of the difference between the estimated source position and the actual source position. The estimation of the ANN model trained with raw data for the training data shows a total average error of 1.391 m, while the ANN model trained with 20%-filtered data for the training data shows a total average error of 0.263 m. Similarly, the 40%-filtered data result shows a total average error of 0.119 m, and the 60%-filtered data result shows a total average error of 0.0452 m. From the perspective of the total average error, it is clear that the more data are filtered, the more accurate the result is. Further study will be conducted to optimize the filtering ratio for the system and measuring time by evaluating stabilization time for position estimation of the source.

        Geometric and dosimetric verification of a recurrent neural network algorithm to compensate for respiratory motion using an articulated robotic couch

        The purpose of this study is to evaluate the performance of a recurrent neural network (RNN)-based prediction algorithm to compensate for respiratory movement using an articulated robotic couch system. A prototype of a real-time respiratory motion compensation couch was built using an optical 3D motion tracking system and a six-degree-of-freedom-articulated robotic system. To compensate for the system latency from motion detection to re-positioning of the system, RNN and double exponential smoothing (ES2) prediction algorithms were applied. Three aspects of performance were evaluated, simulation and experiments for geometric and dosimetric evaluations, using data from three liver and three lung patients who underwent stereotactic body radiotherapy. Overall, the RNN algorithm showed better geometric and dosimetric results than the other approaches. In simulation tests, RNN showed 82% average improvement ratio, compared with non-predicted results. In the geometric evaluation, RNN only showed average FWHM broadening of 1.5 mm, compared with the static case. In the dosimetric evaluation, RNN showed average gamma passing rates of 97.4 ± 1.0%, 89.0 ± 2.4% under the 3%/3 mm, 2%/2 mm respectively. It may be technically feasible to use the RNN prediction algorithm to compensate for respiratory motion with an articulated robotic couch system. The RNN algorithm could be widely used for motion compensation in patients undergoing radiotherapy.

        Risk Assessment of Indirect Exposure from Municipal Solid Waste Incinerators

        본 연구는 위험도 평가 기법을 사용하여 도시 고형 폐기물의 소각서 방출되는 중금속이 간접 파폭 경로를 따라 소각로 주변 주민에게 미치는 위험도를 직접 피폭 경로의 위험도와 비교하여 평가하였다. 중금속은 유기 화학물질과는 달리 소각중에 파괴되지 않고 소각로로 부터 방출된 후. 주변 환경내에서 붕괴되지도 않는다. 따라서 소각로로 부터 방출된 중금속은 지상에 침적된 후 피폭 경로를 따라 주변 주민에게 영구적으로 암을 유발시킬 수가 있다. 본 연구 결과 간접 피폭 경로에 의한 암 위험도는 호흡기를 통한 직접 피폭 경로에 의한 암 위험도의 반 정도로 평가되었다. 그러므로 이러한 종류의 위험도 평가시 대상 지역의 간접 피폭 경로가 면밀히 조사되어야 하고 섭취 피폭 경로와 같은 간접 피폭 경로가 있을 경우 간접 피폭 경로를 통한 암 위험도도 평가되어야 할 것이다.

        LaNbO₄:RE<SUP>3+</SUP> (RE = Dy, Dy/Sm, Sm) 형광체의 구조, 표면, 광학 특성

        The effects of activator ion on the structural, morphological, and optical properties of LaNbO₄:RE<SUP>3+</SUP> (RE = Dy, Dy/Sm, Sm) phosphors were investigated. X-ray diffraction patterns exhibited that all the phosphors showed a monoclinic system with a main (112) diffraction peak, irrespective of the concentration and type of activator ions. The grain size showed a slightly decreasing tendency as the concentration of Sm<SUP>3+</SUP> ions increased. The excitation spectra of the LaNbO₄:Dy<SUP>3+</SUP>, Sm<SUP>3+</SUP> phosphor powders consisted of a strong charge transfer band centered at 259 nm in the range of 220-290 nm and five weak peaks. The emission spectra of the La0.95NbO₄:5 mol% Dy<SUP>3+</SUP> phosphors exhibited two intense yellow and blue bands centered at 575 nm and 479 nm respectively, which resulted from the ⁴F9/2→<SUP>6</SUP>H13/2 and ⁴F9/2→<SUP>6</SUP>H15/2 transitions of Dy<SUP>3+</SUP>. As the concentration of Sm<SUP>3+</SUP> was increased, the intensity of the yellow emission band was gradually decreased, while those of orange and red emission bands centered at 604 and 646 nm began to appear and reached maxima at 5 mol%, and then decreased rapidly with further increases in the Sm<SUP>3+</SUP> concentration. These results indicated that white light emission could be realized by controlling the concentrations of the Dy<SUP>3+</SUP> and Sm<SUP>3+</SUP> ions incorporated into the LaNbO₄ host crystal.

        바람직한 인사청문회제도의 정착과 발전방안에 관한 연구

        인사청문회는 행정, 입법, 사법의 3권 분립이 확실한 대통령제 국가에서 고위공직후보자의 업무능력과 자질을 검증하기 위하여 의회의 검증 절차를 거치게 하여 행정부를 견제하는 제도이다. 그리고 인사청문회제도는 공직후보자를 의회에 출석하도록하여 질의하여 답변 및 진술 등을 듣는 절차로서 의회가 주요한 공직에 임명하기 전에 공직후보자의 업무적합성, 도덕성, 자질 등을 검증하는 것이다. 2000년도 02월 16일 국회법의 개정으로 제46조의3 인사청문특별위원회와 제65조의2 인사청문회 규정을 신설하여 마침내 인사청문제도가 우리나라에도 도입되었다. 현재 운영되고 있는 인사청문회제도의 여러 가지 초래되는 문제점들을 파악하고 그 개선방안을 제시함으로써 고위공직후보자의 인사검증에 있어서 국회의 권력통제력적 기능을 확보하고 실효성을 제고하는 등의 법적 기초를 마련한다. Confirmation hearing is a proceeding adopted by a country with a presidential as asystem of government that strictly separates three powers. The congress holds the proceeding that act as a check on the presidential power to choose personnel for public positions by scrutinizing the eligibility and capabilities of the president’s nominees. The national assembly holds a confirmation hearing to appraise the qualification, morality, and job suitability of a nominee for public office such as constitutions or other equivalents. The candidate is required to attend the national assembly, answer their questions, and make statements. The National Assembly Act was amended in February 16, 2000 to newly insert Article 46(3) of Special Committee on Personnel Hearing and Article 65(2) of Hearings, thereby introducing the proceeding of confirmation hearing to Korea. A diagnosis of multiple problems with the current confirmation hearings and a suggestion of countermeasures would make it more effective to assess prospective high-ranking officials. A legal foundation would be laid to enable the National Assembly to fully keep the presidential power in check.
