Endotracheal intubation sometimes leads to complications, particularly in patients with anticipated difficult intubation. The GlideScope is preferred because of its high success rate in cases with anticipated difficult intubation. However, complications during the process have been reported. Most minor injuries can be treated after early detection. However, treatment can be difficult if complications occur later. We report a patient who underwent an emergency tracheostomy due to a delayed airway obstruction caused by pharyngeal wall bleeding during intubation with a GlideScope.
IEEE 802.15.4 슬롯 기반 CSMA/CA에서 시스템 처리율 향상을 위한 단말 수 추정을 통한 백오프 기간 튜닝 기법

본 논문에서는 IEEE 802.15.4 슬롯 기반 CSMA/CA(Slotted Carrier Sense Multiple Access with Collision Avoidance) 네트워크에서 시스템 처리율 향상을 위해 동작 중인 단말 수 추정을 통한 백오프 기간 튜닝 기법을 제안한다. IEEE 802.15.4 슬롯 기반 CSMA/CA 네트워크에서는 각 단말이 채널을 항상 센싱하지는 않으므로, 제안하는 기법에서는 채널을 항상 센싱할 수 있는 PAN(Personal Area Network) 코디네이터를 사용하여 동작 중인 단말 수를 추정한다. 이 때, 실시간 단말 수 추정을 위해 단순 이동 평균 필터를 사용한다. PAN 코디네이터는 추정한 단말 수에 따라 튜닝한 최적의 백오프 기간을 비콘 프레임에 담아 PAN 코디네이터에 연결된 단말들에게 전달한다. 시뮬레이션을 통해 제안하는 기법의 단말 수 추정에 대한 성능과 시스템 처리율에 대한 성능을 평가한다. 시뮬레이션 결과를 통해 제안하는 기법이 IEEE 802.15.4 표준보다 더 높은 시스템 처리율을 얻을 수 있음을 보인다. In this paper, we propose a scheme that tunes the backoff period for enhancing the system throughput with estimating the number of devices in IEEE 802.15.4 slotted carrier sense multiple access with collision avoidance (CSMA/CA) networks. Since each device does not sense the channel always in IEEE 802.15.4 slotted CSMA/CA networks, a personal area network (PAN) coordinator is used to estimate the number of active devices. The PAN coordinator broadcasts an optimal backoff period for the estimated number of devices through a beacon frame. In order to estimate the number of devices in run time, a simple moving average filter is utilized. We show the performance of our proposed scheme in terms of the estimated number of devices and the system throughput. The simulation results show that our proposed scheme can obtain higher system throughput than the IEEE 802.15.4 standard.
직장암 환자의 지주막하 15 % phenol in Glycerine 주입에 의한 암성통증 차단

This patient who was a 42-year-old female had suffered from bearing down sensation and pain on anal region due to metastatic rectal cancer. She was done intrathecal block with 15% phenol in glycerine. The result was exellant to bearing down sensation and pain on anal area. Because lower back pain, pain on lower extremity and generalized painful sensation was not improved, we has done pharmacologic assistance. Urination difficulty became slightly aggrevated but was improved at post-block day 4th. This pain clinic report that intrathecal block with 15% phenol in glycerine was effective to treatment of cancer pain without major complications.
백서 기관평활근에서 Lidocaine 과 상피 세포가 기관 장력에 미치는 영향

Background : Lidocaine is often administered intravenously to suppress airway reflexes associated with tracheal intubation or tracheal suction. In addition, lidocaine is known to have airway relaxant effects through a direct relaxant mechanism on the smooth muscle. The presence of airway epithelium has been reported to reduce the sensitivity and maximum contractile response to histamine or acetylcholine(ACh). The purpose of this study was to determine whether the cumulative application of lidocaine may cause a concentration-dependent relaxation of the rat tracheal smooth muscle strips with intact or rubbed epithelium. Methods : Using the rat tracheal smooth muscle strips, the effects of 10 6∼3 10 3M of lidocaine pretreatment on isometric tension induced by 40 mM of K+ or 10 5M of ACh in presence or absence of adherent epithelium, and the influences of 10 6M of propranolol, 10 4M of L-NAME and 10 6M of atropine on relaxing response of lidocaine were studied. Results : The tracheal smooth muscle concentration nduced by K+ and ACh was similar magnitude both in presence or absence of adherent epithelium. The removal of epithelium did not affect the relaxant effect of lidocaine on the K+ and ACh-induced tracheal smooth muscle contraction. Lidocaine pretreatment reduced Ca2+-dependent contraction of the rat tracheal smooth muscle. Following pretreatment of the tracheal smooth muscle preparations respectively with propranolol, L-NAME and atropine the relaxing responses to lidocaine of tracheal smooth muscle were not depressed. Conclusions : These results suggest that the effect of the epithelium on lidocaine-induced relaxation of the tracheal smooth muscle is not significant and lidocaine may directly relax tracheal smooth muscle by the influences on the Ca2+ mobilization.
