The aim of this study is to analyze the outcomes of the esophageal atresia with tracheoesophageal fistula over the last 2 decades. The records of 51 patients born between 1987 and 2006 were reviewed. Twenty-seven patients were male. Mean values of the age, gestational age and birth weight were 2.9 days, 296 days and 2.7kg, respectively. All patients had Gross type C anomalies. Thirty-one patients (60.7 %) had one or more associated congenital anomalies and the most common anomaly was cardiac malformation. In 48 cases, primary anastomosis was done and staged operation was done in one case. Circular myotomies in the proximal esophagus were performed in 9 cases. Postoperative complication developed in 26 cases (54 %): pulmonary complication in 12 cases, anastomotic leakage in 10 and anastomotic stricture in 10, recurrent trachoesophageal fistula in one and tracheomalasia in 2 cases. Reoperation was carried out in 2 patients with anastomotic leaks, the remaining leaks were managed non operatively. Three of the strictures were reoperated upon and the others were successfully managed by balloon dilatations. Overall mortality rate was 15.6 %. Mortality rate of the second 10 years (8 %) period decreased significantly compared to that of the first 10 years (23 %) period.
손목자해로 응급실에 내원한 환자수와 기후인자와의 관련성

연구목적 자살행동의 계절성은 잘 알려져 있는 현상이지만, 자해행동의 계절성은 알려진 바가 적다. 본 연구는 자해행동 중 하나인 손목자해행동이 계절성을 나타내는지를 확인하고, 기후인자와의 관련성을 파악하고자 했다. 방 법 자해행동의 계절성을 확인하기 위하여 2014. 12~2019. 5까지 일 병원 응급실에 손목자해행동으로 내원한 226명의 대상자의 월별 평균 내원 숫자가 월별로 차이를 나타내는지를 보았다. 손목자해행동과 기후인자와의 관련성을 확인하기 위하여 월 단위 시간변수와 기상청 데이터를 통해 획득한 월별 기후변수(기온, 일조량, 1개월 전 대기압)를 설명변수로 하고, 월 별로 손목자해행동으로 응급실에 내원한 횟수를 결과변수로 하여 일반화 가법모형을 이용한 다중 포아송 회귀분석을 실시하였다. 결 과 손목자해행동으로 응급실에 내원한 대상자의 월 평균 숫자는 남자에서 유의한 월별 차이가 있었고, 9월에 가장 많았다(남자 : p=0.048, 여자 : p=0.21, 전체 : p=0.28). 다중 회귀분석 결과 손목자해로 응급실에 내원한 월 평균 환자의 숫자는 남자에서는 1달전 대기압과 기온의 교호작용과 관련이 있었고(p=0.010), 여자에서는 일조량과 양의 상관관계[p=0.044, β=4.70×10-3, 95% CI=(1.19×10-4, 9.27×10-3)]를 보였다. 결 론 손목자해행동은 남성에서 두드러진 계절성을 보이며, 이는 기후변수의 변화와 관련성이 있다. Objectives : Although the seasonality of suicide is a well-known phenomenon, little is reported about the seasonality of non-suicidal self-injury. The purpose of this study was to identify the seasonality of wristcutting behavior and to examine its relationship with meteorological factors. Methods : To identify the presence of seasonality, we investigated whether there was a difference in the average number of visits per month to an emergency room (ER) of an urban hospital for 226 patients with wrist-cutting behavior enrolled between December 2014 and May 2019. To ascertain significant meteorological factors, we used the multiple Poisson regression using generalized additive model with time, monthly temperature, monthly sunshine hour, and atmospheric pressure in the prior month as explanatory variables. Results : In males, the average number of monthly visits to the ER for wrist cutting behavior differed by month and was the highest in September (male : p=0.048, female : p=0.21, total : p=0.28). As a result of multiple regression analysis, the average number of patients admitted to the ER for wrist cutting behavior was related to the interaction between atmospheric pressure in the prior month and temperature in males (p=0.010), and showed a positive correlation with sunlight in females [p=0.044, β=4.70×10-3, 95% CI=(1.19×10-4, 9.27×10-3)]. Conclusions : Wrist cutting behavior shows seasonality in male, which is associated with changes in meteorological variables.
저압에서의 사일렌과 디사일렌의 열분해 반응에 관한 연구

SiH₄와 Si₂H_6를 1~3 Torr 정도의 저압에서 열분해시켰을 때, 반응물의 농도 변화를 살펴보고 이로부터 열분해의 반응 기구를 예측하였다. 분석 기기로는 질량 분석기를 이용하였으며, 분해 온도 범위는 SiH₄의 경우는 350~475℃, Si₂H_6의 경우는 275~375℃ 이었다 . SiH₄의 분해 양상은 1차 비가역 반응에 잘 들어 맞았으며, 그 속도 상수는 문헌에 보고되어 있는 상압에서의 속도보다 작았다. Si₂H_6는 낮은 온도 범위에서도 잘 분해되었으며, 중간 생성물로 많은 양의 SiH₄를 만들었다. 그리고 , SiH₄는 고분자화되는 반응을 거치지 않고 고체 실리콘을 생성하지만, Si₂H_6는 중간 생성물로 만들어진 SiH₄와 SiH₂에 의하여, 고분자화 반응을 거쳐서 고체 실리콘을 만들 수 있음을 알았다. We have analyzed the pyrolysis products of SiR₄ and Si₂H_6 at pressures between 1 and 3 Torr under isothermal conditions, and proposed a reaction mechanism of the pyrolysis based on the results. Quadrupole mass spectrometer was used as an analysis instrument, and the pyrolysis was performed at the temperatures between 350℃ and 475℃ for SiH₄, and between 275℃ and 375℃ for Si₂H_6. The rates of SiH₄ decomposition followed the first-order equation and the rate constant was smaller than one reported for atmospheric pressure. Si₂H_6 decomposed relatively easily at lower temperature and produced SiH as an intermediate. The proposed reaction mechanism indicated that SiH₄ decomposed without production of polymer, but Si₂H_6 produced polymers that were initiated the intermediates of Si₂H_6 pyrolysis, i.e., SiH₄ and SiH₂.
