        간이식 중 폐동맥압의 변화

        Background: Pulmonary hypertension (PH) associated with end stage liver disease is rare but the risk of hemodynamic deterioration during liver transplantation may be high. This study was done to characterize the pulmonary hemodynamics during liver transplantation and to seek the relationship between pulmonary artery pressure (PAP) and other hemodynamic variables. Methods: One hundred patients undergoing liver transplantation were chosen and we divided patients into normal and PH groups (mean pulmonary artery pressure [MPAP] > 25 mmHg). Hemodynamic data was collected throughout the surgery. Studied variables between groups were analyzed with an unpaired t-test. The relationship between MPAP and other hemodynamic variables was analyzed with a linear regession test. Survival analysis was performed by cumulative survival analysis (Logrank test). Results: Incidence of PH during liver transplantation was 34%, and true PH (pulmonary vascular resistance index [PVRI] > 150 dyne.sec/cm^5/m^2, MPAP > 25 mmHg) was 7%. MPAP, systemic vascular resistance index, cardiac index, right ventricular ejection fraction, maximum elastance, central venous pressure (CVP), pulmonary artery occlusion pressure (PAOP), and right ventricular end-diastolic volume index were significantly higher in the PH group. In the PH group, right ventricular function curve was abnormal. MPAP correlated significantly with PAOP, and CVP (P < 0.01). One year survival rate showed no significant difference between groups (Logrank test P = 0.49). Conclusions: Episodes of increased pulmonary artery pressure during liver transplantation was not infrequent. PAP was more dependent on preloads. In patients with high PAP, RV diastolic dysfunctirnl was usually observed. Early mortality rate after liver transplantation was not associated with PH. (Korean J Anesthesiol 2001; 40: 340~347)

        Immediate Effect of the Complex Rotational Stretching Method on the Proprioceptive Sensation of the Shoulder Joint, Subacromial Space, Range of Motion, Shoulder Instability and Dynamic Function

        PURPOSE: This study compared the effects of proprioceptive sensation, subacromial space, and dynamic function according to proprioceptive neuromuscular facilitation (PNF), static stretching (SS), and complex rotational stretching (CRS). METHODS: Thirty students without any musculoskeletal disease who volunteered to participate were included in this study. The following metrics were measured to evaluate the function and stability under the normal conditions, with the PNF, SS, and CRS: special test and flexion, extension, abduction, adduction, internal rotation, external rotation (shoulder range of motion) and reaching distance on the medial (ME), superolateral (SL), inferolateral (IL), and subacromial space and proprioceptive sensation were evaluated. All measures were analyzed using one-way ANOVA and repeated measures of ANOVA. RESULTS: A clear difference in adduction in the range of motion was observed in all groups (p < .05). Significant differences could not be identified in all values in the error test, except for Ab (p < .05). Significant differences in reach were noted in all directions of the SS and PNF in the Me, SL, and IL (p < .05). After the intervention, significant differences in the average values could be identified in all groups except for the SS group after rest (p < .05). After the intervention, there was a significant difference between the CRS and SS and PNF groups (p < .05). CONCLUSION: The application of CRS is as helpful as the existing SS and PNF for improving the joint range of motion improvement, shoulder balance, and subacromial space.

        Sensitivity of SNF transport cask response to uncertainty in properties of wood inside the impact limiter under drop accident conditions

        It is essential to ensure the safety of spent nuclear fuel (SNF) transport cask in drop situation that is included in transport accident scenarios. The safety of the drop situation is affected by the impact absorption performance of impact limiters. Therefore, when designing an impact limiter, the uncertainty in the material properties that affect the impact absorption performance must be considered. In this study, the material properties of the wood inside the impact limiter were selected as the variables for a parametric study. The sensitivity analysis of the drop response of the SNF transport cask with impact limiter was performed. The minimum wood strength required to prevent a direct collision between the cask and floor was derived from the analysis results. In addition, the plastic strain response was analyzed and strain-based evaluation was performed. Based on this result, the critical values of wood properties that change the impact dynamic characteristics were investigated. Finally, the optimal material properties of wood were obtained to secure the structural safety of the SNF transport cask. The results of this study can contribute to the development of SNF transport cask, thereby ensuring safety in transport accident conditions

        「禹貢」의 和而不同적 공동체 모델

        「우공禹貢」은 정치지리학(政治地理學)의 전범(典範)이다. 「우공」에는 지리적 구역으로서의 “구주九州”와 정치적 구역으로서의 “오복五服”이 기록되어 있는데, 이들은 일통(一統)된 천하(天下)를 획정(劃定)한다. 「우공」의 오복은 현대정치지리학 이론에 부합하는 모델(model) 가운데 하나로서, 중앙의 구심력을 잘 표현해주고 있는 생동감 있는 정치적 모델이다. 구주에는 산줄기와 물줄기 등 고정된 자연지형으로서의 경계 이외에도 해당지역의 토질, 특산품 등도 기록하고 있는데, 그 가운데 본고에서 주목하는 부분은 지역의 구성원들이다. 「우공」에는 “도이島夷”, “우이嵎夷”, “래이萊夷”, “회이淮夷”, “화이和夷”, “삼묘三苗”, “서융西戎” 등 다양한 이족(異族)들이 등장하는데, 이들은 그 지역의 토착세력들로서 일통(一統)된 천하를 구성하는 다양한 성원들이다. 이들은 고래로부터 이어져 온 다양한 종족과 문화를 대표한다. 구주 속의 구성원과 문화 그리고 생산품들은 고정된 지리에 역동성을 가미해주며 이는 정치적 화해(和諧)를 지지(支持)한다. 기존의 수직적 역사구조를 수평적 역사구조로 전환해보면, 이들 구주와 그 구성원들은 일원(一元)에서 다원(多元)으로의 확대되어가는 문명발전구조와 일치하며, 「우공」의 일통(一統)속에서 이러한 화이부동(和而不同)적 공동체모델을 확인할 수 있다. 「Yugong禹貢」 is the epitome of political geography. 「Yugong」 records Geographical "jiuzhou(九州)" and political "wufu(五服)" that symbolizes a unified world. 「Yugong」’s wufu(五服) is one of the models that conforms to modern political geography theory. and It is a lively political model which expresses the central power of emotion. In addition to the fixed natural topography such as mountains and rivers, the Jiuzhou(九州) also records the soil and specialties of the area. In this paper focus on the members of the region. There is various kinds of nation such as "daoyi(岛夷)", "yuyi(嵎夷)", "laiyi(莱夷)", "huaiyi(淮夷)", "heyi(和夷)", "sanmiao(三苗)", "xirong(西戎)” in 「Yugong」. They are the indigenous forces of the region and various members of the unified nation. They represent the various tribes and cultures that have flowed from ancient. Jiuzhou(九州)'s members, culture, and products add dynamics to fixed geography that supports political harmony. If we convert the existing vertical history structure into a horizontal history structure, Jiuzhou(九州) and its members are consistent with the expanding civilization structure from one source to the other. In Yugong, we can see this unified collective model.

        Relationship between Serum Uric Acid Concentration and Acute Kidney Injury after Coronary Artery Bypass Surgery

        An elevated serum concentration of uric acid may be associated with an increased risk of acute kidney injury (AKI). The aim of this study was to investigate the impact of preoperative uric acid concentration on the risk of AKI after coronary artery bypass surgery (CABG). Perioperative data were evaluated from patients who underwent CABG. AKI was defined by the AKI Network criteria based on serum creatinine changes within the first 48 hr after CABG. Multivariate logistic regression was utilized to evaluate the association between preoperative uric acid and postoperative AKI. We evaluated changes in C statistic, the net reclassification improvement, and the integrated discrimination improvement to determine whether the addition of preoperative uric acid improved prediction of AKI. Of the 2,185 patients, 787 (36.0%) developed AKI. Preoperative uric acid was significantly associated with postoperative AKI (odds ratio, 1.18; 95% confidence interval, 1.10-1.26; P < 0.001). Adding uric acid levels improved the C statistic and had significant impact on risk reclassification and integrated discrimination for AKI. Preoperative uric acid is related to postoperative AKI and improves the predictive ability of AKI. This finding suggests that preoperative measurement of uric acid may help stratify risks for AKI in in patients undergoing CABG.

        고속도로 혼잡 완화를 위한 구간별 차등요금 부과전략

        This paper develops a differentiated pricing strategy over each segment of expressways based on the second-best pricing method for reducing congestion. To this end, a bi-level problem is proposed, in which the upper level of the model is formulated to determine toll level of each segment for minimizing traffic congestion, whereas the lower level of the model is formulated as a variable demand assignment problem. The sensitivity analysis based algorithm is took placed to find optimal solutions of upper level model. An application of the proposed model uses the modified Sioux-Falls network. The results show that the segment-based differentiated pricing strategy performs better than the existing uniform pricing strategy in reducing traffic congestion. This study can be applied as a demand management method to relieve disutility of excessively congested segments of expressways. 본 연구는 고속도로의 혼잡 완화를 위해 차선통행료 방식에 근거한 고속도로 구간별 차등요금 부과전략을 개발하는데 그 목적이 있다. 이를 위해 상위단계의 교통혼잡 최소화를 위한 각 구간의 주행요금을 산정하는 문제와 하위단계의 가변수요 통행배정문제로 구성된 바이레벨 형태의 모형을 제시하였다. 상위단계 문제의 최적 해를 찾기 위해 민감도 분석 기반의 알고리즘을 이용하였으며, 제안된 모형의 검증을 위해 수정된 Sioux-Falls 네트워크에 적용하였다. 적용 결과 차등요금 부과전략 적용 시 균일한 요금을 징수한 경우보다 교통혼잡이 개선된 것으로 나타났으며 이는 고속도로 구간의 혼잡 완화로 인한 것임을 확인할 수 있었다. 본 연구는 고속도로 특정 구간의 과도한 혼잡으로 인해 발생하는 비효용을 절감하기 위한 수요관리 방안으로 적용될 수 있다.
