        간절제 및 간이식 후 간기능 회복의 평가인자로서의 Apolipoprotein A1의 역할

        Purpose: The aim of this study is to determine whether the serum apolipoprotein A1(apoA1) level, as measured at different time points after hepatectomy and liver transplantation, can predict the synthesis ability of the liver and the nutritional status. We also investigated the usefulness of regions of interest(ROIs) as an indicator of the recovery status of the liver after liver transplantation. Methods: 93 patients (21: laparoscopic cholecystectomy, 53: partial hepatectomy, 19: liver transplantation) were operated on under general anesthesia. The serum levels of apoA1, prealbumin, albumin, aspartate aminotransferase and alanine aminotransferase and the prothrombin time were measured at pre- and post-operation. The liver conditions were a normal liver (50 cases), hepatitis (16 cases) and liver cirrhosis (28 cases). The mean hepatic attenuation was calculated by averaging the ROI values that were obtained at different hepatic segments. Results: The serum apoA1 level was minimally changed during the perioperative period in the laparoscopic cholecystectomy group. Yet in most cases, the serum apoA1 level after partial hepatectomy and liver transplantation was decreased on postoperative days (PODs) 1 and 7, but it nearly recovered to the preoperative level on POD 30. There were significant differences I the values of apoA1 between the normal liver and co-existent liver disease at the various time points. The ROI value after transplantation gradually increased and it reached a normal level by POD 30. Conclusion: The serum apoA1 level can be an indicator of liver’s ability to synthesize protein and the nutritional status after partial hepatectomy. In addition, ROIs of the unenhanced CT image can reflect the recovery status of the liver after transplantation.

        전신홍반루푸스 환자에서 발생한 피막성 경화복막염

        Sclerosing encapsulating peritonitis (SEP) is characterized by peritoneal fibrosis and adhesion of the peritoneum with the loops of the small intestine. Although the prevalence is low, most cases are caused by peritoneal dialysis, infection, medication, systemic lupus erythematosus (SLE), and intra-abdominal neoplasm. We describe a 22-year old man who was presented with abdominal pain and distension, which were attributed to SLE with peritonitis. He had no specific history of previous medical illness and peritoneal dialysis. He was treated with intravenous high dose methylprednisolone 1 g/day for 3 days, followed by intravenous methylprednisolone 1 mg/kg daily and immunoglobulin. However, his symptoms did not improve. Eventually, a laparoscopic biopsy was performed for an accurate diagnosis. The histopathologic findings were presented in accordance to the typical characteristics of SEP. In spite of medical treatment, he did not show an improvement of clinical symptoms and radiologic findings. As a result, he died from nutritional deficiency, upper gastrointestinal bleeding, and congestive heart failure.

        상품유사여부 판단에 관한 국내 상표심사실무의 대응 및 과제

        현행 상표심사를 담당하고 있는 심사관은 유사상품서비스업심사기준에 따라 상품의 유사여부를 판단하고 있다. 그런데, 그 심사기준이 거래시장의 현실적합성 및 심사의 정확성 측면에 있어서 문제가 있다는 지적이 있어왔다. 이에 2008년 특허청은 유사군 코드에 따른 심사방식의 문제를 해소하고 심사품질을 높이기 위해서 개별상품의 거래실정에 따른 심사제를 도입하였다. 그러나 이러한 특허청의 개선노력은 주로 절차적 측면에 한정됨으로써 실체적인 부분에 있어서의 접근은 부족하다고 할 것이다. 따라서 본 논문은 현행 상표심사실무의 현황 및 문제점, 특허청의 개선노력에 대한 평가, 향후 유사군코드에 의한 심사방식이 유지될 것을 전제로 한 보완사항 등을 중심으로 상품심사의 정확성 제고 방안을 모색해 보고자 한다. As the examiner's conclusion is bound by the rules for the similarity of goods, trademark examiner do not proceed examination individually to determine whether goods are similar to each other when it comes to examination practices. Since the presumption mostly attributes to the valid conclusion, the determination of similarity of goods along with similar group codes gain strength in examinations. The absence of experts who take full charge of goods trade has caused the lack of knowledge and information on the specific trade terms and conditions. Under this circumstance, KIPO has cursorily and indiscriminately conducted classification of goods and similar group codes that hardly reflected real goods trades from now on. In this regard, the similar group codes have limitations to catching up with the changes in goods and service industries according to the industry development and the changing culture. Considering the classification of goods is the rigid classification system that can not promptly respond to the market changes, it is imperative that the enhanced classification system should be designed to quickly reflect the ever changing market conditions in which new products are on the markets day by day.

        The mark is not registered because of descriptive mark on Trade Mark, if a specific mark represent quality, use, raw materials, etc. On the other hand, the specific mark which seems to hint or emphasize quality of the designation goods is named suggestive mark. The boundry between descriptive mark and suggestive mark is ambiguous more or less. But the suggestive mark must be distinguished from the descriptive mark, because it can be registered. Whether the specific marks have corresponded to the descriptive marks must consider both conception of mark and figure of that. Therefore, this article tries to observe the judgement standards, centering around Precedents, for example judgement subject, degree, and method, etc. or whether it is described with commonly used methods in terms of judging the descriptive mark.

        To find out the optimum design of hydrogen storage and supply tank using Metal Hydride (briefly MH) and to make clear the performance characteristics under various conditions are our research purpose. In order to use the low-temperature exhaust heat, LaNi_{4.7}Al_{0.3} which operates under the low pressure of 1 MPa is chosen, and we measure the basic properties, namely density, specific heat, PCT(Pressure-Concentration-Temperature) characteristics, and effective thermal conductivity. Then, a numerical calculation model of hydrogen storage using MH alloy is suggested and this thermal diffusion equation of model is solved by the backward difference method. This calculation results are compared with the experimental results of the systems which installed 1kg MH alloy and, it is found out that our calculation model can well predict the experimental results. By the experimental using MH alloy, it is recognized that the hydrogen flow rate can control by the step adjustment of brine temperature.

        The effective thermal conductivities of Mm;(La_{0.6-0.8});Ni_{4.0}Co_{0.6}Mn_{0.2}Al_{0.2} (TL-492) with hydrogen and helium have been examined. Experiment results show that pressure has great influence on effective thermal conductivity in Low pressure range (below 0.5 MPa). And that influence decreases rapidly with increase of gas pressure. The reason is at low pressure, the mean free path of gas becomes greater than effective thickness of gas film which is important to the heat transfer mechanism in this research. And, carbon fibers have been used to try to enhance the poor thermal conductivity of TL-492. Three types of carbon fibers and three mass fractions have been examined and compared. Naturally, the highest effective thermal conductivity has been reached with carbon fiber which has highest thermal conductivity, and highest mass fraction. This method has acquired 4.33 times higher thermal conductivity than pure metal hydrides with quite low quantity of additives, only 0.99wt% of carbon fiber. This is a good result comparing to other method which can reach higher effect ive thermal conductivity but needs much higher mass fraction of additives too.

        디자인보호법상 무심사등록제도의 문제점 및 개선방안

        Under exam system, KIPO introduced a unexam system to get design-right earily in 1998. In present design-law, double track systems, which substantial exam system is primary and unexam system is secondary are formed. Class of B1, C1, F3, F4, M1 are the unexamined design's articles. Some of item included each class has been superfluous as a unexam design, and the article to specify a unexam design has been appered due to circumstance and technical change. To improve matters of unexam registration system and establish stablily, coordinating unexam design's article is a main subject. Therefore, a study will review the problem of unexam registration system and grope the revision.

        간내 담석 수술 후 재발에 영향을 미치는 인자에 대한 연구

        Purpose: Hepatolithiasis, which is located in proximal sites of the common hepatic bile duct, is a common disease in Korea. Hepatectomy is a more effective treatment for achieving complete removal of stones and biliary strictures. However, the incidence of remnant or recurrent stones is still high. The aim of this study was to evaluate factors related to the recurrence of stones after surgical treatment of hepatolithiasis. Methods: From February 1999 to February 2009, we conducted a retrospective study of case records of 208 patients who underwent operations for hepatolithiasis at the Department of Surgery, Chonbuk National University Hospital. Results: The male to female ratio was 1:1.97; the mean age was 55 years. Hepatolithiasis was located in the left intrahepatic duct in 69.7% of patients, in the right intrahepatic duct in 14.4%, and in both ducts in 15.9%. Operative procedures were as follows: left lateral sectionectomy in 109 (52.4%), left hemihepatectomy in 26 (12.5%), posterior sectionectomy in 5 (2.4%), a right hemihepatectomy in 8 (3.8%), a choledocholithotomy in 38 (8.6%), a choledocho-enterostomy in 18 (8.6%), a choledochotomy in 1 (0.46%), a choledochal cyst excision in 1 (0.46%), and no primary operation in 2 (0.9%). Complete stone clearance was achieved in 76% after operative treatment. Biliary stricture was significantly associated with remnant stones (p=0.000). In addition, the incidence of recurrent stones was 15.3%. Recurrent stones were associated with biliary stricture (p=0.001), hepatectomy (p=0.003), and bilateral location of stones (p=0.016). During the follow up period, the mortality rate related to operative treatment was 0.96%. Conclusion: Biliary stricture, hepatectomy, and bilateral location of stones are associated with recurrent hepatolithiasis after surgery.
