        대동맥 축착증 수술치험 2례

        Coarctation of the aorta is classically a congenital narrowing of the upper descending thorac aorta adjacent to the site of attachment of the ductus arteriosus which is sufficiently severe that there is a pressure gradient across the area. Recently we have experienced two cases of coarctation of the aorta and successfully performed resection of the sites of coarctation and end to end anastomosis of the aorta. The first case was a juxtaductal type of coarctation of the aorta with PDA and the pathology of the lesion was a diaphragm with central narrow opening. And the resection length was about 0.5cm and aortic clamping time was 20 minutes. The second case was also juxtaductal type coarctation of the aorta with mild tubular hypoplasia of aortic isthmus, left SVC and the pathology was also a diaphragm with central narrow opening. And the resection length was about 0.5cm and aortic clamping time was 29 minutes. Both postoperative course was uneventful and the patients were discharged two weeks after operation.

        Exposure of the cardiac pacemaker is still a difficult problem associated with high morbidity. Grossly infected pacemaker systems had been required removal and insertion of a new system. It would highly desirable to develop a new method that could salvage an exposed well functioning unit because cardiac pacemaker is very expensive. We have experienced one patient with exposed well functioning cardiac pacemaker, which was relocated to contralateral subpectoral pocket and healed without complication. This relocation of the pacemaker to subpectoral pocket is less simple, expensive and should be considered as an alternative to complete replacement of unit.

        기관협착증 치험 3례

        Increasing success in the management of patients with severe respiratory failure by mechanical respirators has produced iatrogenic tracheal stenosis. And the surgical management of these lesions have provided a major field for tracheal reconstructive surgery. Recently we have experienced three cases of postintubation tracheal stenosis between December, 1985 and October, 1987 and successfully performed circumferential resection and end to end anastomosis of the trachea. The lesion of the first case which was located in the subcricoid level was resected about 2cm length with cervical incision. And the lesion of the second case located at the cuff site was also resected about 2.5cm length with cervical and median sternotomy incision. Also the lesion of the third case located at the stoma site was resected about 1.8cm length with cervical incision. The postoperative courses were uneventful but there was extubation difficulty in the third case because of stupor mentality and problem of secretion excretion. So we have observed the postoperative course after T-tube insertion.

        가끔 우리는 생각지도 못한 일이 생겨 돼지가격이 곤두박질치는 것을 본다. 지난 2002년 5월 경기 충북 일대에서 발생한 구제역이 바로 그런 경우인데, 양돈기업들은 그때 전 재산이나 다름없는 수만 마리의 돼지들을 땅속에 묻어야 했다. 돼지 가격을 흔드는 것은 구제역, 돼지콜레라 뿐만이 아니다. 돼지 가격이 좋을 때 갑자기 사육 두수가 늘어나면 언제 또 돼지 값이 폭락할 지 알 수가 없다. 지난 10년간의 돼지 가격은 거의 매년 폭락과 폭등을 반복하고 있다. 가격이 좋을 때에는 괜찮겠지만 문제는 가격이 폭락할 때이다. 가격이 좋아서 큰돈을 버는 것도 좋지만 거의 해마다 반복되는 가격 폭락을 피하고 돼지를 제값에 팔 수 있다면 얼마나 좋을까? 여기에 '선물거래'가 해답이 될 수 있다.

      • 스플라인을 이용한 기하학 형태 매칭방법

        특정한 부분에 정확성을 가지는 기하학 형태인식은 이미지 분석 응용에서 중요하게 다루어져 왔다. 또한, 부분적 혹은 변형된 모양을 계층화하는 새로운 접근방법에 대한 연구가 계속되고 있다. 그러므로 본 논문에서는 자유형태의 부드러운 곡선을 생성하는데 용이한 스플라인 공식들을 이용하여 새로운 기하학 형태 매칭 접근 방법을 제안한다. 이와 같은 방법에서, 여러개의 스플라인 공식들로 생성된 곡선들의 집합은 동일한 형태의 성질을 가진다. 본 논문은 곡선 설계를 위하여 일반적인 상호작용 방법과 다양한 스플라인 공식간에 관계들을 이용함으로서 간단한 설계점(design point)들의 이동으로 형태매칭 방법에 관한 관계성을 보인다. 그러므로, 본 논문에서는 3차 스플라인의 공식(B-splines, Bezier splines, Catmull-Rom splines)을 이용하여 형태 매칭 방법을 제안한다.

        시스템엔지니어링 전산지원환경 구축 절차에 관한 연구

        국내에 다양한 시스템 엔지니어링 전산지원도구(Computer Aided tools for Systems Engineering)가 도입되고 도구의 기능이 향상됨에 따라 시스템 엔지니어링 전산지원환경을 구축하는 사례가 늘어나고 있다. 전산지원환경을 구축하기 위해서는 기존의 국내외 시스템 엔지니어링 표준 및 지침에서 언급되지 않은 구체적인 업무에 대한 고려가 요구되기 때문에, 많은 노력과 시간이 소요된다. 시스템 엔지니어링 본 연구에서는 국내 연구소 및 기업의 시스템 엔지니어링 전산지원환경을 구축하는 과정에서 습득된 문제점과 경험적 교훈(lessons learned)을 정리하였다.

        This paper presents the general system theory and its applications to the safety analysis method that is a recent trend over the traditional event-driven model. This new model is known as STAMP(Systems Theory Accident Modeling and Process)proposed by Nancy G. Leveson in MIT. The new model has benefits of systemic approaches concerning the system safety as a whole including the context it is in, its stimulants and outcomes, and its parts as well as the relationships among them in a holistic manner. The method consists of a hierarchical control structure, a process model, and the safety constraints governing the control. This paper demonstrates an example the contrasts the differences between the approaches of STAMP and the traditional safety models.

      • 국외여행서비스 소비자의 불만족 요인과 불평행동에 관한 연구

        This study is focused on understanding how the consumer's actions of complaint are formed when the consumer begins to complaint about the contents of travel service, on how to remove the main causes of the consumer's discontent with the travel service, and on the way to persuade the consumer to become their regular guest in the long run. On the basis of the empirical analysis, I have examined the difference in the actions of complaint according to the complaint of the dissatisfied contents after the consumer's purchasing of the travel service. To remove the main causes of consumer's dissatisfaction with the travel service I would like to suggest the following : First, as to the consumer who likes to express his complaint about the travel service, it is necessary for the travel counselor to try to help him choose the travel service to satisfy his demands while trying to minimize any information by word of mouth and to make a personal contact with the him during the trip, in an attempt to clear up his discontent and thus cut off any the negative effects by word of mouth in advance. Second, as to the consumer who likes to seek for compensation it is necessary to help him fully recognize the true value of the travel service while making him relived and assured as well. Third, as to the consumer who honours justice and fairness, it seems necessary to try to show him the travel agency's firm determination to correct its mistakes, if any, while suggesting the appropriate alternative plan to compensate for the consumer's loss of opportunity cost.
