Purpose: The purpose of this study is the tooth decay ratio of the children and adolescents in four rural areas of Kyrgyzstan. Methods: Oral examinations were conducted 2,677 children and adolescents in Nizhniy chuy, Bokonbayevo, Chon Tash and Arashan from March 2017 to November 2019. It was investigated decayed tooth ratio(DT), decay permanent tooth ratio(DPT) and missing permanent tooth ratio(DMT) according to ages and areas by direct observation of the oral cavity. Results: In all areas, the rate of all children and adolescents aged 6 to 14 had very high DT of more than 94 percent overall. Furthermore, it was showed that high DPT rates above 78% on average and DMT over 8 years old in Chon Tash and Arashan. Conclusion: These results are higher than the average of DT, DPT and DMT in OECD countries. In order to promote oral health of children and adolescents, increased interest in oral health and policy measures at the Kyrgyzstan government level are considered urgent.
지르코니아 투명도 및 두께에 따른 레진 시멘트의 중합률

Purpose: The purpose of this study was to know of photopolymerization effect of self-etch dual-cured resin cement on different transmittance and thickness of zirconia disks. Methods: The two types of transparent and opaque zirconia speciments were prepared. The five speciments of each groups were seperated with 0.5㎜ and 1.0mm thickness. Degree of conversion(DC) were studied by FT-IR spectroscopy using ATR method before and after irradidaion for 40 sec. Results: The relative DC was showed the higher results of ZS5 as compared with ZS10 (p < 0.05). And OP5 and OP10 were lower results than ZS10 (p < 0.05). Conclusion: The photopolymerization effect of dual-cured resin cement were affected by the transmittance and thickness of zirconia.
CAD/CAM 시스템으로 제작한 지로코니아 코어의 변연 적합도 평가

본 연구의 목적은 CAD/CAM 시스템을 이용하여 지르코니아 코어를 제작한 다음 span의 길이가 변연 적합도에 미치는 영향과, 변연 간격이 임상적으로 허용 가능한 범위 내에 있는지를 평가하였다. 실험군은 지르코니아 single crown, 2-unit bridge, 3-unit bridge, 4-unit bridge의 4그룹으로 나누고 각 그룹별로 10개씩 코어를 제작하였다. 평균 변연 간격은 single 42.95±6.93㎛, 2-unit 43.53±5.27㎛, 3-unit 53.43±13.38㎛, 4-unit 50.85±8.25㎛으로 나타났다. 근심면, 원심면에서는 single, 2-unit 그룹과 3-unit, 4-unit 그룹 간 span의 길이 요인에 따라 통계학적으로 유의한 차이가 있는 것으로 나타났다(p<.05). 협면, 설면에서는 모든 그룹에서 span의 길이 요인에 따라 통계학적으로 유의한 차이가 없었다(p>.05). 이러한 결과는 지르코니아 코어의 span 길이가 변연 적합도에 영향을 주었고, 변연간격은 모든 그룹에서 임상적 허용 범위 내에 존재 하였다. The purpose of this study was to evaluate the marginal fitnesses on the effect of span length of bridge and the marginal gaps within the clinically allowed range. The samples were fabricated with zirconia core using CAD/CAM system. The test groups were divided by four groups, single crown, 2-units bridge, 3-units bridge and 4-units bridge and 10 zirconia cores per each group. The results of mean marginal gaps were 42.95±6.93㎛ in the single crown, 43.53±5.27㎛ in 2-unit, 53.43±13.38㎛ in 3-unit, and 50.85±8.25㎛ in 4-unit on each. The marginal gap of mesial and distal surfaces were statistically significant differences between single, 2-unit group and 3-unit, 4-unit group (p<.05) and this results was effected by the span length. The buccal and lingual surfaces were no statistically significant differences within all groups(p>.05). From this results the span length of the zirconia core may have influence on marginal fitness and the marginal gaps were within the clinically allowed range.

목적 : 감염된 인공 슬관절 치환술이 치료에 항생제를 혼합한 시멘트를 삽입하는 2단계 재치환술의 결과를 분석하고자 하였다. 대상 및 방법 : 1988년 3월부터 2001년 3월까지 감염된 인공 슬관절 치환술의 치료로 2단계 재치환술을 받고 최소 2년 이상 추시 관찰이 가능했던 11예를 대상으로 하였으며, 추시 기간은 평균 39.4 (28~60)개월, 수술 당시의 평균 연령은 64.0세였다. 감염의 진단을 위하여 국소 염증 소견, 백혈구 수치, 감별 혈구 계산, 적혈구 침강 속도, C-반응성 단백, 관절 천자 및 술중 동결 절편 (×400) 검사를 시행하였다. 결과의 평가를 위해 Hospital for Special Surgery (HSS)의 슬관절 평가 지수를 사용하였다. 결과 : 술 전 슬관절 운동 범위는 평균 73.4도에서 최종 추시시 평균 90.2도로 슬관절 운동 범위의 증가를 가져왔으며, HSS 기능 평가 점수는 술 전 평균 50.8점에서 최종 추시시 평균 79.1점으로 증가하였다. 최종 추시시에 임상, 검사 소견 및 방사선 소견상 재발은 없었다. 결론 : 감염된 인공 슬관절에서 항생제를 혼합한 시멘트를 삽입하는 2단계 재치환술을 시행하여 최소 2년 이상의 추시 관찰 결과는 좋았다. Purpose: To evaluate the result of treatment of the infected total knee arthroplasty by two-stage revision using antibiotics-impregnated cement. Meterials & Materials : We observed 2 staged revision cases of 11 infected total knee arthroplasty from March 1988 and March 2001. Average follow-up was 39.4(28~60) months. At the time of infection management, the average age was 64.0 years. Local inflammatory finding, WBC with differential count, erythrocyte sedimentation ratio, C-reactive protein, joint aspiration and intraoperative frozen section(×400) were used to diagnose infection. We assessed knee function according to the knee rating system of the Hospital for Special Surgery (HSS). Results : Prior to revision, the average HSS score was 50.8 points, and the average range of motion was 73.4 degrees. At final follow-up, the average HSS score was 79.1 points, and the average range of motion was 90.2 degrees. At the last follow up, there were no sign of recurrence of infection clinically and radiologically. Conclusion : At least 2 years follow up, the results of two-stage revision using antibiotics-impregnated cement were good.
도재용착용 Ni-Cr계 합금의 이트륨 첨가물이 도재전단결합강도에 미치는 영향

Purpose: In this study, to evaluate the effect of oxide changes on the shear bond strength according to the composition of Ni-Cr alloys for porcelain fused matal crown, T-4 alloys, Zeroy alloys and Zeroy-X alloys were selected. Methods: 20 specimens were fabricated using selected Ni-Cr alloys and porcelain powders. A Ni-Cr alloy having a diameter of 5 ㎜ and a height of 25 ㎜ was produced and the metal surface was polished. Porcelain powder was fired on the polished metal surface to a diameter of 5 mm and a height of 3 mm. The experiment group consisted of three groups, T-4(TNA), Zeroy(ZNA) and Zeroy-X(ZXA). The fabricated specimens were mounted on a jig of a universal testing machine(UTM) and fracture strength was measured by applying a shear force at a UTM crosshead speed of 0.5 ㎜/min. The fracture strength was calculated as the bond strength between the porcelain and the alloy. The surface of the fractured alloy was analyzed by X-ray diffraction(XRD) and scanning electron microscopy(SEM), and the components of the oxide were measured by energy dispersive X-ray spectroscopy(EDX) line profile method. Results: In SEM, XRD and EDX analysis, yttrium tended to increase the mechanical and chemical bonding forces. The shear bond strength of ZXA group containing yttrium showed the highest value at 27.53 MPa. Conclusion: Based on the results of this study, it is considered that the yttrium-added Ni-Cr alloy is clinically acceptable in porcelain shear bond strength.
