Purpose: Alloplastic implants, such as Silastic® , Supramid ®, Porous polyethylene, Teflon® have been used to prevent reherniation of orbital tissue and are known to be inert for many years, though complications are infrequently reported many years after their insertion. Complications associated with implants are infrequent, but infection, orbital hemorrhage, implant extrusion, motility restriction, migration of implant causing dacryocystitis, cystic formation have been described. The latter was known as a rare late complication of blow-out fracture repair. Methods: We report the case of a discovery of a intraorbital hemorrhagic cyst which developed after silicon implant insertion. This patient developed diplopia, unilateral proptosis, exophthalmos, vertical dystopia, ectropion 10 years after repair of blow-out fracture. In this case, orbital CT scan revealed intraorbital cyst surrounding the orbital implant. At surgery, a fibrous capsule surrounded the silicon implant and was filled with mucin pools. Results: Proptosis, diplopia, exophthalmos, ectropion, vertical dystopia were resolved after surgical removal of the cyst and implant. Conclusion: This case illustrate that it is important for us to be aware of the complication of cyst formation around the silicon implants.

다양한 방법을 이용한 이차 상안검 성형술

Surgical alterations of upper eyelids can have significant effect on the appearance of the individual. Blepharoplsty is a relatively simple method but it has the highest rate of complications. It includes supratarsal depression, excessively high and low crease, short or fading crease and asymmetric eyelids. Between March of 2003 and March of 2006, 50 cases of secondary correction of asymmetric eyelids were performed with various methods by authods. All patients had previously undergone primary upper eyelid surgery. The result were graded as excellent, good, fair, or poor, based on the correction of deformities, surgeon's satisfaction, patient's satisfaction. With an average follow up period of 8 months, 32(64%) patients were found to have excellent results, 15(30%) had good results, 3(6%) had fair results. Complications were 2 cases of hematoma, 2 cases of persistent swelling and 3 cases of remaining asymmetry. Secondary correction of eyelids require some considerations that extensive tissue scarring and adhesions, excessive fat removal, injury to the levator aponeurosis, and inaccurate placement of the anchoring sutures and so on. After careful preoperative evaluation and planning, using appropriate methods according to the etiologies of deformity, successful outcomes can be achieved.

Adverse drug reactions are very common in clinical practice, and skin is one of the most frequent organs for adverse drug reactions. We report a case of a 71-year-old male patient who developed skin eruptions after switching formulation of quetiapine immediate release(IR) to quetiapine extended release(XR). He had been taking quetiapine IR(400mg/day) for treatment of manic episode which was developed one year ago. The patient showed great improvement of symptoms after taking quetiapine IR for about one year, thus dosage of medication was reduced to 50mg/day on the average. Unfortunately dose reduction has tended to worsen symptoms, so dose of quetiapine was increased again to 200mg/day with formulation changes to XR. Two days after he took new formulation, erythematous papules were occurred over his anterior neck and ventral side of left wrist. As he stopped quetiapine XR, the skin lesions gradually subsided. And he was successfully treated with readministration of quetiapine IR without any skin lesions.

지방 대도시 도시재생 상 사유재산 활용의 한계와 과제에 관한 고찰: 대구광역시 사례를 중심으로

Since the establishment of a new act in 2003 opened the new era of urban regeneration, the needs on urban regeneration will continually grow in provincial metropolises like Daegu. However the promotion of the urban regeneration is going to be difficult due to the deterioration of the financial situation of local governments. In this context I aimed to find implications for the sustainability of the urban regeneration in provincial metropolises. I undertook a case study on Daegu by means of literature research and qualitative interview und suggested four action programmes from some implications derived from the practice in utilizing private property for public purposes : ① establishment of the “community board”, ② the consideration of private property in frame of the Urban regeneration strategy plan, ③creation of a management inventory, and ④ enactment of an based ordinance. 2013년 「도시재생 활성화 및 지원에 관한 특별법」 제정과 함께 본격적인 “도시재생의 시대”가 개막된 이래, 대구광역시를 비롯한 지방 대도시들의 도시재생 수요는 계속 증가할 것으로 예상된다. 문제는 지방 대도시의 재정상황 악화로 인해 향후 도시재생의 지속적인 추진을 담보하기 어렵다는 사실이다. 본 논문의 목적은 지방 대도시의 도시재생의 지속가능성을 담보하기 위한 방안을 모색하는 것이다. 이를 위해 본 연구에서는 대구광역시를 연구대상으로 하여 문헌조사 및 심층인터뷰를 통해 도출한 지방 대도시의 도시재생 과정상 사유재산 활용의 한계점을 바탕으로 하여 이를 해결하기 위한 4가지 방안을 제시하였다. 제안한 방안은 ① ‘(가칭)우리마을주민위원회’ 구성, ② 도시재생전략계획 내 사유재산 활용계획 수립, ③ 공익목적 활용 사유재산 관리인벤토리 작성, ④ 사유재산 활용을 제약하는 불합리한 규제완화이다.

통행료체계에서의 경로기반 통행배정모형 개발

이용자의 경로선택 형태를 모사하는 통행배정모형 결과의 정확도는 교통계획에 상당한 영향을 미친다. 이용자의 경로선택 결정과정에서 가장 중요한 판단기준은 통행시간과 통행요금이다. 그런데 통행요금은 이용자의 경로 거리에 따라 다양한 방식으로 부과되므로, 링크를 분석단위로 하는 기존의 통행배정모형은 현실적인 통행요금 반영이 힘들었고 또한 수요예측 결과를 이용한 다양한 분석에서 제약을 받아 왔다. 본 연구는 이러한 배경에서 경로교통량을 도출할 수 있는 경로기반 통행배정모형을 구축하였고, 또한 경로거리에 따라 결정되는 현실적인 통행요금을 반영할 수 있는 알고리즘을 개발하였다. 경로기반 배정모형에서는 GP(Gradient Projection) 알고리즘을 이용하였고, 계산상의 효율성 제고를 위해 K-최단경로 알고리즘 중 MPS(Minimal Path Search) 알고리즘을 이용하였다. 개발된 배정모형은 현실적인 통행요금을 반영할 수 있으므로 통행배정 결과의 정밀도를 향상시켰을 뿐만 아니라 기존 배정모형에 비해 최적해로의 수렴속도도 개선되는 것으로 나타났다. 본 논문의 배정모형은 경로교통량이 도출되고 통행요금을 반영할 수 있으므로, 통행요금과 통행거리 관계에 따른 목적함수의 규명과 그에 따른 효과척도를 계량화할 수 있다. 따라서 본 모형은 통행배정에서 실재상황을 보다 현실여건에 맞도록 규명할 수 있고, 기존의 제한적인 효과분석의 문제점을 해결할 수 있으므로 그 활용범위가 넓다.

아동·청소년의 한식 패턴 점수에 따른 식생활 평가: 국민건강영양조사 2007~2013년 데이터를 이용하여

This study was performed to assess the nutrient intake and dietary diversity of Korean children and adolescents using the Korean dietary pattern index developed in previous studies. For this study, 6,462 children and adolescents aged 7~18y who participated in the dietary intake survey (24h recall method) of the 2007~2013 KNHANES were sampled. The food items included in the Korean dietary index were jusik-ryu, guk/tang-ryu, gui/jjim-ryu, namul-ryu, yeomjangchaeso-ryu, jang-ryu and mitbanchan-ryu. All the subjects and both age groups (7~12y, 13~18y) were divided into quartiles. According to the results of this study, the range of the Korean dietary pattern score was 0~58 for all of the subjects and also in the 13~18 age group, and was 0~52 in the 7~12y age group. When the pattern score for each food group was compared across the quartiles of the Korean dietary pattern score, in all the subjects as well as in the 7~12y and 13~18y age groups, the pattern score for jusik-ryu and yeomjangchaeso-ryu was highest in Q1~Q4. or all the food groups, the mean pattern score was highest in Q4. These results suggested that the Korean dietary pattern score is highly associated with jusik-ryu including rice and yeomjangchaeso-ryu including kimchi. Accordingly, it is considered necessary to develop an index that reflects the characteristics of Korean cuisine and, at the same time, assesses the nutritional status and food consumption tendency of Korean children and adolescents.

도시구조적 차원에서 바라본 대구 대도시 유역의 폭염 가중원인 추정과 해결과제

The negative effects of heat waves on our society are well known through active research in the field of health and environmental disasters. In order to systematically reduce the risk of such a heat, it is necessary to search for countermeasures in the field of urban planning. The purpose of this study is to investigate the main points where artificial heat occurs and the wind direction and temperature condition during summer and winter events in Daegu city and surrounding cities. As a result, we confirmed that the urban structure of Daegu city is not appropriate for the heatwave, and the case of Daegu city showed that the solution of the urban structural problem should be basically preceded. In this study, four alternatives were selected as policy alternatives: urban cooperation for watershed management, systematic management of sources of artificial heat, gradual relocation of abundant artificial heat discharge facilities, and customized management of wind routes. 폭염이 우리 사회에 미치는 부정적 영향은 보건, 환경재해 분야의 활발한 연구를 통해 충분히 알려진 상태이다. 그러한 폭염의 위험을 체계적으로 줄이기 위해서는 도시계획 분야에서의 대응 노력 모색이 필요함에도 불구하고 아직까지 그 노력이 미진한 상태이다. 본 논문에서는 대구시와 주변 도시를 대상으로 폭염을 가중시키는 인공열이 발생하는 주요 지점과여름철과 폭염 이벤트 발생 시 풍향, 기온조건을 종합적으로 살펴보았다. 이를 통해 대구시의 도시구조가 폭염 대응에 적절치 않음을 확인함으로써, 대구시의 사례를 통해 폭염 문제 해결에 있어 도시구조적 문제의 해결이 기본적으로 선행되어야 함을 보여주었다. 본 논문에서는 정책대안으로 첫째, 유역단위폭염관리를 위한 도시협력체 구성, 둘째, 인공열 발생원에 대한 체계적인 관리, 셋째, 위치가 부적절한 인공열 다량 배출시설의 점진적인 이전, 넷째, 도시의 자연적 특성을 고려한 바람길의 맞춤형 관리 등을 제시하였다.
