Rupture of the anterior cruciate ligament (ACL) of the knee has been called the beginning of the end for the knee. There is a controversy over the indication for nonoperative teatment or operative management. The patient's goals, motivation, associated ligament injuries and severity of the injury are critical factors. The substitute of ACL has to be placed isometrically as mentioned by Ben Garf. Thirty-five patients with clinical and functional instability of the knee due to anterior cruciate ligament insufficiency underwent the repair or reconstruction. The patients were followed for a minimum of ten months, average twenty-nine months. There were thirty-two males and three females. The average age was twenty-nine years. The most common cause of injuries was traffic accident (85 %). Ruputure sites were found most frequently at the femoral attachment (43 %). Meniscal tears were noted in five patients (14 %). The most frequently combined injury was the reputre of medial collateral ligament. The results were as follows by O'Donough's criteria: 1. In regards to the rupture site and duration from injury to srugery, the results were facorable except for those groups with ruptured at the substance and operation performd in one to two weeks after injury. 2. Significant difference in the final results has been noted between the repair and reconstruction groups. The follow up duration was rather short and further long term study might be necesssry. When the general condition of patient was poor, the surgical treatment could be delayed for 2 to 3 weeks without compromising the final result.
