Dengue-associated hemophagocytic lymphohistiocytosis in adult: a case report and literature review

Infection associated hemophagocytic syndrome (IAHS) is potentially a fatal disease caused by systemic infection complicated by hemophagocytic lymphohistiocytosis (HLH). We report a case of Dengue hemorrhagic fever associated with HLH who visited Thailand. A 33-year-old healthy female patient presented with 3 days of fever, myalgia and skin rash. Serotype 3 dengue virus was isolated. Clinical and laboratory findings fulfilled the criteria of HLH. After the initiation of corticosteroid therapy, the patient recovered and laboratory findings were normalized. This is an interesting case of dengue hemorrhagic fever combined with HLH. It would be important to diagnose dengue-associated HLH differentially from severe dengue. Early recognition and initiation of steroid treatment of dengue hemorrhagic fever complicated by HLH is crucial on the outcome. Dengue infection is increasing worldwide as the international travelers increase. Dengue virus needs to be investigated as an important triggering factor for IAHS with patients who traveled to endemic areas.

Purpose: The purpose of this study was to identify a research trend of studies related to the Do-Not-Resuscitate (DNR) decision making process in Korea. Methods: Assessing through five computerized databases, 889 studies were reviewed and of these 32 were included. An integrative literature review and text network analysis were applied to examine the research. The keywords from each article’s abstracts were extracted by using a program, KrKwic. Results: The number of studies on DNR decision has been increasing, especially since 2011. A descriptive study design (59%) was most commonly used in the research. In relation to factors affecting DNR decision, 97% of the studies stated patient factors and 66% stated family factors. ‘Patient’, ‘DNR’, ‘decision’, ‘treatment’, ‘life’, ‘family’ were the major keywords, and ‘patient’ and ‘care’ were dominant keywords that ranked high in coappearance frequency. Conclusion: Studies related to DNR decision have been increasing, and themes of the studies have also been broader. Further research is required to investigate factors affecting DNR decision in specific populations such as cancer patients, the elderly, patients with end-stage of chronic diseases etc. Moreover, a comparative study is necessary to define differences of research trends related to DNR decision making process between Korea and other countries.
Status and Needs of Continuing Education for Trauma Nursing

Purpose: This study was conducted to status and needs for continuing education for trauma hospital nurses in Korea. Methods: Thirty nurses from the seven level I trauma center hospitals or trauma treatment systems were randomly selected and surveyed. The survey was conducted from March 1 to May 31, 2017. Categorical data were analyzed with Pearson chi-square tests and Continuous variables were analyzed with ANOVA. Results: Only 86 out of 204 nurses had received continuing education (42.1%). The current status of continuing education programs, delivering institution (p<0.001), education method (p<0.001), education period (p=0.003), number of participants (p=0.007), and instructors (p=0.014) were also significantly different from trauma center to trauma center. There were 108 (52.9%) nurses who responded that continuing education programs were “needed” 92 (45.1%) and “very much needed 16 (7.8%). According to each trauma center’s characteristics were significantly differences in the need for continuing education (p=0.089), subject selection method (p<0.001) and the number of continuing education sessions (p=0.043) depending on the hospital. Conclusions: It is necessary to consider differences between the hospitals to develop continuing education programs that reflect the needs of nurses, in order to improve the efficiency of and satisfaction with the educational programs.
Thermochromic Polydiacetylene Nanotube from Amphiphilic Macrocyclic Diacetylene in Aqueous Solution

<P>Creation of tubular structures through the self-assembly of macrocyclic molecules has gained great attention in the chemical, biochemical, and material sciences. Through a designed introduction of two octaethylene oxide and two diacetylene moieties, we prepared a macrocycle <B>MCDA-BisOEG</B>, which is water-soluble and photopolymerizable and displays a lower critical solution temperature (LCST) behavior. The hydrodynamic diameter of <B>MCDA-BisOEG</B>, measured by dynamic light scattering method, was ca. 6.5 nm at 25 °C and increased sharply to ca. 2 μm at temperatures above 34 °C (LCST). Below the LCST, the macrocycle in aqueous solution formed tubular structures, which upon UV irradiation generated blue conjugated polydiacetylene (PDA) nanotubes. The PDA nanotubes undergo a blue-to-red color change when heated above the LCST. No polymerization occurs above the LCST owing to the disordered aggregation of the diacetylene monomer. This is the first example of the preparation of a macrocycle-based, thermoresponsive, conjugated polymer nanotube in aqueous solution.</P> [FIG OMISSION]</BR>
