<▼1><P>Supplemental Digital Content is available in the text</P></▼1><▼2><P><B>Abstract</B></P><P>Data regarding kidney transplantation (KT) and dialysis outcomes are rare in Asian populations. In the present study, we evaluated the clinical outcomes associated with KT using claims data from the Korean national public health insurance program. Among the 35,418 adult patients with incident dialysis treated between 2005 and 2008 in Korea, 1539 underwent KT. An optimal balanced risk set matching was attempted to compare the transplant group with the control group in terms of the overall survival and major adverse cardiac event–free survival. Before matching, the dialysis group was older and had more comorbidities. After matching, there were no differences in age, sex, dialysis modalities, or comorbidities. Patient survival was significantly better in the transplant group than in the matched control group (<I>P</I> < 0.001). In addition, the transplant group showed better major adverse cardiac event–free survival than the dialysis group (<I>P</I> < 0.001; hazard ratio, 0.49; 95% confidence interval, 0.32–0.75). Korean patients with incident dialysis who underwent long-term dialysis had significantly more cardiovascular events and higher all-cause mortality rates than those who underwent KT. Thus, KT should be more actively recommended in Korean populations.</P></▼2>

To date, only a few large-scale studies have measured the effect of dialysis modality on mortality in Asian populations. Here, we sought to compare survival between incident hemodialysis (HD) and peritoneal dialysis (PD) patients using the Korean Health Insurance Review & Assessment Service database. This enabled us to perform a population-based complete survey that included 32,280 incident dialysis patients and followed them for a median of 26.5 months. To reduce biases due to nonrandomization, we first matched 7049 patient pairs with similar propensity scores. Using the log-rank test, we found the mortality rate in PD patients was significantly higher than that in HD patients. Subsequent subgroup analyses indicated that in older patients (55 years and older), with the exception of the subgroup of patients with no comorbidities and the subgroup of patients with malignancy, PD was consistently associated with a higher mortality rate. In younger patients (under 55 years), regardless of the covariates, the survival rate of PD patients was comparable to that of HD patients. Thus, while the overall mortality rate was higher in incident PD patients, mortality rates of some incident PD and HD patients were comparable in Korea.

Rabbit hemorrhagic disease virus (RHDV) is highly contagious and often causes fatal disease that affects both wild and domestic rabbits of the species Oryctolagus cuniculus. A highly pathogenic RHDV variant (RHDVa) has been circulation in the Korean rabbit population since 2007 and has a devastating effect on the rabbit industry in Korea. A highly pathogenic RHDVa was isolated from naturally infected rabbits, and the gene encoding the VP60 protein was cloned into a baculovirus transfer vector and expressed in insect cells. The hemagglutination titer of the Sf-9 cell lysate infected with recombinant VP60 baculovirus was 131,072 units/50 μl and of the supernatant 4,096 units/50 μl. Guinea pigs immunized twice intramuscularly with a trial inactivated RHDVa vaccine containing recombinant VP60 contained 2,152 hemagglutination inhibition (HI) geometric mean titers. The 8-week-old white rabbits inoculated with one vaccine dose were challenged with a lethal RHDVa 21 days later and showed 100% survival rates. The recombinant VP60 protein expressed in a baculovirus system induced high HI titers in guinea pigs and rendered complete protection, which led to the development of a novel inactivated RHDVa vaccine.

Face deformation is caused by various causes such as facial paralysis or asymmetry. Therefore, accurate cause analysis and diagnosis are required, but CT, MRI, and Nottingham Grading System are subjective in nature and are affected by clinician proficiency and fatigue. In order to reduce the time and cost burden of patients who have to continuously visit specialized institutions for treatment, this study aims to develop a facial analysis system according to changes in Korean vowels and facial expressions. First, the coordinate value of the face landmark was extracted based on the webcam image, and then the distance value between the left and right face landmarks was calculated. In addition, quantification was performed based on the actual length of the eyes and lips and the results of the system derivation, and the final system analysis results could be checked with figures and charts.

Recently, global environmental regulations getting more strict, and hence it is increasing that the interest and investment of automobile companies on the aluminum die casting parts to make light-weight vehicles. Die casting molds are fabricated by the machining of billets of tool steels such as AISI 4140 or H13, in general. But, it has disadvantages like a large material loss and long lead time. Meanwhile, a Wire Arc Additive Manufacturing (WAAM) process could be an alternative fabrication method of the die casting mold. It has advantages such as less material loss, short lead time, easy repair and modification, the chance to make a reinforced mold using dissimilar materials, etc. The 5 Cr - 4 Mo tool steel wire is a commercial tool steel solid wire which designed for the repair and modification of tools and molds. It has superior hot wear resistance and toughness as well as the high strength, thus has high potential to produce die casting molds via the WAAM process. On the other hand, the WAAM process is basically the process that stacking the layers repetitively using a conventional arc welding process based on the designed tool paths by CAD/CAM. Thus, the predictability and the reproducibility of every single layer are the most important core factor to obtain the advantages of the automated WAAM process. Due to its nature, a new process parameter whose named an interpass distance should be considered as well as the conventional process parameters such as current, voltage, travel speed, etc. However, there are lack of researches on the interpass distance of WAAM process. In this research, it was found that the interpass distance greatly affect the arc stability due to arc interference induced by the variation of distance from the prior deposited pass. That is to say that the more arc interference was observed as the less interpass distance was given. This phenomenon had been proven through the analysis of high-speed camera results and the variation in the amount of spatter. Further, it was observed that the interpass distance affects the dimension precision and predictability of automated WAAM process of the 5 Cr - 4 Mo tool steel. And it was found the critical interpass distance that could be utilized in the automated WAAM process of the 5 Cr - 4 Mo tool steel. The effect of the interpass distance on the arc interference would exist unless the interpass distance would be given over 100 %, thus, it is important to find out a point of compromise based on the arc stability, dimension predictability, and reproducibility.

Endocrine test data from a 13-year old intact female Maltese was indicative of the presence of an insulinoma, however ultrasonography identified a pancreatic mass only after 10 months after the first admission. Following identification of both pancreatic tumor and hepatic metastasis on computed tomography (CT), surgical excision of the mass was attempted. However, total excision failed because of tumor adhesion to adjacent large vessels. The pancreatic mass was monitored over the next 25 months via ultrasonography, CT, and positron emission tomography-computed tomography (PET-CT). Histopathological and immunohistochemical data confirmed the diagnosis of insulinoma with hepatic metastasis.
Assessment of energy saving effects in membrane-based seawater desalination

<P>In reverse osmosis (RO) process for seawater desalination, the most important issue is the energy consumption. Intensive efforts have been contributed to freshwater production from seawater at low energy so far. This work analyzes the amount of energy consumption for various energy saving methods proposed by literatures using five commercial RO simulation programs (e.g., CSMPRO Ver 5.0, IMS-Design - 2016, ROSA 9.0, LG Chem NanoH(2)O, and Toray DS2). The RO energy consumption can be saved by: (1) adopting high flux RO membranes (0.23-0.67 kWh/m(3)), (2) increasing feed water temperature (0.01-0.03 kWh/m(3)/degrees C), (3) increasing RO train size (1.17-1.35 kWh/m(3) for sizing up from 1,000 to 10,000 m(3)/d), (4) increasing RO membrane area (0.01-0.02 kWh/m(3)/% increase), and (5) decreasing feed concentration (0.02 kWh/m(3)/% decrease). If fouling occurs, the RO energy consumption increases by 0.05 kWh/m(3)/bar. Comparing the positive effect of energy saving methods and the negative effect of fouling, one can set an optimal strategy for energy saving in RO-based seawater desalination according to a specific situation.</P>
