Objectives: The A2142G and A2143G mutations in the 23S ribosomal ribonucleic acid (rRNA) of <i>Helicobacter pylori</i> are the most common mutations associated with clarithromycin resistance. This study aimed to determine the differences in <i>H. pylori</i> eradication rates in patients infected with bacteria carrying the A2142G and A2143G mutations who were treated with clarithromycin-based triple therapy. Methods: Data from a previous randomized controlled trial were analyzed retrospectively. Eradication rates were compared based on the presence of <i>H. pylori</i> carrying the A2142G and A2143G mutations. A meta-analysis was also conducted of relevant studies containing data regarding patients who received clarithromycin-based therapy due to infections with <i>H. pylori</i> harboring 23S rRNA mutations. Results: No significant difference was observed in <i>H. pylori</i> eradication rates between patients infected with wild-type bacteria (95.7% [44/46]) compared with those infected with bacteria carrying the A2142G mutation (100.0% [3/3]; <i>p</i>>0.9). However, the eradication rate was significantly lower for patients infected with bacteria carrying the A2143G mutation (16.7% [1/6]; <i>p</i><0.001) than for those infected with wild-type bacteria or bacteria with the A2142G mutation (100.0% [3/3]; <i>p</i>=0.048). In the meta-analysis, the between-group comparisons yielded similar results. Although patients infected with bacteria having the A2142G mutation exhibited no significant risk difference (RD) for eradication compared with those infected with wild-type bacteria (RD=-0.05 [-0.18 to 0.08]; I<sup>2</sup>=0%; <i>p</i>=0.42), those infected with bacteria having the A2143G mutation demonstrated a lower <i>H. pylori</i> eradication rate compared with patients infected with either wild-type (RD=0.72 [0.64–0.80]; I<sup>2</sup>=0%; <i>p</i><0.001) or A2143G mutant bacteria (RD=0.76 [0.61–0.91]; I<sup>2</sup>=0%; <i>p</i>< 0.001). Conclusions: The A2143G mutation may play a more significant role in clarithromycin triple therapy <i>H. pylori</i> eradication failure than does the A2142G mutation. Additionally, <i>H. pylori</i> strains with the A2142G mutation can be treated effectively with clarithromycin-based triple therapy.
Painful Hashimoto Thyroiditis in a 7-Year-Old Girl: Differential Diagnosis and Medical Treatment

Painful Hashimoto thyroiditis (HT) is a rare pediatric condition. Because of the rarity of painful HT, it can be misdiagnosed as other thyroid diseases, and there is limited evidence regarding its clinical course and treatment. A 7-year-old girl presented to the emergency room with neck pain. A physical examination revealed diffusely enlarged thyroid gland with firm consistency and without tenderness. Her serum free thyroxine level was decreased and levels of thyroid-stimulating hormone, thyroglobulin antibody, and thyroid-stimulating antibody were elevated. The ultrasonography revealed a diffusely enlarged thyroid gland with homogeneously hypoechoic parenchyma and lobulated contours. She was diagnosed with painful HT based on the clinical presentations, laboratory tests, and ultrasonography findings, and was treated with a steroid, levothyroxine, and nonsteroidal anti-inflammatory drugs, after which her clinical symptoms improved dramatically. We should consider the possibility of painful HT in children with neck pain; HT could be treated with medical treatment.

In multiple-input multiple-output (MIMO) systems, maximum likelihood (ML) detection is an optimal detection method. However, there is a problem for actual implementation since the computational complexity increases exponentially as the dimension of the MIMO systems or the modulation order becomes large. Metric Feedback Detection (MFD) is an detection algorithm which can perform ML detection with the low computational complexity through using the adaptive threshold. If the threshold calculation layer in MFD is changed according to the channel environment, the efficiency of the algorithm can be more increased. In this paper, we propose an efficient algorithm, which has the similar error performance to the ML detection, by applying clipping and adaptive layer techniques into the MFD in order to reduce the computational complexity. We verify the algorithm via computer simulation.

EPCglobal은 RFID 데이터 공유를 위해 EPC(Electronic Product Code)에 기반한 표준 아키텍처를 제안 하였다. 이 중에서 EPCIS (EPC Information Service)는 개별 아이템에 대한 이벤트를 저장하는 역할을 한다. 하지만 EPCIS 성능 테스트에 대한 방법이 기존에 연구 되지 않아 EPCIS의 성능을 측정할 방법이 없다. 따라서 이 논문에서는 EPCIS를 테스트 하기 위한 대상 환경 모델링과 테스트 데이터 생성을 위한 도구를 제안 한다. 이 도구는 사용자가 대상환경을 설정 할 수 있는 인터페이스를 제공하고 입력된 환경을 분석하여 이벤트를 고속으로 생성하는 기능 생성된 데이터를 고속으로 EPCIS에 입력하는 기능을 제공 한다.

최대우도 (ML: Maximum Likelihood) 검출은 다중 입출력 (MIMO: Multiple-Input Multiple-Output) 시스템에서 최적 신호검출 방법이지만 다중 입력의 개수나 변조 차수가 커질수록 계산 복잡도가 지수적으로 증가하여 실제 구현이 거의 불가능해진다. 본 논문은 MIMO 시스템에서 Breadth-First 알고리즘을 기반으로 최소 구현 복잡도를 가지면서도 정확히 ML 검출이 가능한 알고리즘을 제시하고 증명하며, 컴퓨터 모의 실험을 통해 검증한다. In multiple-input multiple-output (MIMO) systems, maximum likelihood (ML) detection is well known as an optimal detection for minimizing the average error probability of MIMO systems. The main drawback of ML detection, however, is that the computational complexity grows exponentially with the number of inputs and the modulation order. In this paper, we propose a simple ML detection algorithm based on the Breadth-First algorithm for MIMO systems. We show that the proposed algorithm is equivalent to the exact ML detection with the minumum computational complexity of the Breadth-First algorithm and it is validiated via computer simulation.
