        Clinical Features of Wrist Drop Caused by Compressive Radial Neuropathy and Its Anatomical Considerations

        Objective : Posture-induced radial neuropathy, known as Saturday night palsy, occurs because of compression of the radial nerve. The clinical symptoms of radial neuropathy are similar to stroke or a herniated cervical disk, which makes it difficult to diagnose and sometimes leads to inappropriate evaluations. The purpose of our study was to establish the clinical characteristics and diagnostic assessment of compressive radial neuropathy. Methods : Retrospectively, we reviewed neurophysiologic studies on 25 patients diagnosed with radial nerve palsy, who experienced wrist drop after maintaining a certain posture for an extended period. The neurologic presentations, clinical prognosis, and electrophysiology of the patients were obtained from medical records. Results : Subjects were 19 males and 6 females. The median age at diagnosis was 46 years. The right arm was affected in 13 patients and the left arm in 12 patients. The condition was induced by sleeping with the arms hanging over the armrest of a chair because of drunkenness, sleeping while bending the arm under the pillow, during drinking, and unknown. The most common clinical presentation was a wrist drop and paresthesia on the dorsum of the 1st to 3rd fingers. Improvement began after a mean of 2.4 weeks. Electrophysiologic evaluation was performed after 2 weeks that revealed delayed nerve conduction velocity in all patients. Conclusion : Wrist drop is an entrapment syndrome that has a good prognosis within several weeks. Awareness of its clinical characteristics and diagnostic assessment methods may help clinicians make diagnosis of radial neuropathy and exclude irrelevant evaluations.

        Pulsed Radiofrequency Neuromodulation for the Treatment of Saphenous Neuralgia

        A 65-year-old male presented with pain in his right medial calf. An imaging study revealed no acute lesions, and a diagnosis of saphenous neuralgia was made by a nerve conduction study. He received temporary pain relief with saphenous nerve blocks twice in a one-week interval. Pulsed radiofrequency neuromodulation reduced pain to 10% of the maximal pain intensity. At 6 months after the procedure, the pain intensity was not aggravated even without medication. Pulsed radiofrequency neuromodulation of the saphenous nerve may offer an effective and minimally invasive treatment for patients with saphenous neuralgia who are refractory to conservative management.

      • Emerging Variant Glioma: Glioblastoma with a Primitive Neuro-Ectodermal Tumor(PNET) Component

        A 81-year old male with multiple mass lesion of the right fronto-temporo-parietal lobe and right frontal lobe was admitted to our hospital due to hemiparesis. Neuroimaging studies showed ring-enhancing lesions in right fronto-temporo-parietal lobe and right frontal lobe on magnetic resonance imaging (MRI). Tumor tissue evaluated to be consistent with glioblastoma radiologically was subjected to total excision. Using a morphological and immunophenotypic approach, the predominant component of the tumor was found to bear the properties of classic glioblastoma. The other component was composed of undifferentiated areas that exhibiting small cell morphology and diffuse neuronal immunophenotype. The case was diagnosed as ‘Glioblastoma with primitive neuroectodermal tumor (PNET) component’. The patient who had been performed postoperative radiotherapy, showed shrinkage of non-operative lesion and no sign of recurrence and neurological deficit during 12 months follow-up. We report a rare case of glioblastoma with a PNET component that has a relatively favorable outcome following surgery with radiotherapy and then review previous reports.

        동작에 따른 피부변화 분석을 통한 동작센서 부착의 최적위치 탐색: 조정 동작을 중심으로

        하이테크놀로지를 여러 가지 다른 영역과 융합하고자 하는 노력이 다양한 연구 분야에서 시도되고 있으며, 스포츠웨어를 개발함에 있어 운동선수의 운동능력을 향상시키기 위한 분야에 이러한 첨단 기술들이 도입되고 있다. 본 연구는 스포츠 훈련에 도움이 되는 동작 센싱 스마트 스포츠웨어를 개발하기 위한 기초 연구로서, 조정 동작 시 관절의 움직임을 측정할 수 있는 동작 센서를 부착하기 위한 인체상의 최적 위치를 탐색하는 것을 목적으로 한다. 본 연구에서는 일관된 동작을 반복적으로 수행할때 관절의 변화가 큰 조정을 대상 스포츠로 선정하였으며, 조정 선수의 대표 체급인 중량급과 경량급의 피험자를 선정하여 동작에 따른 피부의 변화율을 측정하여 체급별 차이를 분석하였다. 먼저, 3차원 모션캡처 시스템을 이용하여 조정 동작 시 등, 팔꿈치, 엉덩이, 무릎 부위의 피부 변화를 촬영하고, 각 마커 간 거리의 변화율을 분석함으로써 체급에 따른 차이를 보이지 않으면서 동작에 따라 피부의 변화가 큰 부분을 인체 상에 도시하였다. 이를 바탕으로 동작 센싱용 스마트 스포츠웨어를 위한 센서 부착의 가이드라인을 제시하였다. Lots of interdisciplinary studies for fusion of high technologes and the other areas of research had been tried in these days. In sports training area, high technologies like a vital sign sensor or an accelerometer were adopted as training tools to improve the performance of the sports players. The purpose of this study is finding the proper locations on the human body for attaching the motion sensors in order to develop a smart sportswear which could be helpful in training players. The rowing was selected as a subject sport as lots of movements of the joint on human body could be seen in rowing motions. The players of rowing could be devided into two weight divisions, the lightweight and the heavyweight. In this study, the change rates of distance between markers on human skin as the players moved were took on the back, the elbow, the hip and the knee area on human body by 3D motion capturing system. The distances between markers and the differences between the lightweight and heavyweight were analyzed. Finally, this study provided the guide lines for designing a motion sensing smart sportswear.

        고종 친정 주도세력의 개방 정책 추진과 조일수호조규 체결

        조일수호조규의 체결은 조선이 근대 국제법 체제로 들어가는 첫걸음으로 주목되어 왔다. 당시 조약의 체결을 주도한 세력은 박규수로 대표되는 시무개혁 세력과 이유원으로 대표되는 기존 관료세력으로, 이들은 1862년 농민항쟁의 수습과 세도정권의 교체, 대원군 실각과 고종의 친정 등 19세기 조선 집권층이 걸어온 정치 노선을 대표하는 존재였다. 이들이 가지고 있었던 현실에 대한 이해는 사림정치의 이상을 추구하며 발전한 조선의 전통적 정치 문화와 사상의 범위를 뛰어 넘는 것은 아니었다. 이들은 전통적 세계관 속에서 연합하여 고종의 친정을 주도하고 친정 정부에 참여하였다. 또한 대원군의 강경한 척화론이 불러올 수 있는 전쟁의 위험을 우려하였고, 일본 서계의 접수를 주장하며 일본에 대한 개방 정책을 추진하였다. 일본의 요구에 대응하는 데 있어 이들이 가지고 있었던 정책 목표는 무력충돌의 방지를 통한 민생의 안정이었다. 그렇기 때문에 일본과 협상 자리에 조선측 대표로 외교관이 아닌 무관 신헌이 파견되었으며, 조일수호조규 체결 과정에서 보여준 조선 정부의 대응은 평화 관계 유지라는 정책 목표 하에 일본의 침략 시도에는 단호하게 대처한다는 강력한 대응책이 함께하고 있는 것이었다. 협상의 현장에서 전견 대관 신헌은 이러한 조선 정부의 입장을 충실히 구현하였으며, 평화적인 결말을 이끌어 냈다. 이로써 고종의 친정을 이끌었던 조선 집권층의 목표는 달성될 수 있었다. The signing of the Korea-Japan Treaty has been noted as a first step into the modern international law system. However, those who led the signing of the treaty were the reformists, represented by Park Gyu-su, and the existing bureaucrats, represented by Lee Yoo-won. They represented the ruling cliques’ political line in Joseon in the 19th century, including handling of the farmers" uprising in 1862, the replacement of clan politics, and Daewon-gun’s fall and King Gojong’s direct ruling. Their understanding of the reality did not go beyond the scope of Joseon"s traditional political culture and ideas that was developed in pursuit of the ideals of Sarim politics. They united and participated in King Gojong’s directly ruling government sticking to the conservative worldview. Also, King Gojong’s advocacy force concerned about the danger of a war that Daewongun"s firm stance toward the rejection of negotiations could lead to; therefore, they asserted that Joseon accept the letter from Japan and carried forward an open-door policy toward Japan. Their first goal was to stabilize the people"s livelihood by preventing armed conflict. Thus, they dispatched Sin Heon, who was a military officer and not a diplomat. The Joseon government"s response to the signing of the Korea-Japan Treaty was based on the basic principle of maintaining a peaceful relationship along with a strong countermeasure against invasions. In this negotiation situation, Sin Heon, who was the chief negotiator, carried out his duties faithfully based on the Joseon government’s stance. The goal of Joseon"s ruling elites who led King Gojong"s direct ruling could be achieved by drawing a peaceful conclusion.

        CaO-SiO2-Al2O3-MgO계 슬래그 중 Cu와 Ni의 용해도

        In this study, the solubility of Cu and Ni, which are the main metal components of waste PCB in a CaO-SiO2-Al2O3-MgO slag system, was investigated by the chemical equilibration technique in the temperature range 1673-1823 K, under a CO-CO2 atmosphere. The concentrations of Cu and Ni in the slag increased with increasing oxygen partial pressure and MgO content in the slag. The concentrations of Cu and Ni in the slag decreased with increasing temperature. The concentration of Cu in the slag decreased with increasing activity of CaO, but that of Ni increased. The dissolution mechanisms of Cu and Ni in the CaO-SiO2-Al2O3-MgO slag could be described by the following equations from the effect of oxygen partial pressure and slag basicity on Cu and Ni dissolution behaviors: From the results obtained, the enthalpy changes from dissolution of Cu into the CaO-SiO2-Al2O3-MgO slag were calculated to be -59.17 kJ/mol, and that for Ni was calculated to be -78.89 kJ/mol.
