Lichen nitidus is an uncommon chronic inflammatory disease characterized by multiple minute discrete flesh-colored dome shaped papules. Its distribution is frequently localized on trunk, arm or penis. There are a few cases reported to occur on palm and sole. We report a case of lichen nitidus which presents erythematous or brownish colored hyperkeratotic plaques, composed of 1-2 m sized small papules on the right palm. Its histopsthologic findings were consistent with a diagnosis of lichen nitidus. (Korean J Dermatol 2003;41(3) : 383∼384)

Basal cell carcinoma(BCC)_s are most frequently found on the sun-exposed skin of the head and neck regions. Rarely, BCC_s occur in odd sites such as the scrotum, vulva, perineum, inpple, and axilla without any apparent carcinogenic factor such as arsenic, rediation, chronic ulceration or scars. Genital BCC accounts for fewer than 1% of all BCC_s and vulvar BCC_s represent fewer than 5% of all vulvar neoplasms. We report a case of BCC arising on the mons pubis clinically mimicking malignant melanoma, which presents with 2㎝ sized, deep purple colored protruding mass. (Korean J Dermatol 2003;41(12):1674~1676)

A blue protein was purified from the Methylobacillus sp. strain SK1 that is grown on methanol in the pres-ence of copper ion. This protein was found to be a monomer with a molecular weight of 13,500. The Isoelectric point of the protein was estimated to be 8.8. The spectrum of the protein that was treated with fer-ricyanide showed a broad peak around 620 nm, but that of the dithionite-treated protein revealed no peaks. It contained 0.83 mol of EDTA-stable copper per mol protein. Under air, the protein accelerated the inacti-vation of methanol dehydrogenase (MDH). The pro-tein was reducible by phenazine methosulfate or by active MDH that was prepared from cells that were grown in the absence of added copper, but not by methanol, dichlorophenol indophenol, or inactive MDH that was prepared from cells that were grown in the presence of added copper. It was also reducible by active MDH in the presence of methanol. The absorp-tion peak at 340 nm of the active MDH disappeared after the enzyme was treated with ferricyanide, hy-drogen peroxide, or the purified blue protein. The in-active MDH also showed no peak at 340 nm. The 340-nm peak was not recovered after incubation of the inactive MDH and blue protein-treated active MDH with dithionite or methanol. The inactive MDH and blue protein-treated active MDH co-migrated with the active MDH preparation on nondenaturing poly-acrylamide gel, and contained two non-identical sub-units with molecular weights that were identical to those of the active MDH. The N-terminal amino acid sequence of the protein was Ala-Gly-Cys-Ser-Val-Asp-Val-Glu-Ala-Asn-Asp-Ala-Met-Gln-Phe. An analysis of the amino acid composition revealed that the pro-tein contained no tryptophan. It contained three cys-teines per mol protein. The blue protein in Methyloba-cillus sp. strain SK1 was produced only in the cells that were grown in the copper-supplemented medium.

Background: Coughing is a side effect of opioids that is rarely studied. Here, we evaluated the incidence of remifentanil induced coughing during anesthesia induction in an attempt to identify its risk factors and to examine the preventive effects of lidocaine and salbutamol. Methods: A total of 237 patients scheduled to undergo general anesthesia were allocated randomly into three groups. Group C received no medication, while Group L received 2% lidocaine at 0.5 mg/kg intravenously 1 minute prior to remifentanil infusion and Group S inhaled one metered aerosol puff of salbutamol 15 minutes prior to entering the operating room. Remifentanil was infused at 5 ng/ml by target controlled infusion and coughing was measured for five minutes and graded as none, mild, moderate, or severe based on the number of coughs. Results: The incidences of coughing were 30.4%, 25.3%, and 35.4% in Groups C, L, and S, respectively. The incidences, onset times, and severity of coughing did not differ significantly among groups. In addition, multivariate analysis showed that non-smoking and a lower body weight were risk factors of remifentanil-induced coughing (odds ratio, 8.13; P = 0.024, 1.11, and 0.004, respectively). Conclusions: The incidence of remifentanil-induced coughing was 30%. A total of 0.5 mg/kg lidocaine and 1 metered aerosol puff of salbutamol did not prevent coughing. Non-smoking and low body weight were found to be risk factors of remifentanil-induced coughing. (Korean J Anesthesiol 2010; 59: 319-322)
