        Intraoperative femoral condyle fracture in primary total knee arthroplasty - a case-control study in Asian population

        Purpose: Intraoperative femoral condyle fracture is a significant but rarely reported complication during primary total knee arthroplasty (TKA). This is the first study to identify the incidence, risk factors, location and outcome of these fractures in an Asian population with modern posterior-stabilized (PS) TKA. Materials and methods: We reviewed 2682 consecutive primary TKAs performed between 2011 and 2017 in a single centre; 23 femoral condyle fractures were identified and analysed. Results: Fractures were managed conservatively with screw fixation or revision arthroplasty. Mean follow up was 3.0 years (range 3 months to 5.9 years). All patients achieved bone union and good functional outcome. The mean Knee Society Knee score was 89.4 (range 71–100) and the function score was 80.2 (range 60–95) at a mean of 3.0 years post-operation. Bilateral surgery was found to be a significant risk factor for femoral condyle fracture, while there was a higher trend of fracture in female patients and Stryker articular surface mounted (ASM) navigation. Conclusions: Intraoperative fracture is not uncommon with modern PS TKA. Postulated risk factors for fracture were discussed. Early identification of risk factors and a rigorous surgical technique may reduce risk of fracture. A good functional result was expected after proper treatment.

        Overview of systematic reviews with meta-analyses on acupuncture in post-stroke cognitive impairment and depression management

        Background: Acupuncture has been using as an alternative non-pharmacological therapy in the management of post stroke depression and cognitive impairment but its effectiveness and safety remain controversial. We conducted an overview of systematic reviews with meta-analyses to evaluate the evidence on the effect of acupuncture in the treatment of stroke with conventional medicine intervention. Methods: Systematic reviews summarized the treatment effects of acupuncture for post stroke cognitive impairment and post stroke depression were considered eligible. Methodological quality of included systematic reviews was assessed using A MeaSurement Tool to Assess systematic Reviews 2 (AMSTAR 2). Results: Four systematic reviews on post stroke cognitive impairment and ten systematic reviews on post stroke depression with good methodological quality were included. Meta-analyses revealed that acupuncture plus cognitive rehabilitation; and acupuncture or moxibustion plus cognitive rehabilitation, versus cognitive rehabilitation demonstrated statistically significant increase in Mini-Mental State Examination scores in compared to cognitive rehabilitation after 4 weeks treatment [Pooled weighted mean difference (WMD) = 3.14, 95% confidence interval (CI) = 2.06 to 4.21, I2 = 36%]; and (Pooled WMD = 3.22, 95% CI = 2.09 to 4.34, I2 = 0%). Furthermore, acupuncture versus antidepressant demonstrated statistically significant improve depression measured by increasing in 17-item Hamilton Depression Rating Scale in comparing to cognitive rehabilitation after 2 weeks treatment (Pooled WMD= -2.34, 95% CI= -3.46 to -1.22, I2 = 5%). Acupuncture usage was not associated with increased risk of adverse events. Conclusions: Acupuncture is safe and improves cognitive function and depressive disorder without obvious serious adverse events for post stroke patients.

        N/A To evaluate the diagnoostic value of ADA and lysozyme activity of serum and pleural fluid, we measured the ADA and lysozyme activity of serum and pleural effusions in 50 cases of pleural effusions due to tuberculosis (n=33), malignancy (n=13), and congestive heart failure (n=4). The results were as follows: 1) The mean value of serum ADA activity of tubercu-lous effusion was 45.533,3 U/L and was significantly higher than that of nontuberculous effusion(p<0,05). 2) The mean value of pleural ADA activity of tuber-culous effusion was 11.7±47.9 U/L and was significantly higher than that of nontuberculous effusion(p<0.005). 3) The mean value of the pleural and serum ADA ratio of tuberculous effusion was 2.9±1.4 and was significantly higher than that of nontuberculous effusion (p<0.05). 4) The serum lysozyme activity was not significant between tuberculous and nontuberculous effusions. 5) The mean value of pleural lysozyme activity of the tuberculous effusion was 18.0±10.3 μg/ml and was significantly higher than that of nontuberculous effusion. 6) The mean value of the pleural and serum lysozyme ratio of tuberculous effusion was 1.8±1.1 and was significantly higher than that of nontuberculous effusion. The ratio above 1.2 had a sensitivity of 70% and a specificity of 100% when used as a screening test for tuberculous effusion compared with malignant effusion. In conculusion, the measurements of serum and pleural ADA, lysozyme and their ratios were useful in the differential diagnosis between the tuberculous and nontuberculous pleural effusions.

        Analysis of RF-DC Conversion Efficiency of Composite Multi-Antenna Rectifiers for Wireless Power Transfer

        This paper studies the radio frequency to direct current (RF-DC) conversion efficiency of rectennas applicable to wireless power transfer systems, where multiple receive antennas are arranged in serial, parallel or cascaded form. To begin with, a 2.45 GHz dual-diode rectifier is designed and its equivalent linear model is applied to analyze its output voltage and current. Then, using Advanced Design System (ADS), it is shown that the rectifying efficiency is as large as 66.2% in case the input power is 15.4 dBm. On the other hand, to boost the DC output, three composite rectennas are designed by inter-connecting two dual-diode rectifiers in serial, parallel and cascade forms; and their output voltage and current are investigated using their respective equivalent linear models. Simulation and experimental results demonstrate that all composite rectennas have almost the same RF-DC conversion efficiency as the dual-diode rectifier, yet the output of voltage or current can be significantly increased; in particular, the cascade rectenna obtains the highest rectifying efficiency.

        Flight Test Measurement and Assessment of a Flapping Micro Air Vehicle

        Flight test of flapping micro air vehicles (FMAVs) is carried out using an instrumented measurement system to obtain various engineering parameters and hence to assess the flight performance of the vehicles through the data investigation. An indoor flight test facility equipped with a motion capture system and tracking cameras is used for the work presented in this paper. Maneuvers including straight-level flight, ground flapping, takeoff and landing are tested. Spatial position and orientation data are obtained from the retro-reflective tracking markers attached to the vehicles. Subsequent test analysis is carried out by generating performance parameters from raw data and then assessing the flight performance by comparison of the vehicles. The main findings of this work confirm that the test method and procedures presented here enable the systematic numerical data measurement and assessment of the flying performances of these vehicles, and show the applicability for the test and evaluation of general flapping MAVs.

        Non-Hodgkin’s Lymphoma Presenting as Intracardiac Mass: A Rare Presentation

        Primary cardiac lymphoma (PCL) comprises of 1%–2% of primary cardiac tumors. This case report describes a rare case of PCL with rapid disease progression. A 51-year-old female presented with 6 months of dyspnea. Her transthoracic echocardiogram showed a large exudative circumferential pericardial effusion and bi-atrial masses. An endovascular tissue biopsy using intra-cardiac echocardiography guidance enabled the diagnosis of diffuse large B-cell lymphoma. Staging computed tomography staged the disease at Ann Arbor stage IV E. She achieved a complete response to rituximab, cyclophosphamide, hydroxydaunorubicin, vincristine, and prednisolone chemoimmunotherapy.

        <B>Purpose</B> - The purpose of this paper is to present the procedure and results from instrumented flight test performed on the flapping MAVs being developed by the authors. A test is performed using a test measurement system to obtain the real characteristics of the flapping vehicles during their flight. <B>Design/methodology/approach</B> - The test is performed in an indoor flight test facility, equipped with a motion capture system and tracking cameras. Spatial position data are obtained from the vehicles with retro-reflective tracking markers attached. A quantitative analysis is carried out through the investigation and interpretation of the test data for the flight performance assessment of the vehicles. <B>Findings</B> - The finding of the analysis addresses that the test enabled the numerical measurement of vehicles' flying performance and shows the present vehicles have combined characteristics of both birds and insects. <B>Practical implications</B> - The test metrics attempted in the present study are applicable to the test and evaluation of general flapping micro air vehicles. Thus, this testing method will be useful for the development of future micro air vehicle system. <B>Originality/value</B> - Full-scale instrumented flight test and measurement of performance parameters of flapping micro air vehicles other than visual observation are unprecedented and expected to present the guideline of systematic test and evaluation of flapping micro air vehicles.

        만성 신부전 환자의 영양 상태에 관한 연구

        N/A The purpose of the present study was to evaluate the nutritional status of 46 chronic renal failure (CRF) patients. Forty-one chronic renal failure patients and 61 normal control persons were included in the study. There were no differences in mean age and sex distribution between the two groups. We measured serum transferrin with a radioimmunodiffusion method, serum albumin with a bromo cresol Green method and plasma zine with atomic absorption spectrometry. The anthropometric measurements were triceps skinfold thickness (TSF) and midarm circumference (MAC), and from these, midarm muscle circumfrence (MAMC), midarm muscle area (MAMA), and midarm fat area (MAFA) were calculated. The results were as follows: 1) The mean height of the CRF group (164.11±6.50 cm: N=46) was not different from that of the control group (164.01±8.17cm: N=40), but the mean weight of the CRF group (55.27±8.33kg: N = 4ti) was significantly (p=0.33) lower than that of the control group (59.41±9.26kg: N=39). 2) The mean MAMA of the CRF group (40.81±10.11㎠. N =46) was significantly (p=0.005) lower than that of the control group (46.35±9.73㎠. N=61), and the mean MAFA of the CRF group (13.34±7.56㎠: N=46) was significantly (p=0.005) lower than that of the control group (18.03+ 8.97㎠: N =61). 3) The mean serum transferrin of the CRF group (172.23±26.70mg/dl: N=46) was significantly (p=0) lower than that of the control group (307.14±67.23mg/ddl: N =41), the mean plasma zinc of the CRF group (69.56±42.93㎍/dl: N =46) was significantly (p=1.90E-6) lower than that of the control group (123.38±55.49㎍/dl: N = 42), and the mean seurm albumin of the CRF group (3.81±0.52g/dl: N=16) was significantly (p=4.00E-15) lower than that of the control group (4.7±0.52 g/dl: N=41). 4) There was a positive correlation (r=0.78, p=0.00) between MAMA and body weight, a positive correlation (r=0.368, p=0.044) between MAMA and MAFA in CRF patients. 5) There was a positive correlation (r=0.416, p=0.0039) between plasma zinc and serum transferrin, but a negative correlation (r=0.31, p=0.034) between plasma zinc and body weight in CRF patients. 6) There was no correlation between the duration of hemodialysis and body weight, MAMA, serum transferrin, and plasma zinc. We concluded that 1) there were impairments of protein, fat and trace metal metabolism in CRF patients, 2) hemodialysis duration was not a factor influencing the nutritional status of CRF patients, and 3) MAMA, MAFA, serum transferrin and plasma zinc would be good indices to reflect the nutritional status of CRF patients.

        Blood flow in intracranial aneurysms treated with Pipeline embolization devices: computational simulation and verification with Doppler ultrasonography on phantom models

        Purpose: The aim of this study was to validate a computational fluid dynamics (CFD) simulationof flow-diverter treatment through Doppler ultrasonography measurements in patient-specificmodels of intracranial bifurcation and side-wall aneurysms. Methods: Computational and physical models of patient-specific bifurcation and sidewallaneurysms were constructed from computed tomography angiography with use ofstereolithography, a three-dimensional printing technology. Flow dynamics parametersbefore and after flow-diverter treatment were measured with pulse-wave and color Dopplerultrasonography, and then compared with CFD simulations. Results: CFD simulations showed drastic flow reduction after flow-diverter treatment in bothaneurysms. The mean volume flow rate decreased by 90% and 85% for the bifurcation aneurysmand the side-wall aneurysm, respectively. Velocity contour plots from computer simulationsbefore and after flow diversion closely resembled the patterns obtained by color Dopplerultrasonography. Conclusion: The CFD estimation of flow reduction in aneurysms treated with a flow-divertingstent was verified by Doppler ultrasonography in patient-specific phantom models of bifurcationand side-wall aneurysms. The combination of CFD and ultrasonography may constitute a feasibleand reliable technique in studying the treatment of intracranial aneurysms with flow-diverting stents.
