      • Antiproliferative Effect of Metformin on the Endometrium - a Clinical Trial

        Background: Unopposed estrogen has a central role in development of endometrial benign, premalignant and malignant lesions. The aim of this study was to evaluate the anti-estrogenic effect of metformin on endometrial histology in comparison with progesterone. Materials and Methods: A total of 43 patients who were referred to our center for abnormal uterine bleeding and had a histologic diagnosis were disordered proliferative endometrium or simple endometrial hyperplasia were included and randomly distributed in two groups treated with metformin (500mg Bid) or megestrol (40mg daily), respectively, for three months. After this period the patients were evaluated by another endometrial biopsy to assess the impact of the two drugs in restoring normal endometrial histology. Results: Our findings revealed that metformin could induce endometrial atrophy in 21 out of 22 patients (95.5%) while this positive response was achieved in only 13 out of 21 patients (61.9%) in the megstrol group. In addition two low grade endometrial carcinomas in the metformin group responded very well. Conclusions: We conclude that metformin could be used as an effective antiestrogenic agent in control of abnormal endometrial proliferative disorders.

        A Simple Spectrofluorimetric Method for Determination of Mefenamic Acid in Pharmaceutical Preparation and Urine

        A simple, sensitive and rapid spectrofluorimetric method was developed for determination of mefenamic acid in pharmaceutical preparation and human urine. The procedure is based on the oxidation of mefenamic acid with cerium (IV) to produce cerium (III), and its fluorescence was monitored at 354 nm after excitation at 255 nm. The variables affecting oxidation of drug were studied and optimized. Under the experimental conditions used, the calibration graphs were linear over the range 0.03-1.5 mg $L^{-1}$. The limit of detection was 0.009 mg $L^{-1}$ and the relative standard deviation for 5 replicate determinations of mefenamic acid at 1.0 mg $L^{-1}$ concentration level was 1.72%. Good recoveries in the range of 102-107 and 102-109% were obtained for pharmaceutical preparation and human urine, respectively. The proposed method was applied to the determination of MF in one pharmaceutical preparation and human urine. The amounts of mefenamic acid found are very similar to those obtained by a standard method.

        Developing National Functional Accreditation Model for Primary Healthcares with Emphasis on Family Practice in Iran

        Background: Accreditation is an approach toward quality improvement which has been increasingly implemented in healthcare. This study aimed at developing a national functional accreditation model for primary healthcare with emphasis on family practice in Iran. Methods: This mixed-method study utilizes a set of research methods purposefully. Initially, the reference models were used for benchmarking accreditation standards through a systematic review. Then, the primary accreditation standards were developed and then they were assessed and approved by the experts of the field via Delphi technique. In the following and after developing essential parts of the standards, the necessary changes in developed model were done according to the pilot test results. Results: The results of systematic review suggested the superiority of accreditation models of the United States, Australia, Canada, and the United Kingdom globally; and the models of Jordan, Saudi Arabia, Lebanon, and Egypt in Eastern-Mediterranean region. Then, the primary standards including 39 functional standards with 231 measures were developed according to the benchmarked models, and were approved by the experts in Delphi-based study. In pilot test step, the compliance rate of developed standards by primary healthcare centers was calculated 61.61% and 26.37% for self-evaluation and external evaluation phases, respectively. Conclusion: Regarding the comprehensiveness of developed accreditation model due to its focus on all functional dimensions and the consensus over the developed standards by the experts, it can be an underlying ground for the establishment and evaluation of functional improvement programs in Iranian primary healthcare system.

        Determination of Mefenamic Acid in Human Urine by Means of Two Spectroscopic Methods by Using Cloud Point Extraction Methodology as a Tool for Treatment of Samples

        Cloud point extraction was used to extract mefenamic acid (MF) from human urine, and spectrofluorimetry and spectrophotometry were used to analyze extracted MF. The variables affecting extraction and phase separation, i.e. HCl and Triton X-114 concentration, temperature and time of equilibration, were optimized. Under the experimental conditions used the limit of detection for extraction of 25 mL of sample was 0.006 and 0.045 mg $L^{-1}$, with relative standard deviations of 2.52 and 1.45% (n = 5) for spectrofluorimetric or spectrophotometric methods, respectively. Good recoveries in the range of 95-107% were obtained for spiked samples. The proposed methods were applied to the determination of MF in human urine.

      • Role of Electromagnetic Field Exposure in Childhood Acute Lymphoblastic Leukemia and No Impact of Urinary Alpha-Amylase - a Case Control Study in Tehran, Iran

        Childhood acute lymphoblastic leukemia (ALL) is one of the most common hematologic malignancies which accounts for one fourth of all childhood cancer cases. Exposure to environmental factors around the time of conception or pregnancy can increase the risk of ALL in the offspring. This study aimed to evaluate the influence of prenatal and postnatal exposure to high voltage power lines on the incidence of childhood ALL. It also examines the role of various factors such as environmental factors and alpha-amylase as a marker in the development of leukemia.This cross-sectional case control study was carried out on 22 cases and 100 controls who born and lived in low socioeconomic families in Tehran and were hospitalized for therapeutic purposes in different hospitals ofrom 2013-2014. With regard to the underlying risk factors; familial history and parental factors were detected as risk factors of ALL but in this age, socioeonomic and zonal matched case control study, prenatal and childhood exposure to high voltage power lines was considered as the most important environmental risk factor (p=0.006, OR=3.651, CI 95% 1.692-7.878). As the population study was from low socioeconomic state, use of mobiles, computers and microwaves was negligible. Moreover prenatal and postnatal exposure to all indoor electrically charged objects were not detected as significant environmental factors in the present study. This work defined the risk of environmental especially continuous pre and postnatal exposure to high voltage power lines and living in pollutant regions through the parents or children as well as the previously described risk factors of ALL for the first time in low socioeconomic status Iranian population.

      • Health Promoting Behavior and Influencing Factors in Iranian Breast Cancer Survivors

        Background: The purpose of this study was to investigate the associations among the internal health locus of control, depression, perceived health status, self efficacy, social support, and health-promoting behavior in Iranian breast cancer survivors and to determine influential variables. Materials and Methods: A predictive design was adopted. By convenient sampling the data of 262 breast cancer survivors in Iran were collected by questionnaires during 2014. Data were analyzed applying descriptive statistics, t-tests, one-way ANOVA, Pearson's correlation coefficients, and stepwise multiple regression. Results: The internal health locus of control, depression, perceived health status, self efficacy, social support and undergoing chemotherapy all correlated significantly with the health-promoting lifestyle. Stepwise multiple regression analysis revealed that social internal health locus of control, depression, perceived health status, self efficacy and social support and chemotherapy accounted for about 39.8% of the variance in health promoting lifestyle. The strongest influence was social support, followed by self efficacy, perceived health status, chemotherapy and depression. Conclusions: The results of the study clarifed the seriousness of social support, self efficacy, perceived health status and depression in determining the health-promoting lifestyle among Iranian breast cancer survivors. Health professionals should concentrate on these variables in designing plans to promoting a healthy lifestyle.

        Performance of CNT-CNP aerogel as electrode in capacitive deionization system

        In this study, hybrid aerogels containing carbon nanoparticles (CNP) and multi-walled carbon nanotubes (MWCNT) were synthesized via sol–gel method using resorcinol/formaldehyde precursors through a hydrolysis-condensation reaction mechanism. Porous carbonaceous structures were achieved by freeze-drying of the organic gels followed by controlled carbonization under an inert gas. The samples were characterized by various techniques such as FTIR, BJH, FESEM, CV, and EIS. The specific surface area and total pore volume of the aerogel sample were measured to be as high as 452 m2/g and 0.782 cm3/g, respectively, thus enhancing the electric double-layer formation. Electrochemical tests on the samples showed a large specific capacitance (212 F/g) and an excellent cyclic stability over 3000 cycles. Performance of the synthesized structures was subsequently assessed as electrodes in a capacitive deionization (CDI) process. At the operating conditions of 1.6 V voltage, flow rate of 20 mL/min, and NaCl concentration of 1000 mg/L a promising adsorption capacity around 42.08 mg/g was achieved.

        A Cloud Point Extraction-Spectrofluorimetric Method for Determination of Thiamine in Urine

        A simple and efficient cloud point extraction-spectrofluorimetric method for the determination of thiamine in human urine is proposed. The procedure is based on the oxidation of thiamine with ferricyanide to form thiochrome, its extraction to Triton X-114 micelles and spectrofluorimetric determination. The variables affecting oxidation of thiamine, extraction and phase separation were studied and optimized. Under the experimental conditions used, the calibration graphs were linear over the range 2.5-1000 ng $mL^{-1}$. The limit of detection was 0.78 ng $mL^{-1}$ of thiamine and the relative standard deviation for 5 replicate determinations of thiamine at 400 ng $mL^{-1}$ concentration level was 2.42%. Average recoveries between 93-107% were obtained for spiked samples. The proposed method was applied to the determination of thiamine in human urine.
