        Gangrenous cholecystitis in male patients

        Backgrounds/Aims: The prevalence and risk factors of gangrenous cholecystitis in male are unknown. Objective: To verify the prevalence and risk factors of gangrenous cholecystitis in males. Methods: This cross-sectional study includes 95 patients (59.5±17.1 years), with clinical and histopathological diagnosis, operated laparoscopically on 2012-2016. Eligibility was decided based on the variables of age; tachycardia (>100 bpm); leukocytosis (>10,000/mm³); Murphy’s sign; gallbladder wall thickness (>4 mm); biochemical tests, morbidities (diabetes, alcoholism, smoking) and mortality. Multivariate regression, the chi-squared and Prevalent Chances Ratio (PCR) were used to define a few parameters. Results: The prevalence of gangrenous cholecystitis in men older than 50 years was 29.3%. The risk factors for the disease were as follows: diabetes mellitus (p=0.006, RCP=4.191), leukocytosis (p=0.003), gallbladder thickness greater than 4 mm (p=0.035, RCP=3.818), which increased mortality [(p=0.04) (RCP=8.001)]. Murphy’s sign showed a negative association (p=0.002, RCP=0.204). Values close to significance were observed in relation to gamma glutamyl transferase (p=0.083, RCP=3.125) and hospital stay (p=0.061, RCP=2.765). Conclusions: Male gender, and age older than 50 years, were correlated with a high prevalence of necrosis, higher than that reported in females. The risk factors for developing necrosis are the same as those described for female patients.

        Short-term outcomes in patients undergoing laparoscopic surgery for deep infiltrative endometriosis with rectal involvement: a single-center experience of 168 cases

        Purpose: The surgical management of deep infiltrative endometriosis (DE) involving the rectum remains a challenge. The objective of this study was to assess the outcomes from a single tertiary center over a decade with an emphasis on the role of a protective loop ileostomy (PI).Methods: A retrospective review of outcomes for 168 patients managed between 2008 and 2018 is presented including 57 rectal shaves, 23 discoid excisions, and 88 segmental rectal resections.Results: The nodule size (mean±standard deviation) in the segmental resection group was 32.7±11.2 mm, 23.4±10.5 mm for discoid excision, and 18.8±6.0 mm for rectal shaves. A PI was performed in 19 elective cases (11.3%) usually for an ultra-low anastomosis <5 cm from the anal verge. All Clavien-Dindo grade III/IV complications occurred after segmental resections and included 5 anastomotic leaks, 6 rectovaginal fistulas, 2 ureteric fistulas, and 1 ureteric stenosis. Of 26 stomas (15.5%), there were 19 PIs, 3 secondary ileostomies (after complications), and 4 end colostomies. The median time to PI closure was 5.8 months (range, 0.4–16.7 months) in uncomplicated disease compared with 9.2 months (range, 4.7–18.4 months) when initial postoperative complications were recorded (P=0.019). Only 1 patient with a recurrent rectovaginal fistula had a permanent colostomy.Conclusion: In patients with DE and rectal involvement a PI is selectively used for low anastomoses and complex pelvic reconstructions. Protective stomas and those used in the definitive management of a major postoperative complication can usually be reversed.

      • Lessons from Research on Peace Education in Israel/Palestine

        Despite the relative uniqueness of peace education in contexts of intractable conflict, research on peace education carried out in the context of the Israeli-Palestinian conflict can be instructive, to a greater or lesser extent, in other contexts. Six researchbased lessons are presented: (1) The short life of peace education's positive effects and so the need for continuous reinforcement; (2) ways of restoring the eroded effects; (3) the benefits of learning about another conflict; (4) the differential effects of peace education on different groups of participants; (5) the limitations of peace education; and (6) joint projects with common important goals as an alternative approach to peace education. In addition, two common challenges facing peace education and other kinds of programs are presented: the challenge of maintaining the attained positive effects, and the challenge of spreading the effects to wider realms of society.

        Structure and Foaming Properties of Viscous Exopolysaccharides from a Wild Grape-Associated Basidiomycetous Yeast Papiliotrema flavescens Formerly Known as Cryptococcus flavescens

        Exopolysaccharide produced by the yeast Papiliotrema flavescens, isolated from wine grape berries of Champagne vineyard, was investigated for both chemical and functional characterization. SECMALLS and colorimetric assay analyses showed that the EPS is a high MW heteropolymer (2.37 × 106 g/mol) majorily consisting of mannose, glucose, xylose and glucuronic acid as monosaccharide constituents, with two substituents (sulphate and phosphate groups), and a minor protein moiety. Structural enchainment of these carbohydrates based on methylation, GC-MS and NMR analyses revealed a linear main backbone built up of α-(1 →3)-D-mannopyranosyl residues on which are branched side chains consisting of a single β-D-glucopyranosyluronic acid residue and β-(1 →2)- xylopyranoses (2-5 residues). Suggestion of some xylopyranose side chains containing a mannose residue at the nonreducing terminal end was also proposed. This is first report on EPSs from the grape P. flavescens yeast with such structural characteristics. Furthermore, investigations for valuating the application performance of these EPS in relation with their structural features were carried out in 8% alcohol experiment solutions. Very exceptional viscosifying and foaming properties were reported by comparison with commercial biopolymers such as Arabic, gellan and xanthan gums. The intrinsic properties of the natural biopolymer from this wild grape-associated P. flavescens yeast make it a potential candidate for use in various biotechnology applications.

        This first comparative study of the principles of ordering of graphs in alphabetic and syllabic writing systems shows that the sequences are most commonly determined by phonological principles, graphic resemblances, mnemonic methods, or various combinations thereof. But in many systems the underlying rationale, if any, of the ordering remains obscure, most notably in the Semitic/European alphabets and their many descendants.

      • Khipu and Alphabetic Script:

        This paper concerns the precolumbian medium khipu, which is constructed from colored and/or knotted cords. Khipu flourished as the bureaucratic registry of the Inka Empire (c. 1450 CE–1532 CE). Although the Spanish viceroyalty enforced the alphabetic authority of scribes, it also recognized khipu as a complementary registry proper to Indians. Despite the development of a protocol for court khipu use, khipu remained opaque to Spaniards, and the medium acquired an aura of ethnic privacy. Some native Quechua- and Aymara-speaking communities retained khipu records of internal religious and administrative matters into the 20th century. Such ethnographic cases may shed light on the still-controversial basic makeup of the cord medium, whose standing as “true writing” or otherwise remains in debate.

      • Fusion imaging using US and MRI: preliminary experience from the LUMIERE research platform

        Objective US and MRI are two validated techniques for fetal brain expertise, each presenting their own advantages. Fusion imaging (FI) might result in one final merged image with the exactness of both modalities. The objective was to evaluate the clinical use of the off-line US-MRI FI in the assessment of the fetal brain development. Material and Methods This was a prospective study in Necker Hospital (NCT04142606) conducted from January to May 2021. We included normal singleton pregnancies between 20 and 36 weeks. Each woman has had a volumic 3D US and a T2w MRI brain exam (1.5 T) on the same day. Then, we merged US and MRI images with the dedicated 4D View Fusion software (GE Healthcare) by using precise landmarks to optimize the registration process. We developed a standardized method to identify 29 brain structures previously defined to provide an acute expertise the fetal brain development in FI. Results More than 100 patients were included and we obtained workable US and MRI acquisitions in the orthogonal planes allowing for fusion imaging in the vast majority of cases. Overall, FI gave access to a larger amount of information to establish a complete fetal neurosonogram than each modality by itself. At late gestational age, the examination of the parenchyma became very difficult due to fetal position, shadow cones and skull ossification. US resolution highlighted some median brain structures such as basal ganglia, trigone, Sylvius aqueduct or cerebellar foliation; On the opposite on the same plane, MRI gave ac- cess to the full brain gyration, brain stem or peri-cerebellar spaces. Moreover, a 3D Doppler acquisition merged to an MRI could provide some vascular information, so far unattainable with fetal MRI. Conclusion We present a promising off-line US-MRI FI tool allowing to navigate within two merged volumes acquired during the same day. This tool could be of major interest for educational purpose, brain development assessment and in case of fetuses at high risk for brain damages such as CMV infection or TTTS.
