        Long term recurrence free survival in a stage IV gallbladder cancer treated with chemotherapy plus trastuzumab and salvage liver resection

        Surgery is the only treatment for biliary tract cancer with long term survival. Unfortunately, most patients are diagnosed at stage IV with distant metastases. In these circumstances, life expectancy is less than one year due to aggressive tumour biology and a lack of effective systemic therapies. HER2 overexpression or amplification is predominantly seen in extrahepatic cholangiocarcinoma and gallbladder cancer (10-18%) and rarely in intrahepatic cholangiocarcinoma (1%). Trastuzumab is a monoclonal antibody that targets HER-2. We present a clinical case with a stage IV gallbladder cancer (liver and interaortocaval lymph node metastases), which presented progression during first-line chemotherapy treatment, which prompted a change in therapy to study the Her 2/Neu mutation which showed an intense positive overexpression. A combination of HER2/Neu-directed therapy (Trastuzumab) with second-line chemotherapy, was able to achieve a long term complete radiological, metabolic, and biochemical response. A curative intention surgery was performed and the patient is alive and recurrence-free at five years. To the best of our knowledge, we present a case which is the first report of a patient with a Stage IV gallbladder cancer who achieved a five-year survival without recurrence after a conversion therapy combining chemotherapy plus Trastuzumab and radical salvage surgery.

        A case of COVID-19 immediately after liver transplantation: Not only bad news

        COVID-19, the illness caused by the SARS-CoV-2 virus originated in December 2019 in Wuhan, China and has caused more 3,3 million cases and more than 230,000 deaths throughout the world, with 25,000 of them only in Spain, where the first case was diagnosed on January 31st, 2020. As COVID-19 is a “new” disease, we still do not have data on prognosis or treatment in transplant patients or on how to manage immunosuppression in this complex scenario. We present a case of COVID-19 diagnosed during the early postoperative period in a recipient whose liver transplantation was performed on late March during the lockdown in Spain, with donor and recipient previously negative rRT-PCR to SARS-CoV-2. In the first post-operative week the patient suffered COVID-19 pneumonia that was treated with immunosuppression minimization, oral Hydroxycloroquine and Azithromycin with favorable outcome. The patient was discharged on POD 21 without complications. To date, few early post-liver transplantation SARS-CoV-2 infected recipients have been published, but only one was an early postoperative infection. In our case the outcome was favorable, even though it was an early post -liver transplantation COVID-19 in a frail patient.

        A new method to measure the accuracy of intraoral scanners along the complete dental arch: A pilot study

        PURPOSE. The purpose of this study is to assess the accuracy of three intraoral scanners along the complete dental arch and evaluate the feasibility of the assessment methodology for further in vivo analysis. MATERIALS AND METHODS. A specific measurement pattern was fabricated and measured using a coordinate measuring machine for the assessment of control distances and angles. Afterwards, the pattern was placed and fixed in replica of an upper jaw for their subsequent scans (10 times) using 3 intraoral scanners, namely iTero Element1, Trios 3, and True Definition. 4 reference distances and 5 angles were measured and compared with the controls. Trueness and precision were assessed for each IOS: trueness, as the deviation of the measures from the control ones, while precision, as the dispersion of measurements in each reference parameter. These measurements were carried out using software for analyzing 3-dimensional data. Data analysis software was used for statistical and measurements analysis (α=.05). RESULTS. Significant differences (P<.05) were found depending on the intraoral scanner used. Best trueness values were achieved with iTero Element1 (mean from 10 ± 7 µm to 91 ± 63 µm)while the worst values were obtained with Trios3 (mean from 42 ± 23 µm to 174 ± 77 µm). Trueness analysis in angle measurements, as well as precision analysis, did not show conclusive results. CONCLUSION. iTero Element1 was more accurate than the current versions of Trios3 and True Definition. Importantly, the proposed methodology is considered reliable for analyzing accuracy in any dental arch length and valid for assessing both trueness and precision in an in vivo study.

        Investigation of thermal-related effects in Hot SPIF of Ti-6Al-4V alloy

        The present work focuses on a new approach to hot form hard-to-work materials by Single Point Incremental Forming (SPIF) technology using a global heating of the sheet. A set of trials was carried out in order to identify the optimum temperature cycles to minimize geometric distortions associated to each process stage on the fabrication of parts made of Ti–6Al–4V. On the one hand, heating trials allowed defining the optimal procedure to improve the temperature distribution homogeneity along the sheet and consequently to minimize its thermal distortion previous to the forming stage. On the other hand, the influence of both working temperature and the applied cooling on the geometric accuracy was evaluated by means of SPIF trials. For this purpose, a generic asymmetric design with typical aeronautical features was used. These trials pointed out that high forming temperatures allow reducing significantly the material springback whereas a controlled cooling (with an intermediate stress relief treatment) minimizes both the distortion of the part during the cooling and the mechanical stresses accumulated on the clamping system. Furthermore, the work includes a post-forming material evaluation to determinate the influence of the employed processing conditions on microcracks, alpha-case layer, microstructure and hardness.

        Fine particle flow pattern and region delimitation in fountain confined conical spouted beds

        A novel borescopic technique together with the monitoring of pressurefluctuation signals (powerspectral distribution, PSD) has been used to trackfine particles and characterize solidflow dynamics infountain confined conical spouted beds. Radial and axial particle velocity profiles have been obtained fordifferent configurations, and spout-annulus and fountain core-periphery interfaces have beendelineated. The downward particle velocities in the annulus peak at intermediate positions in thiszone, whereas the upward velocities in the dilute zones (spout and fountain core) peak at the axis or closeto this position. Among the different configurations analysed in this work, the system without draft tubeshows the greatest vertical particle velocities in almost all the different radial and axial positions. Theevolution of the spout size along the bed depends on the configuration used, but all of them differ fromthose commonly reported in the literature. Thus, the spout expands from the bed bottom to the surface,without any neck at an intermediate bed level. Furthermore, the cross-sectional spout shape has beendelineated in the systems with open-sided draft tube, and significant spout expansion is observed due toair percolation from the spout into the annulus through the opened faces. Finally, the average spoutdiameters of the systems without draft tube and with open-sided draft tubes have been compared withthose predicted by literature correlations. Those proposed by San José et al. and Volpicelli et al. providethe bestfit for the configurations without draft tube and with open-sided draft tube, with their relativeerrors being 9.83% and 8.88%, respectively.

        Patterns of Personality according to Age and Gender in Basque Speakers

        The most important changes in personality dimensions seem to take place under the age of 30. Nevertheless, the influence of work status has not been systematically assessed in the examination of when these big changes happen. Besides, gender differences in some personality dimensions are still not conclusive. The aim of this study was to examine the age differences in personality dimensions when work status (students vs. workers) for young adults is taken into account, as well as to examine possible gender differences in the Basque speakers' population. Participants were 1,441 Basque speakers (students and civil servants) whose ages ranged from 16 to 65 years. Results showed that younger students had higher scores in Extraversion, regardless of the work status. However, the work status variable was more important than the age effect to explain changes in Neuroticism and Conscientiousness. Besides, women showed higher scores for Neuroticism and Conscientiousness, although there were not gender differences for other dimensions.

        Use of measuring gauges for in vivo accuracy analysis of intraoral scanners: a pilot study

        PURPOSE. The purpose of this study is to present a methodology to evaluate the accuracy of intraoral scanners (IOS) used in vivo. MATERIALS AND METHODS. A specific feature-based gauge was designed, manufactured, and measured in a coordinate measuring machine (CMM), obtaining reference distances and angles. Then, 10 scans were taken by an IOS with the gauge in the patient's mouth and from the obtained stereolithography (STL) files, a total of 40 distances and 150 angles were measured and compared with the gauge's reference values. In order to provide a comparison, there were defined distance and angle groups in accordance with the increasing scanning area: from a short span area to a complete-arch scanning extension. Data was analyzed using software for statistical analysis. RESULTS. Deviations in measured distances showed that accuracy worsened as the scanning area increased: trueness varied from 0.018 ± 0.021 mm in a distance equivalent to the space spanning a four-unit bridge to 0.106 ± 0.08 mm in a space equivalent to a complete arch. Precision ranged from 0.015 ± 0.03 mm to 0.077 ± 0.073 mm in the same two areas. When analyzing angles, deviations did not show such a worsening pattern. In addition, deviations in angle measurement values were low and there were no calculated significant differences among angle groups. CONCLUSION. Currently, there is no standardized procedure to assess the accuracy of IOS in vivo, and the results show that the proposed methodology can contribute to this purpose. The deviations measured in the study show a worsening accuracy when increasing the length of the scanning area.
