      • Design of a metasurface-based dual-band Terahertz perfect absorber with very high Q -factors for sensing applications

        <P><B>Abstract</B></P> <P>We report on a novel very high Q -factor dual-band Terahertz perfect absorber composed of a metasurface located on top of a flexible polyimide spacer deposited on a silver ground layer. The metasurface is a 2D-array of plasmonic nanoantennas with the shape of two concentric square rings and a cylinder positioned at their centre. By performing numerical simulations, we studied the polarisation insensitive electromagnetic response of the absorber for incident angles varying from 0° up to ± 30°. The two resonant modes centred at <SUB> f 1 </SUB> = 1 .80 THz and <SUB> f 2 </SUB> = 2 .26 THz have Q -factors Q ( <SUB> f 1 </SUB> ) = 120 and Q ( <SUB> f 2 </SUB> ) = 94 and absorption coefficients A ( <SUB> f 1 </SUB> ) = 99 . 8 % and A ( <SUB> f 2 </SUB> ) = 99 . 6 %. Moreover, we investigated how the resonant mode frequencies change with the refractive index and thickness of transparent analytes adsorbed on the metasurface. In terms of the Refractive Index Units (RIU), we obtained sensitivities equal to 187.5 GHz/RIU and 360 GHz/RIU for the <SUB> f 1 </SUB> and <SUB> f 2 </SUB> resonance frequencies, respectively, and figure of merits up to FOM = 19 . 1 and <SUP> FOM ∗ </SUP> = 431 . These results make the dual-band absorber to be employed as a sensing device able to detect the presence and/or the physical/chemical modifications of the adsorbed analytes. Moreover, we investigated the dependence of the sensitivity as a function of slight modifications of the metasurface nanoantenna shape, demonstrating that a more homogeneous distribution of the electric field intensity on the metasurface improves the sensitivity of the absorber without affecting the Q -factors.</P>

        Intraoperative near-infrared spectroscopy for pedicled perforator flaps: a possible tool for the early detection of vascular issues

        Background Pedicled perforator flaps can present postoperative complications similar to those encountered in free flap surgery. Beyond a clinical evaluation, there is still no reliable technical aid for the early prediction of vascular issues. The aim of this study was to assess the support of near-infrared spectroscopy technology as an intraoperative tool to anticipate postsurgical flap ischemia. Methods We prospectively enrolled 13 consecutive patients who were referred to our hospital from March 2017 to July 2018 and required a reconstructive procedure with a pedicled fasciocutaneous perforator flap. We measured flap peripheral capillary oxygen saturation (SpO<sub>2</sub>) in each patient with a Somanetics INVOS 5100C Cerebral/Somatic Oximeter (Medtronic), both before and after transposition. Patient demographics, operative data, and complications were then recorded during the following 6 months. We analyzed the data using the Wilcoxon signed-rank test and linear regression. Results The mean flap SpO<sub>2</sub> before and after transposition was 92%±3% and 78%±19%, respectively. The mean change in SpO<sub>2</sub> was 14%±17%, with a range of 0% to 55%. The change in saturation and mean saturation ratio were significantly different between patients with and without postoperative flap necrosis. Conclusions An immediate quantitative analysis of flap peripheral capillary SpO<sub>2</sub> after transposition has never before been described. In our experience, an intraoperative drop in SpO<sub>2</sub> equal to or greater than 15%-20% predicted vascular complications in pedicled perforator flaps. Conversely, flap size and rotation angle were not correlated with the risk of flap necrosis.

        Total mandible and bilateral TMJ reconstruction combining a customized jaw implant with a free fibular flap: a case report and literature review

        Background The need for whole mandibular bone reconstruction and bilateral joint replacement is fortunately rare, but it is an extremely challenging topic in maxillofacial surgery, due to its functional implications. CAD-CAM techniques development has opened new broad horizons in the surgical planning of complex maxillofacial reconstructions, in terms of accuracy, predictability, and functional cosmetic results. The review of the literature has revealed a small number of scientific reports on total mandibulectomy including the condyles, with only eleven cases from 1980. Most of the works describe reconstructions secondary to dysplastic or inflammatory diseases affecting the lower jaw. The aim of this work, reporting a rare case of massive fibrous dysplasia of the whole mandible, is to share our experience in the management of extended mandibular and bilateral joint reconstruction, using porous titanium patientspecific implants. Case presentation The authors present a 20-year-old male patient suffering from massive bone fibrous dysplasia of the mandible. The mandibular body and both the rami and the condylar processes had been involved, causing severe functional impairment, tooth loss, and facial deformation. The young patient, after repeated ineffective conservative surgical treatments, has required a biarticular mandibular replacement. Using virtual surgical planning (VSP) software, the authors, in collaboration with medical engineers, have created a custom-made original titanium porous mandibular implant, suspended from a bilateral artificial temporomandibular joint. The mandibular titanium implant body has been specifically designed to support soft tissues and to fix, in the alveolar region, a free fibular bone graft, for delayed dental implant prosthetic rehabilitation. Conclusion The surgical and technical details, as well as the new trends in mandibular reconstructions using porous titanium implants, are reported, and discussed, reviewing literature reports on this topic. Satisfactory functional and cosmetic restorative results have been obtained, and no major complications have occurred. The patient, currently in the 18th month clinical and radiological follow-up, has recently completed the functional restoration program by an implant-supported full-arch dental prosthesis.
