      • The Role of Home-Based Care Managers in Japan

        Surveys were conducted to study the role and function of care managers (CMs) in Japan` s long-term care insurance system by examining work hours and subjective evaluations. Between October and December 2011, we used a self-report time study method with 745 CMs in 308 home-based care management agencies and received responses from 373 people in 153 businesses (50.1% response rate). By performing cluster analysis based on the values of necessity, burden, and achievement in the subjective evaluations of the CMs` duties, we were able to classify 16 duties into two groups; cluster 1 included conclusion of contracts, in-service/seminars, staff supervision, patients` rights advocacy, complaint resolution, direct/nursing care activities, quality improvement activities and facility administration-related activities, and cluster 2 included pre-admission consultations, assessment, care planning activities, team management, consultation, cooperation with the community, indirect care activities, and other. Cluster 2 had a greater rate of occurrence than cluster 1, and more time was spent on these duties. In addition, necessity was higher in cluster 2 than in cluster 1, although burden and achievement were lower. The duties in cluster 2 are consistent with the role of CMs required under the long-term care insurance system. Eleven years of practice have passed since the birth of care management along with the long-term care insurance system; CMs are recognizing the necessity of these duties as realities confronting them become apparent, and the CMs` role has been clarified. The duty with the highest necessity and achievement was patients` rights advocacy. Complaint resolution had the highest burden. In terms of the CMs` role, the possibility of further proactive involvement in patients` rights advocacy and improving the ability to cope with complaint resolution could lead to a reduced burden on CMs.

        Genetic aberrations on the short arm of chromosome 8 (8p) in tongue carcinomas

        Aberrations on the short arm of chromosome 8 (8p) are frequently observed in several human cancers. In this study, 20 squamous cell carcinoma (SCC) specimens from the tongue were examined in order to evaluate the role of 8p in SCC of the tongue. Microsatellite analysis using 14 markers demonstrated two commonly deleted regions (CDRs) on 8p. Reverse transcription-polymerase chain reaction (RT-PCR) revealed frequent downregulation of the FEZ1 gene, mapped to 8p22, and frequent over-expression of the cathepsin B gene, mapped to 8p-21-22. These results suggested that genetic aberrations are involved in the development of SCC of the tongue. However, no significant relationship was observed to be established between the genetic alterations and clinicopathological features. Thus, further investigation is necessary in order to clarify the clinical role of 8p in carcinoma of the tongue.

        Rating criteria to evaluate student performance in digital wax-up training using multi-purpose software

        PURPOSE. The aim of this study was to introduce rating criteria to evaluate student performance in a newly developed, digital wax-up preclinical program for computer-aided design (CAD) of full-coverage crowns and preliminarily investigate the reliability and internal consistency of the rating system. MATERIALS AND METHODS. This study, conducted in 2017, enrolled 47 fifth-year dental students of Okayama University Dental School. Digital wax-up training included a fundamental practice using computer graphics (CG), multipurpose CAD software programs, and an advanced practice to execute a digital waxup of the right mandibular second molar (#47). Each student’s digital wax-up work (stereolithography data) was evaluated by two instructors using seven qualitative criteria. The total qualitative score (0-90) of the criteria was calculated. The total volumetric discrepancy between each student’s digital wax-up work and a reference prepared by an instructor was automatically measured by the CAD software. The inter-rater reliability of each criterion was analyzed using a weighted kappa index. The relationship between the total volume discrepancy and the total qualitative score was analyzed using Spearman’s correlation. RESULTS. The weighted kappa values for the seven qualitative criteria ranged from 0.62 - 0.93. The total qualitative score and the total volumetric discrepancy were negatively correlated (ρ = -0.27, P = .09, respectively); however, this was not statistically significant. CONCLUSION. The established qualitative criteria to evaluate students’ work showed sufficiently high inter-rater reliability; however, the digitally measured volumetric discrepancy could not sufficiently predict the total qualitative score.
