      • A new approach for measurement of anisotropic tensile strength of concrete

        In this paper, a compression to tensile load converter device was developed to determine the anisotropic tensile strength of concrete. The samples were made from a mixture of water, fine sand and cement, respectively. Concrete samples with a hole at its center was prepared and subjected to tensile loading using the compression to tensile load converter device. A hydraulic load cell applied compressive loading to converter device with a constant pressure of 0.02 MPa per second. Compressive loading was converted to tensile stress on the sample because of the overall test design. The samples have three different configurations related to loading axis; 0, $45^{\circ}$, $-45^{\circ}$. A series of finite element analysis were done to analyze the effect of hole diameter on stress concentration of the hole side along its horizontal axis to provide a suitable criterion for determining the real tensile strength of concrete. Concurrent with indirect tensile test, Brazilian test and three point loading test were also performed to compare the results from the three methods. Results obtained by this device were quite encouraging and show that the tensile strengths of concrete were similar in different directions because of the homogeneity of bonding between the concrete materials. Also, the indirect tensile strength was clearly lower than the Brazilian test strength and three point loading test.

        Measuring the Managerial Efficiency of Insurance Companies in Saudi Arabia: A Data Envelopment Analysis Approach

        This paper applies the Data Envelopment Analysis (DEA) to compute the managerial efficiency of 30 insurance companies listed on the Saudi stock exchange for the duration of four years from 2015 to 2018. The companies taken as a sample of study included both conventional and Takaful insurance companies. The insurance sector of KSA is one of the largest sectors in the country, contributing a substantial percentage in the non-oil economy. Efficiency measurement and evaluation will provide a venue to introspect and benchmark frontiers to the sector. In the present study, we have utilized the basic Banker Charnes Cooper and Charnes Copper Rhodes models of DEA. Two inputs, namely, general & administrative expenses and policy & acquisition costs, and two outputs (Net premium earned and Investment Income & other incomes) were taken for efficiency calculations. The final outcomes of the study reveal that a good number of insurance companies operating in KSA are found to be efficient on managerial efficiency scale. Three firms remain the leader on the frontier of the managerial efficiency. And no company found with zero (0) efficiency or a negative efficiency. It is expected that the outcome of the study will provide benchmarks to managers and a road map to further improvement.

      • Breast Lesions in Adolescents and Young Women in Pakistan - a 5 Year Study of Significance of Early Recognition

        Background: Breast cancer is the most frequent malignant disease amongst young women. If we review local data in Pakistan then breast cancer represents approximately a third of all cancers in females. The age standardized incidence rate (ASR) world per 100,000 is 53.8 and crude incidence rate is 30.9. We have observed during our surgical pathology practice and it is also reported by other Asian studies that breast carcinoma is amongst the leading malignancies in the region and the patients are at least a decade younger than counterparts in developed nations. Age is an important issue in effective screening, diagnosis and management of breast cancer, especially in this geographical region where late presentation and poor prognosis are a hallmark of the disease. Objective: The aim of this study is to determine the frequency of malignant breast lesions in symptomatic young females presenting with breast lumps. Materials and Methods: This is a retrospective study conducted at the Pathology Department, Liaquat National Hospital and Medical College, Karachi. Descriptive and pathology data of malignant breast tumors 1st January 2004 to 31st December 2009 were reviewed, using the departmental archived data. It included both male and female patients up to the age of 25 years. Results: A total of 714 surgical specimens from/of symptomatic breast lesions were received at the pathology department of Liaquat National Medical, in the five years study period, in young females. There were 575 (80%) benign, 119 (16%) inflammatory and 20 (2.8%) malignant lumps. Conclusions: The obtained data for females only up to 25 years of age suggest a massive burden which requires urgent attention. Early assessment of lesions is essential in order to avoid mortality from malignancies.

      • Accuracy of Frozen Section Analysis of Sentinel Lymph Nodes for the Detection of Asian Breast Cancer Micrometastasis - Experience from Pakistan

        Background: Intraoperative sentinel lymph node biopsy has now become the standard of care for patients with clinically node negative breast cancer for diagnosis and also in order to determine the need for immediate axillary clearance. Several large scale studies confirmed the diagnostic reliability of this method. However, micrometastases are frequently missed on frozen sections. Recent studies showed that both disease free interval and overall survival are significantly affected by the presence of micrometastatic disease. The aim of this study was to determine the sensitivity and specificity of intraoperative frozen section analysis of sentinel lymph nodes (SLNs) for the detection of breast cancer micrometastasis and to evaluate the status of non-sentinel lymph nodes (non-SLNs) in those patients subjected to further axillary sampling. Materials and Methods: We performed a retrospective study on 154 patients who underwent SLN biopsy from January 2008 till October 2011. The SLNs were sectioned at 2 mm intervals and submitted entirely for frozen sections. Three levels of each section submitted are examined and the results were compared with further levels on paraffin sections. Results: Overall 40% of patients (62/154) were found to be SLN positive on final (paraffin section) histology, out of which 44 demonstrated macrometastases (>2mm) and 18 micrometastases (<2mm). The overall sensitivity and specificity of frozen section analysis of SLN for the detection of macrometastasis was found to be 100% while those for micrometastasis were 33.3% and 100%, respectively. Moreover 20% of patients who had micrometastases in SLN had positive non-SLNs on final histology. Conclusions: Frozen section analysis of SLNs lacks sufficient accuracy to rule out micrometastasis by current protocols. Therefore these need to be revised in order to pick up micrometastasis which appears to have clinical significance. We suggest that this can be achieved by examining more step sections of blocks.

        First-principles investigation of structural, elastic, electronic and magnetic properties of Be0.75Co0.25Y (Y=S, Se and Te) compounds

        We have theoretically investigated the structural, elastic, electronic and magnetic properties of Be0.75Co0.25Y (Y=S, Se and Te) alloys, in their zinc-blend phase. This study is carried out by using the fullpotential augmented plane wave plus local orbitals method within the density functional theory. Foe computing the exchange-correlation potential, the Wu and Cohen generalized gradient approximation is employed to calculate structural and elastic properties whereas the modified Becke and Johnson potential local density approximation is utilized to examine electronic and magnetic properties. By minimizing the total energy in paramagnetic (PM) and ferromagnetic (FM) phases, it is found the studied compounds are stable in FM structure. The mechanical behavior of the studied compounds is reported with the calculation of shear modulus, Young's modulus, and Poisson's ratio provides. Such mechanical aspects might be useful for the experimentalists to study the mechanical properties upon alloying BeY compounds with Co. We also compute electronic structures, density of states (total and partial), pdexchange splitting and magnetic moments. Moreover, bond nature is studied by estimating the spin polarized charge densities of Be0.75Co0.25Y (Y=S, Se and Te).

      • Distribution of Ki67 Proliferative Indices among WHO Subtypes of Non-Hodgkin's Lymphoma: Association with other Clinical Parameters

        Background: Non-hodgkin lymphoma (NHL) is a diverse group of disease encompassing divergent tumor types with contrasting clinical behaviors. We aimed to evaluate the usefulness of Ki67 index in segregating indolent from aggressive NHL and its association with clinical parameters. Materials and Methods: During a study period of 4.5 years, a total of 215 cases of lymphomas were diagnosed among of which 172 cases were NHL. Ki67 immunohistochemical staining was performed by the DAKO envision method. Average proportion of tumor cells stained was calculated to determine the proliferative index. Results: The mean age at diagnosis was 46.2 years +19.8 (3-81) with a male to female ratio of 1.5:1. Mean Ki67 index for indolent NHL included 23% for small cell, 25% for mantle cell, 28.5% for marginal zone and 34.6% for follicular lymphoma. On the other hand, mean Ki67 index for aggressive lymphomas were 66.4%, 66.9%, 80.3%, 83.3% and 94.4% for diffuse large B cell, T cell (NOS), anaplastic large cell, lymphoblastic and burkitts lymphoma respectively. No significant correlation was found between Ki67 index and other clinical parameters like age and extra nodal involvement. Conclusions: Ki67 index is a valuable IHC marker to distinguish indolent from aggressive lymphomas especially in small needle biopsies where exact typing may not be possible.

        The study of electronic, elastic, magnetic and optical response of Zn1-xTixY (Y=S, Se) through mBJ potential

        To explore the structural, electronic, magnetic and optical properties of Zn1-xTixY (Y=S, Se) alloys, in the composition range 0 ≤ x ≤ 1, we have applied full-potential linearized augmented plane wave plus local orbital (FP-LAPW + lo) scheme, which is based on the density functional theory (DFT). Structural optimizations have been done in ferromagnetic (FM) and antiferromagnetic (AFM) phase by using Wu eCohen generalized gradient approximation (WC-GGA), whereas recently developed modified Becke and Johnson (mBJ) potential was employed to study electronic and optical properties. The lower value of ground state energy and negative enthalpy of formation confirm stability in the FM phase. Due to the spin polarization of electrons in the Ti-d orbital, origin of half-metallic ferromagnetism has been expressed by the calculated band structures, electronic density of states (DOS) and magnetic moments. We also have calculated the exchange splitting energy Dx (d), crystal field energy (DEcrystal = Et2g Eeg) and exchange constants (N0a and N0b). The negative value of exchange constant (N0b) and large splitting of 3d-states of Ti show that the down spin potential is more effective than up spin. Finally, the results of optical parameters such as complex dielectric constant ε (u), refractive index n (u), normal incident reflectivity R (u), absorption coefficient ɑ (u), optical conductivity s (u) and optical loss factor L (u) are discussed in the energy range 0e14 eV. Moreover, we have verified the Penn's model by showing the inverse relation between the static dielectric constant and the optical band gap. The direct relation between static dielectric constant and static refractive index has been observed by increasing the composition of Ti. The calculated parameters provide valuable theoretical information for optical and spintronics device applications.
