Breast tissue expanders (TEs) with magnetic infusion ports are labeled "MR Unsafe." Therefore, patients with these implants are typically prevented from undergoing magnetic resonance imaging (MRI). We report a patient with a total submuscular breast TE who inadvertently underwent an MRI exam. She subsequently developed expander exposure, requiring explantation and autologous reconstruction. The safety profile of TEs with magnetic ports and the use of MRI in patients with these implants is surprisingly controversial. Therefore, we present our case report, a systematic literature review, and propose procedural guidelines to help ensure the safety of patients with TEs with magnetic ports that need to undergo MRI exams.

Hemifacial microsomia (HFM) is a complex congenital condition with heterogeneous malformations of the facial skeleton that almost always involves mandibular hypoplasia. Here we introduce a unique case in which a patient with HFM had initially successful optimization of facial symmetry using a polyetheretherketone implant for mandibular augmentation. However, multiple factors associated with the intraoperative and postoperative course, including hardware failure and infection, led to diminished mechanical strength of the mandible, ultimately resulting in a mandibular fracture. In this unique case presentation of HFM, we discuss the various factors that contributed to mandibular weakness and increased susceptibility to fracture.

Total and subtotal sternectomy oncological defects can result in large deficits in the chest wall, disrupting the biomechanics of respiration. Reviewing the current literature involving respiratory function and rib motion after sternectomy, autologous rigid reconstruction was determined to provide the optimal reconstructive option. We describe a novel technique for sternal defect reconstruction utilizing a double-barrel, longitudinally oriented, vascularized free fibula flap associated with rib titanium plates fixation. Our reconstructive approach was able to deliver a physiological reconstruction, providing rigid support and protection while allowing articulation with adjacent ribs and preservation of chest wall mechanics.

The recent launch of the Geostationary Environment Monitoring Spectrometer (GEMS) provides an important catalyst for increased dialogue and cooperation among Asian countries to address air quality. Other emerging efforts to support GEMS validation and international cooperation include the Pandora Asia Network (PAN) and the Pan Asia Partnership for Geospatial Air Pollution Information (PAPGAPI). These efforts represent long-term commitments to bridging satellite observations with ground-based monitoring to inform air quality. Aircraft observations can provide invaluable context to the satellite and ground-based perspectives that are used more routinely to inform air quality models used for both forecasting and attribution. Important information from aircraft includes measuring detailed composition for source fingerprinting, vertical profiling of composition for satellite validation and model assessment, observing chemical and dynamical processes affecting secondary pollution (i.e., fine particles and ozone), relating specific VOC mixtures to satellite HCHO, providing fine scale pollution mapping with remote sensors, etc. Such information is critical for understanding the local factors influencing air quality for a specific location, quantifying emission sources, and assessing potential mitigation strategies for decision makers. ASIA-AQ proposes to provide airborne observations over three to five Asian megacities with repetitive observations that will observe the diurnal and vertical distribution of primary emissions and secondary pollutants with at least four flights over each location. In combination with satellite and ground observations, data would support analyses for assessment of emissions, model evaluation, process-level understanding of secondary pollutants (i.e., fine particles and ozone), and satellite validation and interpretation. Current status of the ASIA-AQ white paper, nominal plans, and opportunities for involvement will be presented.
