Purpose:The aim of this study is to analyze the outcome of a variety of treatments, including local surgical treatments, diverting stoma, and combined medical therapy, for patients with a rectovaginal fistula complicating Crohn's disease. Methods: Between 1994 and 2003, twenty-one patients with a rectovaginal fistula complicating Crohn's disease from a prospectively compiled 422-patient Crohn's disease database were reviewed. Results: All three patients treated by seton and fibrin glue recurred despite having relatively long tracts. Of six patients with infliximab treatment combined with a seton procedure, five patients had an improvement of their symptoms, but were not cured. Of eight patients with a transanal or endovaginal advancement flap techniques, three had successful closure, three eventually required a proctectomy, and two had a recurrent fistula without symptoms. Four (2 without any local treatments, and 2 with seton placement) of 16 patients who had a diverting stoma during treatment had successful closure. All proctectomy patients (n=8) had rectal involvement of Crohn's disease. Two patients who underwent a proctectomy with a presumptive diagnosis of ulcerative colitis and indeterminate colitis turned out to have Crohn's disease. Overall, except for the proctectomy patients, seven patients (54%) had successful closure, but six (four without symptoms, and two with symptoms) following a wide spectrum of treatments had recurrence after a mean follow-up of 44 months. Conclusions: Combining different treatments for a rectovaginal fistula in Crohn's disease can be successful in a reasonable number of cases. The presence of uncontrolled perianal sepsis and/or complicated anorectal problems is likely to lead to a proctectomy.

Malignant ameloblastoma is characterized by aggressive behavior and a poor prognosis. No histomorphologic criteria exist to differentiate malignant ameloblastoma from their benign, locally-aggressive counterparts. However, biological markers such as histone (H3) expression have proven to be an important indicator of bioaggressive potential and may be a useful predictor of disseminated ameloblastoma. We hypothesize that malignant ameloblastoma, while histologically identical to their benign counterparts, will have a demonstrably higher proliferation rate by histone (H3) mRNA labeling. Seven cases of formalin-fixed, paraffin-embedded tissues from surgical biopsies of mandibular follicular ameloblastomas were obtained. One of the seven specimens was obtained from an ameloblastoma which metastasized to the right and left lungs. Proliferation rates were determined using RNA in situ hybridization with ^35S-labeled histone (H3) antisense and sense riboprobes. A blinded correlative assessment was made between the labeling index of H3 and the clinical outcome. The quantitative tumor labeling index of the malignant ameloblastoma (HLI=0.329) was higher than the indices of the benign, locally aggressive ameloblastomas (HLI=0.049 to 0.111). This preliminary examination suggests that the basis of the rare metastatic behavior of ameloblastoma is possibly a reflection of an elevated cellular proliferation rate.

This report describes the inception, development and extensive use over 30 years of elliptical polarimeters at the University of Pennsylvania. The initial Mark I polarimeter design utilized oriented retarder plates and a calcite Foster-Clarke prism as the analyzer. The Mark I polarimeter was used on the Kitt Peak 0.9 m in 1969-70 to accomplish a survey of approximately 70 objects before the device was relocated to the 0.72 m reflector at the Flower and Cook Observatory. Successive generations of automation and improvements included the early-80’s optical redesign to utilize a photoelastic modulated wave plate and an Ithaco lock-in amplifier–the photoelastic modulating polarimeter. The final design in 2000 concluded with a fully remote operable device. The legacy of the polarimetric programs includes studies of close binaries, pulsating hot stars, and luminous late-type variables.
