      • Lack of Prognostic Value of Mean Corpuscular Volume with Capecitabine Therapy in Metastatic Breast Cancer

        Background: Capecitabine is an oral fluoropyrimidine derivative which is frequently used alone or in combination regimens for the treatment of metastatic breast cancer. Although overall and progression free survivals have increased in recent years with the use of new generation drugs, predictive factors that would further improve the outcomes are needed. Previous studies have demonstrated the relation between post-treatment increase in mean corpuscular volume (MCV) and predicting therapy response as well as survival. The present study investigated the clinical impact of MCV elevation in metastatic breast cancer patients treated with capecitabine. Materials and Methods: The data of a total of 82 patients from three centers followed between June 2005 and June 2013 were retrospectively analyzed. The demographic data and hormone receptor status of the patients, as well as initial examination before and after treatment and data concerning progression were recorded. MCV ${\geq}100$ fl was considered as macrocytosis. Capecitabine was given at a dose of $2500mg/m^2$ daily for 14 days every three weeks. Pre-treatment and post-treatment MCV and other parameters of complete blood count were recorded. Post-treatment initial evaluation was performed after 2 cycles of therapy. Results: The median age of the patients was 46.5 years (range 26-72 years) and 54% were premenopausal. Performance status was ECOG 0 and 1 in 81 (99%) patients. The median number of cycles for capecitabine therapy was 5 (min-max: 2-18). The median ${\Delta}MCV$ level (post-treatment values at sixth week - baseline) was 6.4. Whilst ${\Delta}MCV$ was ${\geq}6.4$ in 42 patients, it was <6.4 in 40 patients. Clinical benefit (complete response+partial response+stable disease) was observed in 37 (88%) of 42 patients with a median ${\Delta}MCV$ ${\geq}6.4$ and in 30 (75%) of 40 patients with ${\Delta}MCV$ <6.4 with no statistically significant difference (p=0.158). No significant difference was determined between the group with ${\Delta}MCV$ ${\geq}6.4$ and the group with ${\Delta}MCV$ <6.4 in terms of progression-free survival (11 vs 12 months) (p=0.55) and overall survival (20 months vs. 24 months) (p=0.11). Conclusions: The identification of new predictive markers in metastatic breast cancer is very important. In some recent studies, increase in MCV has been suggested as a marker in tumor response. In the present study, however, no significant difference was determined between tumor response and increase in MCV. Further studies including higher numbers of patients are needed to determine whether increase in MCV is a predictive marker or not.

        Subarachnoid Hemorrhage Presenting with Seizure due to Cerebrospinal Fluid Leakage after Spinal Surgery

        Cerebrospinal fluid leakage may commonly occur during spinal surgeries and it may cause dural tears. These tears may result in hemorrhage in the entire compartments of the brain. Most common site of such hemorrhages are the veins in the cerebellar region. We report a case of hemorrhage, mimicking aneurysmal subarachnoid hemorrhage due to a cerebrospinal fluid leakage following lumbar spinal surgery and discuss the possible mechanisms of action.

      • Clinicopathological Characteristics and Prognosis of Patients According to Recurrence Time After Curative Resection for Colorectal Cancer

        Purpose: To investigate clinicopathological features in patients with recurrent colorectal cancer within 1 year and more than 1 year after curative resection. Materials and Methods: We retrospectively evaluated 103 patients with disease recurrence before versus after 1 year of resection. Thirty-two patients (31%) were diagnosed with recurrence less than 1 year after curative resection for colorectal cancer (early recurrence) and 71 (69%) after more than 1 year (non-early recurrence). Results: The early recurrence group displayed a significantly lower overall survival rate for both colon cancer (p=0, 01) and rectal cancer (p<0.001). Inadequate lymph node dissection was a significant predictor for early relapse. There were no statistically significant differences in clinicopathological variables such as age, sex, primary tumor localization, stage, depth of invasion, lymphovascular invasion and perineural invasion between the early and non-early recurrence groups. However, a K-ras mutation subgroup was significantly associated with early recurrence (p<0.001). Conclusions: Poor survival is associated with early recurrence for patients undergoing resection for non-metastatic colorectal cancer, as well as K-ras mutation.

        Effects of Boron Supplementation to Diets Deficient in Calcium and Phosphorus on Performance with Some Serum, Bone and Fecal Characteristics of Broiler Chickens

        Three levels of boron (0, 30, 60 ppm) were supplemented in practical corn-soybean based starter and grower diets, containing either adequate or inadequate Ca or P. A total of 1,800, 1-day-old sexed broiler chicks were assigned to six dietary treatments and fed with the experimental diets for 42 days. Boron improved the overall feed conversion ratio, but increased body weight only at 21 days of age (p<0.01). Boron decreased feed intake in the case of feeding on a diet deficient in Ca and P, and tended to increase feed intake when birds received a diet adequate in Ca and P, signifying significant boron by Ca-P interaction (p<0.01). Mortality was not influenced by boron (p>0.05). Dietary Ca and P deprivation reduced body weight and feed consumption significantly, but did not influence the feed conversion ratio and mortality (p>0.05). Serum Ca level, ALP and ALT activities were not influenced either by dietary Ca and P deficiency or boron supplementation. Serum P content increased with respect to boron at 30 ppm. Bone breakage strength was not affected by dietary variables. Tibia ash, Ca and P were increased in response to the supplementation diet with 30 ppm boron, whereas 60 ppm showed no effect in most cases. Accordingly, the dietary boron supplementation of 30 ppm significantly decreased fecal Ca and P excretion, while there was a numerical decline in the 60 ppm boron as compared to the 0 ppm boron group. Data presented herein indicated that boron, either at the 30 ppm or 60 ppm supplementation level, was effective in conversion of feed to body weight, whereas only boron at 30 ppm contributed to the mineralization of bone thereby augmenting more Ca and P while excreting less through faeces.

        The Long Term Effects of Fairly Low-level of Supplemental Fat on the Productive Performance of Commercial Layers

        Laying hens were fed commercial diets added with supplemented fat (SFAT) at 0.6, 1.2, and 1.8% in order to study the long term dietary effects - on - their productive performance from 22 to 75 wk of age. Five hundred and seventy six Single Comb White Leghorn hens were assigned to one of the four dietary treatments. The experimental phase consisted of three periods of 18 wk each. The final body weight and gain of hens fed on diets with SFAT at 1.2% and 1.8% were lower (p<0.05) than those hens given no SFAT. The SFAT at the 1.2% and 1.8% levels improved egg production rate, egg weight and mass, as well as FCR. Mortality and feed consumption were not affected by dietary SFAT. Administration of a diet with SFAT significantly decreased the cracked-broken egg ratio (p<0.01). The beneficial effects of SFAT on egg production performance were particularly more pronounced at intermediate and later ages. Hence, SFAT by period interactions were significant for all traits studied except feed intake. Hens fed SFAT deposited significantly higher abdominal fat than those on the no-SFAT diet. As a result, SFAT at 1.2% and 1.8% inclusion levels provided benefits in terms of efficient table egg production.

      • Efficacy and Safety of Raltitrexed Combinations with Uracil-Tegafur or Mitomycin C as Salvage Treatment in Advanced Colorectal Cancer Patients: A Multicenter Study of Anatolian Society of Medical Oncology (ASMO)

        Background: There is no standard treatment for patients with colorectal cancer (CRC) progressing after irinotecan and oxaliplatin treatment. Here we aimed to retrospectively evaluate the efficacy and tolerability of raltitrexed in combination with oral 5-fluoropyrimidine (uracil tegafur-UFT) or mitomycin C as salvage therapy in mCRC patients. Materials and Methods: A total of 62 patients who had received raltitrexed combined with UFT or mitomycin C were identified between December 2008 and June 2013. They were given raltitrexed 2.6 $mg/m^2$ (max 5 mg) i.v. on day 1 in combination with either oral UFT 500 mg/day on days 1-14 every 3 weeks (group A) or mitomycin C 6 $mg/m^2$ i.v. on day every 3 weeks (group B). Results: Forty-two patients (67.7%) were in group A and 20 (32.2%) in group B. In 15 patients (24%) grade 3/4 toxicity was observed, resulting in dose reduction, and in 13 patients (20.9%) dose delay was necessary. The median progression free survival (PFS) was 3 months (95%CI 2.65-3.34) and median overall survival (OS) was 6 months (95%CI 2.09-9.90) in the whole group. Median PFS was 3 months (95%CI 2.60-3.39) in group A vs 3 months (95%CI 1.64-4.35) in group B (p=0.90). Median OS was 6 months (95%CI 2.47-9.53) in group A vs 12 months (95%CI 2.83-21.1) in group B (p=0.46). Conclusions: The combination of raltitrexed with UFT or mitomycin C seem to be a salvage therapy option due to safety profile and moderate clinical activity in heavily-pretreated mCRC patients.

        Monte Carlo approach for calculation of mass energy absorption coefficients of some amino acids

        This study offers a Monte Carlo alternative for computing mass energy absorption coefficients of any material through calculation of photon energy deposited per mass of the sample and the energy flux obtained inside a sample volume. This approach is applied in this study to evaluate mass energy absorption coefficients of some amino acids found in human body at twenty-eight different photon energies between 10 keV and 20 MeV. The simulations involved a pencil beam source modeled to emit a parallel beam of mono-energetic photons toward a 1 mean free path thick sample of rectangular parallelepiped geometry. All the components in the problem geometry were surrounded by a 100 cm vacuum sphere to avoid any interactions in materials other than the absorber itself. The results computed using the Monte Carlo radiation transport packages MCNP6.2 and GAMOS5.1 were checked against the theoretical values available from the tables of XMUDAT database. These comparisons indicate very good agreement and support the conclusion that Monte Carlo technique utilized in this fashion may be used as a computational tool for determining the mass energy absorption coefficients of any material whose data are not available in the literature.

        Schizophrenia-Like Psychosis and Dandy-Walker Variant Comorbidity: Case Report

        Dandy-Walker variant is a developmental malformation consisting of cerebellar hypoplasia and cystic dilatation of the fourth ventricle. Previous research has proposed a possible role for the cerebellum in cognition and in schizophrenia. In this paper we report a schizophrenia-like psychotic disorder in a 30 year-old woman with Dandy-Walker variant. The patient was treated with risperidone 6 mg/day, biperiden4 mg/day and risperidone depot 50 mg injections fortnightly, and most of the symptoms were ameliorated within 2 months. Thesimilar cognitive profile to populations with cerebellar pathology and rarity of the condition strongly suggests that there may be directrelationship between cerebellar pathology and appearence of psychotic symptoms.

        Tibial Base Plate for Total Knee Arthroplasty: Symmetric or Asymmetric?

        Background: Ideal positioning and best coverage of the tibial base plate are essential in total knee arthroplasty. There are 2 types of tibial base plates: symmetric and asymmetric. The superiority of one to the other is still controversial. The aim of this study was to compare symmetric and asymmetric tibial base plates for total knee arthroplasty in terms of rotational alignment and coverage. Methods: The study was conducted on a total of 80 cadaveric tibial bones. Two surgeons were asked to place 20 symmetric (group 1) and 20 asymmetric (group 2) tibial base plates taking care to ensure the best coverage that they were able to determine. Afterwards, the rotational errors and coverage were assessed with reference to the posterior tibial margin and posterior condylar axis on the three-dimensional computed tomography (3D CT) scan. In the second part of the study, the surgeons were asked to place 20 symmetric (group 3) and 20 asymmetric (group 4) base plates taking care to ensure the best rotational alignment. The rotational errors and the areas uncovered or overstuffed after the application were measured on the 3D CT scan. Results: On the comparison of rotational errors, while there was no significant difference between group 1 and group 2 in terms of coverage (p = 0.624), the mean external rotation error was significantly greater in group 2 (p = 0.034). On the comparison of coverage, while there was no significant difference between group 3 and group 4 in terms of rotation (p = 0.36), the mean ratios of the uncovered tibial surface to the total tibial surface (p = 0.041) and also the overstuffed area to the total base plate surface (p = 0.029) were significantly greater in group 4. Conclusions: The determination of correct size and rotation of the tibial component is essential for favorable outcomes of total knee arthroplasty. In this study, the symmetric tibial base plate design was more effective than the asymmetric design in providing the ideal tibial rotation and coverage.
