족근관은 내과와 종골을 연결하는 굴건 지대 아래 쪽에 존재하는 터널로 후경골건, 장족지 굴곡건, 후경골 동정맥, 후경골 신경 및 장무지 굴곡건이 지나가는 구조물이다. 족근관 증후군은 족근관 내에서 후경골 신경 및 신경 분지가 압박되어 발생하는 일종의 압박성신경 증후군이다. 족근관 증후군을 발생시키는 원인으로는 공간 점유 병소, 비후된 굴건 지대, 골극, 족근골 결합증, 정맥류 및 외상 등으로 알려져 있으며 족부 내측의 통증이 가장 흔한 증상이며 압박되는 신경 분지에 따라서 감각 저하나 이상 감각이 나타날 수 있다. 진단은 자세한 병력 청취 및 내과 아래의 압박 검사 등의 신체 검사를 통해 진단할 수 있으며 결절종, 지방종 및 신경종 등과 같은 공간 점유 병소가 있는 경우는 초음파나 자기공명영상이 도움을 준다. 신경 근전도 검사가 이환된 신경 분지를 구분하는 데 도움을 주지만 신경 근전도 검사가 음성이라고 하여 족근관 증후군을 배제할 수 없으므로 병력 청취와 신체 검사가 가장 중요한 진단의 기준이 된다. 치료로는 물리 치료, 야간 부목 고정술, 스테로이드 주사 요법 등의 보존적인 치료가 우선 권장되나 증상의 완화가 관찰되지 않을 때에는 수술적인 감압술이 추천된다. 수술적인 치료 결과는 다양하게 보고되고 있으나 젊은 환자에서 종물에 의해 발생한 경우 조기에 수술적인 처치를 시행하면 만족할만한 결과를 얻을 수가 있다고 알려져 있다. The tarsal tunnel is located beneath the flexor retinaculum, which connects the medial malleolus and calacaneus. The tarsal tunnel contains the posterior tibialis tendon, flexor digitorum longus tendon, posterior tibial artery and vein, posterior tibial nerve, and flexor halluces longus tendon. Tarsal tunnel syndrome is a compressive neuropathy of posterior tibial nerve and its branches under the flexor retinaculum. The etiologies of tarsal tunnel syndrome are space-occupying lesion, hypertrophied flexor retinaculum, osteophytes, tarsal coalition, varicose vein, and trauma. The symptoms are foot pain and hypoesthesia or paresthesia at dermatome according to involving nerve branches. Clinical diagnosis can be obtained from a detailed history and physical examination such as compressive test at the tarsal tunnel area. Ultrasonography and magnetic resonance imaging can reveal the space-occupying lesion, such as ganglion, lipoma, and neuroma. The initial treatments of tarsal tunnel syndrome are conservative management, such as physical therapy, night splint, and steroid injection. Surgical decompression is indicated after failure of conservative managements. Variable results of surgical treatment have been reported. Favorable result after decompression could be obtained from young patients, early onset symptoms, and space-occupying lesion.

이 연구의 목적은 농업기술센터 교육담당자들이 농업인 교육프로그램,특히 농업인대학 교육과정을 개설하여 운영하는데 있어 현장에서 활용할 수 있는 농업인대학 교육과정 개발·운영 매뉴얼을 개발하는데 있었다. 농업인대학 운영 매뉴얼은 현재 운영되고 있는 농업인대학의 운영 실태를 분석하여 농엽인 대학을 설립하여 운영하고자 하는 실무자가 현장에서 필요로 하는 내용이 무엇인지를 도출하였고,이러한 내용을 체계적으로 단계화 하였으며,최종적으로는 각 단계에 대하여 구체적으로 안을 제시하는 절차에 따라 개발되었다. 이를 위해 이 연구에서는 농업인대학 운영과 관련한 국내·외에서 발행한 각종 연구보고서와 학술지 논문 분석을 포함한 문헌고찰,농업인대학 교육과정을 운영하고 있는 7개 농업기술 센터에 대한 현장조사 및 인터뷰,농촌지도사 빛 농촌진흥청 지도관과의 전문가협의회,일선 농업기술 센터 농업인 교육 담당자와의 2차에 걸친 관계자 간담회를 실시하였다. 이 연구의 결과는 다음과 같다. 농업기술센터의 농업인대학 운영을 위한 매뉴얼을 개발하기 위하여,기존에 존재하던 교육과정 개발 모형을 다각도로 검토하여 6단계의 모형을 도출하였다. 이는 일반적인 교육과정 개발 모형에 농업인대학의 운영에 있어 중요하다고 판단되는 인프라 구축 단계와 사후관리 단계를 추가한 것이다. 이에 따른 농업인대학 운영 매뉴열의 6단계를 1)교육요구 분석,2)인프라 구축,3)교육과정 개설,4)교육과정 운영,5)교육과정 평가,6)사후관리로 설정하였다. 각각의 단계는 다시 4∼8가지의 하위 단계로 구성되며,이에 따라 매뉴얼은 총 38단계로 구성되었다. The purpose of this Study was to develop the manual for farmers' Academy program development and implementation in Agricultural Extension Services. For this purpose, we analysed current status of 47 Farmer's Academy, investigated the contents needed in the field, formulated the contents to make process, and described each process. The manual was developed through a literature review, a content analysis of farmers' Academy Program, a expert conference, and an informal gathering. Expert conferences were held to discuss the direction of manual development, and validation of manual framework and content elements, Informal gatherings were held two times to discuss application of manual in fields and to supplement contents. The result was as follows. Both infra construction phrase and post service phrase were required for farmers' academy program development and implementation in Agricultural Extension Services. The two phrases was added to ISD model basically used for development of curriculum. The framework of Manual is consist of six phrases such as educational needs analysis, infra construction, development of curriculum, implementation of curriculum, evaluation of curriculum, and post service. Each process is organized with 4~8 subordinate phrases as well. Ultimately, the manual was made of detailed 38 subordinate phrase. Based on the results, some recommendations were suggested.
