Bipolar endoprosthesis의 양극간의 Motion에 관한 분석

The motion of two components in bipolar endoprosthesis has been argued in several reports, dependant on types of cup, and condition of hips until now. The authors reviewed 28 hips of bipolar endoprosthesis ultilizing Bicentric cup without positive eccentiricity (A group) and 22 hips of that utiliring UHR and self centering cup with positive eccentricity (B group). Also analyzed motion and its amount of two components in radiologic extreme adduction and abduction view. The follow up period ranged from 3 years to 5 years with an average of 4 year in a group nd from l year to 3 years and 3 months with an average 1 year and 10months in B group. The following results were obtained, 1) In A group, there was both bearing motion in 14 cases (50%), outer bearing in 9 cases (32%) and inner bearing in 5 cases (18%). In B group, there was both bearing motion in all cases except one patient who had suffered from infection. The case of both bearing motion in B group was more than in A group(p<0.05). 2) In cases with both bearing motion in A group and B group, total amount of motion was larger in B group than in A group(p<0.05). 3) In B group with both bearing motion, the amount of motion was larger in cases with 32mm femoral head than in those with 22mm head (p<0.05). 4) In B group with both bearing motion, the amount of outer bearing motion was larger than that of inner bearing motion in all cases of femur neck fracture(p<0.05) 5) Cup discrepancy or leg length discrepancy had no relationship to motion. 6) Average Harris hip score was 87.1 in A group and 94.1 in B group. The hip score of B group was higher than that of A group(p<0.05). Conclusively, Bipolar cup with positive eccentricity is better than that of egative eccentricity in total amount of motion, both bearing character, and hip function score.
